What's New for 2022!

ImpaxRx


ImpaxRx is a concierge pharmacy program specifically targeting high-cost drugs by taking advantage of alternate distribution programs. Income-eligible individuals will qualify to receive their prescriptions at no cost.

Lake County will be implementing ImpaxRx for all regional entities.

Benefits Overview

Medical Mutual: will continue as you Medical carrier

  1. EPO Plan: 100% coinsurance with no deductible. (Benefit Change- $250 copay)
  2. Lake Blue POS Plan


Caremark: will continue as your Prescription Drug carrier


Guardian: will continue as your Basic Life, Voluntary Life, Dental and Buy-Up Vision carrier


Eye-Med: will continue as your Vision Exam Only carrier

Enroll or Qualifying Event Change

Full-Time Employees

*Employees are eligible 1st of the month following date of hire. If hired on the 1st, employee is eligible the following month.


EMPLOYEE ELIGIBILITY: If you do not enroll during this Open Enrollment period, you must wait until the next Open Enrollment period unless you experience a Qualifying Life Event. If you experience a Qualifying Life Event (i.e., birth or adoption of a child, marriage, divorce, loss of other coverage, etc.) you must enroll within 30 days of the Qualifying Event.


DEPENDENT ELIGIBILITY: You may also enroll eligible dependents for benefits coverage. A ‘dependent’ is defined as the legal spouse and/or ‘dependent child(ren)’ of the plan participant or the spouse.

  • Your Legal Spouse: If your legal spouse has coverage at his/her employer, they are not eligible for the City of Kirtland per the spousal waiver policy. You must submit a copy of your marriage license, if you have submitted documentation previously, you do not need to resubmit.
  • Your Dependent Children: Children are eligible up to the end of the month of 26th birthday regardless of marital or student status. You must submit a copy of each child's birth certificate, if you have submitted documentation previously, you do not need to resubmit.


What is a Qualifying Life Event?

• Marriage

• Divorce

• Birth/Adoption of a child

• Death of a spouse or other enrolled dependent

• Change in spouse’s benefits or employment status

• A dependent becomes eligible for Medicare or Medicaid


If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.

All team members have access to your online benefits enrollment platform 24/7 where you have the ability to enroll, select or change your benefits online during the annual open enrollment period, new hire orientation, and for qualifying events.

This is an active enrollment!

This means that you must log in to BenXpress to enroll yourself and your dependents for benefits. All employees will use BenXpress to confirm or change benefit elections.

Medical

Medical Mutual

You must utilize providers that are in MMO’s network to receive benefits. Employees who seek care from a provider that is not in MMO’s network, will be responsible for 100% of the costs incurred. 


Your prescription drug coverage is through Caremark. You can receive generic medications for hypertension, diabetes, cholesterol and asthma at $0 copay. If you are on a maintenance or long term medication, then you must fill these prescriptions through mail order. You can receive a 90 day supply at any CVS Pharmacy.

Tobacco Cessation Program

In order to improve the health and well-being of our employees, while also maintaining a high level of coverage at an affordable rate, we have implemented a Wellness Program for Tobacco Cessation.


  • Each employee has access to Medical Mutual's QuitLine.
  • QuitLine helps employees decide which medications may be best suited for ending tobacco use.
  • The City of Kirtland pays for two cycles of tobacco cessation medication under your prescription drug plan. 
  • Proof of obtaining the cessation drugs or completing the reasonable alternative solution program is necessary to avoid paying the $20/$40 per month if you are a tobacco user (employee or spouse). 


With this program all employees covered under the medical plan who use tobacco will be charged an additional $20 per month in their health insurance premium. Additionally, any eligible spouses covered under the plan who use tobacco will also be charged an additional $20 per month.


RANDOM testing will occur:

  • A positive test resulting from any tobacco cessation medication (gum or patches included) will not cause an employee to be responsible for the additional fee per month.
  • employees and spouses will be randomly generated each quarter to be tested for nicotine
  • A failed test, unless noted above, will result in immediate change to tobacco user status, and that employee and/or spouse will be back charged $20 per month penalty to the beginning of that calendar year.


**If your tobacco status changes throughout the year, you must immediately notify HR.

 

Dental

Guardian remains as the carrier for your dental benefits.


You may go to any dentist, however those who belong to Dental - DentalGuard Pref - Ohio network will be most cost effective.

You may go to any eye care provider, however those who belong to the Guardian Vision - Full Feature network will be most cost effective.

Basic & Voluntary Life

Guardian remains as the carrier for your Basic and Voluntary Life benefits.

  • To individual life policy when coverage ends under the plan

Set up your account now so its ready when you need it!

Complete patient registration for medical history, pharmacy preference, primary care physician contact information and insurance information.

Each time you visit you will be asked brief medical questions. If appropriate you will be connected to a doctor using secure live audio and video.

You and your doctor will discuss your medical issue and if appropriate your doctor may write a prescription.
