CeriFi is committed to providing our employees with a benefits program that is both comprehensive and competitive. Our benefits program offers health care, dental and vision coverage, as well as financial security to our employees and their families. This guide provides a general overview of your benefit choices and enrollment information to help you select the coverage that is right for you.
If you are a full-time employee working 30 hours or more per
week, the chart below lists the benefits you may be eligible for
after meeting each plan’s eligibility requirements.
DEPENDENT ELIGIBILITY
You can enroll your dependents in plans that offer dependent coverage. Eligible dependents are defined as your legal spouse or domestic partner and eligible children who reside in your household and depend primarily on you for support. This includes: your own children, legally adopted children, stepchildren, a child for whom you have been appointed legal guardian, and/or a child for whom the court has issued a Qualified Medical Child Support Order (QMCSO) requiring you or your spouse or domestic partner to provide coverage.
MEDICAL, DENTAL, AND VISION PLAN
DEPENDENT COVERAGE
You may cover your eligible dependent children up to age 26, regardless of marital or student status (this does not include spouses or domestic partners of adult children). Dependent coverage will cease for your covered dependent children at the end of the month in which an eligible dependent reaches age 26.
DOMESTIC PARTNER COVERAGE
Domestic partners are eligible to enroll as dependents in the benefit plans. You and your partner must meet specific criteria to qualify for domestic partner coverage. A domestic partnership
is different than a marriage with an individual of the same-sex. A same-sex spouse is a federal tax dependent for group health plan purposes; whereas, a domestic partner often is not. If you cover a domestic partner, a domestic partner’s child or another person who is not considered an IRS tax dependent for group health plan purposes, CeriFi is required to report income for you that reflects the value of coverage for tax-reporting purposes. This is known as imputed income. You will receive a W-2 annually for the value of coverage for any dependent who is not an IRS tax dependent.
New Employees
As a new employee of CeriFi, you become eligible for benefits on the first of the month following date of hire. Our benefits plan year runs from January through December.
Open Enrollment
As a benefits-eligible employee, you have the opportunity to enroll in or make changes to your benefit plans during our annual open enrollment period. Open Enrollment is usually held in November with benefit elections effective January.

CeriFi offers three medical plan options for full-time employees. All options provide high-quality, affordable medical care, including preventive care, doctor's visits, hospitalization, emergency care and prescription drug coverage. However, each plan has unique characteristics and advantages. The medical plans offered to CeriFi employees utilizes the Open Access Plus Network which is an open access provider network through Cigna.

Once enrolled with Cigna Healthcare you can create a MyCigna account to utilize all of the features that can be found in the MyCigna app or website. To navigate to the MyCigna Healthcare login page use the MyCigna Login button below. For new users you will need to create an account with a username and password.

NOTE: Your medical plan options must offer certain preventive care benefits to you in-network without cost sharing and these preventive care benefits generally are updated annually. Under the Affordable Care Act, the medical plans generally may use reasonable medical management techniques to determine frequency, method, treatment or setting for a recommended preventive care service. CeriFi will be making a generous contribution to medical coverage for employees and their dependents. Any deductibles and copays shown in the chart below are amounts for which you are responsible.
To search for your medication tier, click on the "Medication Finder" button. Once the page is opened, select "National Preferred 4 Tier Specialty" and type your medication in the search box. Alternatively, you are able to search by first letter by selecting "View A-Z/0-9 Drug List" to the right of the search box.

CeriFi believes strongly in improving our employee's overall wellness. With your Cigna Healthcare coverage you have access to many different wellness support resources and can earn rewards earning up to $100 for completing different wellness activities. If you enroll in the Cigna HSA Open Access plan, you’ll have access to an HSA through Optum Bank. You can think of your HSA as a personal savings account for your health care expenses, with some impressive tax advantages. Whatever you don’t use rolls over for future years and in some circumstances may be invested. CeriFi will contribute up to $2,000 a year provided you complete a qualified wellness activity prior to July 1st.
Health Savings Accounts (HSAs) are a great way to save money and budget for qualified medical expenses. HSAs are tax-advantaged savings accounts that accompany the Cigna HSA Open Access Plus Plan.
WHAT ARE THE BENEFITS OF AN HSA?
There are many benefits of using an HSA, including the following:
Additionally, if you are age 55 or older, you may make an additional “catch-up” contribution of $1,000. You may change your contribution amount at any time throughout the year as long as you don’t exceed the annual maximum.

FSAs help you save money by allowing you to pay for certain types of health care and dependent care expenses on a pre-tax basis. You decide how much money to put aside each payday to cover these expenses up to the maximum. This amount is then deducted from your pay before taxes and deposited into your FSA. When you need money to cover an eligible expense, you can get reimbursed using a variety of reimbursement methods. Remember to always keep your receipts.
NOTE: Your maximum contribution to the Health Care Spending Account will be limited to $3,300. The maximum for the Dependent Care Spending Account is $5,000.
IMPORTANT: USE IT OR LOSE IT!
According to IRS rules, any money remaining in a Health Care or Dependent Care Spending Account after the deadline for filing claims will be forfeited. If you have money left in your Health Care FSA at the end of 2024, you may carry over up to $660 for use in 2025. The money you carry over doesn’t count against the IRS annual contribution maximum, which means you can start the year with an amount greater than the IRS limit in your Health Care FSA. You can use the amount throughout the 2025 plan year. This rule applies each subsequent calendar year. This does not apply to the Dependent Care FSA.

CeriFi offers dental coverage through a plan administered by Cigna.
In addition to protecting your smile, dental insurance helps pay for dental care and usually includes regular checkups, cleanings and X-rays. Several studies suggest that oral diseases, such as periodontitis (gum disease), can affect other areas of your body—including your heart. Receiving regular dental care can protect you and your family from the high cost of dental disease and surgery.

CeriFi offers employees vision coverage through EyeMed that includes coverage for eye exams and eyeglasses or contact lenses.

ACCIDENT INSURANCE
Major injuries are painful. But the financial impact of the medical treatment doesn’t have to be. Accident insurance pays benefits directly to you if you suffer a covered injury such as a fracture, burn, ligament damage, or concussion. Benefits are paid even if you have other coverage. The benefit amount is calculated based on the type of injury, its severity, and the medical services required in treatment and recovery. The plan covers a wide variety of injuries and accident-related expenses, including:
• Injury treatment (fractures, dislocations,
concussions, burns, lacerations, etc.)
• Hospitalization
• Physical therapy
• Emergency room treatment
• Transportation

CRITICAL ILLNESS INSURANCE
You can protect yourself from the unexpected costs of a serious illness. Even the most generous medical plan does not cover all of the expenses of a serious medical condition like a heart attack or cancer. Critical illness insurance pays a full lump sum benefit directly to you if you are diagnosed with a covered illness that meets the plan criteria. The benefit is paid in addition to any other insurance coverage you may have.

• End stage renal (kidney) failure
• Coronary artery bypass surgery*
• COVID-19*
Plan Features
• You do not have to be terminally ill to receive benefits.
• Coverage options are available for your spouse/domestic partner and children as riders to your coverage.**
• Coverage is portable — you can take your policy with you if you change jobs or retire. The cost of the benefit will vary depending upon factors such as your age, whether you use tobacco, and the dependent coverage you choose.
*The coverage pays 25% of the face amount of the policy once per lifetime for coronary bypass surgery and COVID-19.

HOSPITAL INDEMNITY INSURANCE
Receive payments to help cover the cost of a hospital stay. If you are admitted into a hospital, it doesn’t take long for the out-of-pocket costs to add up. Hospital indemnity insurance pays benefits directly to you if you are admitted into a hospital for care or childbirth. Benefits are paid even if you have other coverage. You receive a benefit as soon as you are admitted and then an additional benefit based on the number of days you are confined to the hospital. The benefit increases if you are admitted and confined to an intensive care unit or inpatient rehabilitation.

BASIC LIFE AND AD&D
CeriFi provides eligible employees with $20,000 in basic life accidental death and dismemberment (AD&D) insurance administered by Mutual of Omaha.
CeriFi pays for the full cost of this benefit—meaning you are not responsible for paying any monthly premiums.
VOLUNTARY LIFE AND AD&D
While CeriFi offers basic life insurance, you have the option to purchase additional life insurance coverage for you, your spouse, and your child(ren). With voluntary life and accidental death and dismemberment (AD&D) insurance through Mutual of Omaha, you are responsible for paying the full cost of coverage through payroll deductions.
You must elect coverage for yourself in order to elect coverage for your spouse and dependent children.

Disability benefits replace a portion of your pay if a non-work-related illness or injury prevents you from doing your job for a long period of time.
SHORT TERM DISABILITY (STD)
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Benefits begin on the 22nd day of an injury or accident and are equal to 60% of covered monthly earnings up to a maximum of $2,500 per week.
LONG TERM DISABILITY (LTD)
At CeriFi, we want to do everything we can to protect you and your family. That’s why CeriFi pays for the full cost of long term disability insurance. Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long term disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 90-day elimination period are equal to 60% of covered monthly earnings up to $10,000 for full-time employees, and will continue for the length of your disability or until you reach retirement age.

The CeriFi 401(k) retirement plan is a powerful way to build your retirement savings. All employees who are at least 21 years old and have completed 6 months of service with CeriFi is eligible in our 401 (k) Plan. Enrollment is measured from your date of hire, provided that you are an eligible employee at the end of that period. CeriFi will contribute a matching contribution to your Safe Harbor Matching Contribution Account in an amount equal to 100% of the Matched Employee Contributions that are not in excess of 4% of your Plan Compensation. Matching contributions will be allocated to the Safe Harbor Matching Contribution Accounts of Participants as soon as administratively feasible after the end of the Plan Year.
