We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. All Benefits Eligible associates working at least 30 hours per week are eligible to participate in the benefit plans offered by Mike Maroone Automotive. Newly hired associates will be eligible the 1st of the month following 30 days of employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children are eligible for benefits as follows:
•Medical, Dental and Vision: Until the last day of the year in which the child turns 26
•Child Life Insurance & AD&D: Up to age 26
ENROLLMENT INSTRUCTIONS:
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Cigna will continue to be your medical and prescription carrier. You have the choice between three plans for the 2024 plan year. The three plan options include two PPO Copay plans and an QHDHP plan. View the Providers & Networks for more information on how these plans networks differ from each other as well as buttons to locate participating providers.
If you are already enrolled in one of the MMA Cigna medical plans today and have not already done so, please be sure to register through the Cigna member portal where you can access important information pertaining to your medical plans with Cigna including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the Cigna portal as of your effective date of coverage. Information on how to register can be found www.mycigna.com.

You can go online to see the most up-to-date list of medications your plan covers. Go to myCigna App or myCigna.com, click on the Find Care & Costs tab, select Price a Medication, and type in your medication name.
Use Cigna.com/PDL to access your Cigna Advantage 4-tier prescription drug list (all specialty medications covered on tier 4. You can also determine whether there may be lower cost alternatives for your prescriptions.

MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. ASK YOUR DOCTOR OR PHARMACIST – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.

Mike Maroone Automotive now provides a competitive benefits package for all associates as part of our total compensation package. We realize you make benefit choices based on what’s important to you and the needs of your changing lifestyle. Therefore, along with our core benefits, we offer these additional benefits which help to provide financial support to you. These benefits are paid for through payroll deduction on a post-tax basis so that the benefits are tax free.
Accident, Critical Illness, & Hospital coverage is provided through Cigna. Hopefully you are never in the position to use it, but if you were to experience an accident, illness, or hospitalization, it could end up costing you a lot of money. With these plans, you will be paid money for qualifying situations/services to cover out of pocket expenses or you may use the funds for whatever you like.
Key Features to Consider:
› Flexible. Use the money however you want. Pay for anything, including medical deductibles, child care, groceries, etc. It’s up to you.
› Supplement your medical plan. Benefits are paid in addition to other coverage you may have.

For the 2024 plan year, Direct Primary Care (DPC) services will be offered through Everside with enhanced member technology resources and access.

You have the opportunity to contribute to a variety of savings and/or spending accounts on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by Mike Maroone Automotive.
The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. The Bronze Medical Plan is qualified for an HSA. By contributing to an HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and any employer provided funds to the HSA. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.
$4,150 for a single contract | $8,300 for a two person or family
$1,000 "Catch Up" contribution for those age 55+

Mike Maroone's dental plans provide comprehensive coverage to help you and your family maintain good dental health. The dental PPO plan includes both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in one of the Mike Maroone Automotive's plans today and have not already done so, please be sure to register through the Ameritas (DPPO) or Cigna (DHMO) member portal where you can access important information pertaining to your dental plan with Ameritas or Cigna including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the portals as of your effective date of coverage.
You have the choice of two dental plans - The DPPO Plan through Ameritas includes a deductible (for basic and major services only), ortho coverage, cost sharing for your basic and major services, and a $2,000 annual maximum - OR - The DHMO Plan through Cigna which is an in-network only plan with a fee schedule that requires you to designate a Cigna General DHMO Dentist.

Ameritas offers one of the nation's largest dental networks. Plus, you will not be balance billed if you see an Ameritas Classic PPO dentist - giving them added savings. If you do choose to visit a nonparticipating dentist you may be balance billed and have to pay more.
The Cigna Dental Care (DHMO) plan includes coverage for many procedures that may not be available on other dental plans. There are no deductibles to pay before coverage begins, no annual dollar maximums on covered services, no claim forms, and no waiting periods.

Ameritas offers vision coverage through VSP to help pay for eye exams, prescription glasses and contact lenses. Ameritas also provides coverage for Lasik.
If you are already enrolled in the Mike Maroone Automotive's vision plan today and have not already done so, please be sure to register through the Ameritas member portal where you can access important information pertaining to your vision plan with Ameritas including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Ameritas portal as of your effective date of coverage.
Your vision network with Ameritas is the VSP network. You may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services out-of-network, you may be required to pay that provider at time of service and will need to submit a claim to VSP for the applicable reimbursement based on the out-of-network reimbursement schedule.

Mike Maroone Automotive understands the importance to you and your family to have strong financial protection. We provide all benefit eligible associates Basic Life/AD&D at no cost to you. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, associate paid basis so you can select what is best for your family. All of these coverages are provided through UNUM to protect your income and out of pocket expenses.
A sudden accident or death can leave you or your loved ones in a vulnerable position. Mike Maroone Automotive provides all benefit eligible associates Basic Life and Accidental Death & Dismemberment (AD&D) coverage in the amount of $35,000 at no cost to you. Associates have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in associate coverage, this will be in addition to your employer provided Basic Life coverage.
Everyday illnesses or injuries can interfere with your ability to work, more serious issues can impact your ability to work indefinitely. Even a few weeks away from work can make it difficult to manage household costs. Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
