We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
Employees working at least 30 hours per week are eligible to participate in benefits offered by Ceeva, Inc. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

ENROLLMENT
If you are newly hired, you are eligible for benefits on your 90th day of employment. You must complete the appropriate enrollment forms in order to be set up with the coverages. Once completed, please submit them to your employer to sign and to process.

- Guardian - Dental Coverage
You can modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.
MEDICAL/RX
UPMC HealthPlan will continue to administer your medical and prescription drug benefits this year.
The plan requires you select a Primary Care Physician.

If you reside outside of the counties highlighted in deep purple on the attached map, you will need to enroll with Ceeva Inc.'s National Plan.

Health Care FSA - for 2025 is $3,300 Max Contribution
- Use Pre-tax monies to cover your out-of-pocket costs for qualified medical, dental, vision and OTC expenses for yourself, spouse and/or dependent child(ren).
- Full FSA election amount available Day 1
- At the end of the plan year, you may roll over up to $660 of unused funds.
Dependent Care FSA - for 2025 is $5,000 Family Max Contribution
- Use pre-tax monies to cover day-care expenses for qualified dependents such as pre-school programs, Latch Key programs through age 13, Daycare Centers for Adults
- Only have access to the current account balance
DENTAL
Ceeva Inc.'s dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the National Guardian network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your out of pocket costs will be higher.

VISION
DID YOU KNOW?
- Eye exams can help detect serious eye and general health conditions sooner: high blood pressure, diabetes, heart disease, high cholesterol
- Babies should receive their first professional eye exam at 6 months
- 80% of learning in the first 12 years comes through the eyes.
Ceeva, Inc. offers Vision coverage through UPMC HP, who partners with National Vision Administrators to offer a National Network of providers. To find out if your vision provider participates in the network, please visit the UPMC Health Plan website below.
