Welcome

Helping you and your family reach and maintain good health is very important to City Wide Insulation. City Wide Insulation is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by City Wide Insulation. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.


New Hires/Enrollment

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances called "Qualifying Events."

QUALIFYING LIFE EVENTS

You can change your benefit election during the year if you have a qualifying life event. Qualifying life events include, but are not limited to:

  • Marriage, divorce or legal separation
  • Birth or adoption of a child
  • Change in child’s dependent status
  • Death of a spouse, child or other qualified dependent
  • Change in employment status or a change in coverage under another employer-sponsored plan

If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.

Medical

Medica

Medica will administer your medical and prescription drug benefits this year.


All options have been carefully selected to provide the most value to you and your families. City Wide Insulation is offering the $5,000-0% HRA Plan with the Passport and Elect network options. Medica Choice Passport gives you access to a large, national network. The Elect network is a mid-sized regional network and requires you to enroll in a primary care clinic. Choose your own primary care clinic (PCC) – each family member on your plan can select their own too.

FSA

Flexible Spending Accounts - Health Equity 

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.

Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.

You may contribute as follows:

Traditional Health Care FSA

  • Up to $2,500 annually
  • All benefit eligible can participate, unless you or your spouse are contributing to an HSA.
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses

Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns

How the Plan Works

You must incur your eligible expenses during the plan year — March 1 to February 28. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until May 31st annually to file your claims. Any funds after May 31st unclaimed will be forfeited.

HRA

Health Reimbursement Account - Health Equity

City Wide has set up a Health Reimbursement Arrangement (HRA) for you, it is a tax-advantaged account that allows you as the employee to save on the cost of healthcare. With an HRA, City Wide has set aside a specific amount of pretax dollars for you to pay for healthcare expenses. You pay 50% of your claims on an annual basis and City Wide will pay for 50% of your claims.

Dental

Principal

Your dental health is an important part of your overall health. Make sure you take advantage of your preventive dental visits. Preventive care services are covered at 100% if you visit an In-Network provider.

In addition to protecting your smile, dental insurance helps pay for dental care and usually includes regular checkups, cleanings and X-rays. Several studies suggest that oral diseases, such as periodontitis (gum disease), can affect other areas of your body—including your heart. Receiving regular dental care can protect you and your family from the high cost of dental disease and surgery.

City Wide Insulation's dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Principal network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher.

  • 80% of learning in the first 12 years comes through the eyes

City Wide Insulation offers vision coverage through Principal to help pay for prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the Principal Network.

Life & Disability

Principal

City Wide Insulation provides you with Basic Life/AD&D coverage as well as Voluntary Short-Term Disability through Principal.


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for $10,000 coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Short-Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through Principal 66.67% of your income to a maximum of $350 per week.
