There are two steps to enrolling:
All employees should complete their election through Employee Navigator by Monday, November 28th. You will receive an email from Employee Navigator on Monday, 11/14/2022 inviting you to log on and enroll.
Anyone enrolling with Sedera for the first time must complete their online enrollment with Sedera through the portal by Monday, November 21st, to ensure they will have a December 1st effective date.
Helpful Tips To Consider Before You Enroll
Do you plan to enroll an eligible dependent(s)?
If so, make sure to have their social security numbers and birthdates available. You cannot enroll your dependent(s) without this information.
Have you recently been married/divorced or had a baby?
If so, remember to add or remove any dependent(s) and/or update your beneficiary designation.
Did any of your covered children reach their 26th birthday this year?
If so, they may no longer be eligible for benefits, unless they meet specific criteria.
Christian Temple Church offers two options for your medical needs. The first is a fully insured medical plan through BlueCross Blue Shield. The second option is a cost sharing plan through Sedera combined with a MEC (Minimum Essential Coverage) plan for your preventative care and an accident plan through Colonial. All medical options will cover your preventative care at 100%.
There are two dental plan options and a vision plan offered through Dental Select.
Each employee will recieve $675 to use towards their medical, dental, and vision benefits.

This is the part of your combo that covers your preventative care at 100%. When you enroll in Sedera you are automatically enrolled in this plan.
You will receive an ID card for this plan.

●Click on the green Select Network button & choose PHCS
●Click the green Select Network box
●Choose PHCS in the pop-up box
●Select Preventive Services Only

Christian Temple Church’s dental plans provide comprehensive coverage to help you and your family maintain good dental health.
You have two plan options to choose from. Both plans cover preventative care at 100% (in network).
Option 1: PPO
This plan uses the Dental Select Platinum Co-Insurance Network. The plan allows you the freedom to use providers in and out-of-network. If you utilize services from an out of network provider, you may be balance billed.
Option 2: Co-Pay
This plan uses the Platinum (Co-Pay) network. This plan has set copays for services. You are encouraged to stay in network on this plan.

Christian Temple Church offers voluntary vision coverage through Dental Select’s Eyemed network to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the Ameritas Network. See summary below.
