Introduction

Open Enrollment is your annual opportunity to enroll in, make changes to, or waive benefit coverage for you and your family!


Making changes to existing elections during the year (outside of the Open Enrollment window) will require a qualifying event.

How To Enroll

NO ACTION, NO COVERAGE!


Please note the 2024 Open Enrollment is an ACTIVE enrollment, which means you need to actively elect the coverage you want and need to ensure you have benefits effective 7/1/2024.


For CSB/SJU ONLY - All employees are required to log into the NEW Employee Navigator system to make your elections. This is to ensure you and your dependents have the benefits you want and need for the next plan year. You will receive an email directly from Employee Navigator with log-in instructions. You may also click here taken to the Employee Navigator log-in page. If you haven't already registered, you will need to do so prior to accessing the system. Simply click on "Register as a new user" from the login page to get started!


For OSB ONLY - All employees must log into ADP to make your benefit elections. Please use your existing log-in credentials.

Medical

Provider: HealthPartners


Effective July 1, 2024, your medical benefits are offered through HealthPartners!


You have the choice of three (3) medical plans -- two traditional options and one high deductible health plan option. All three plans come with the Open Access network which is HealthPartners' broadest network with the widest nationwide access.


Refer to your 2024-2025 Benefit Resource Guide for additional information on the plans and networks.


As a HealthPartners member, you have additional resources available to assist in your overall well-being. Please refer to your HealthPartners resource guide for information regarding how to best utilize your medical plan and the health and wellbeing resources available to you.


To personalize your experience as a HealthPartners plan member, we encourage you to set up your member profile on the HealthPartners website (you will be able to do this after 7/1/2024).

Kavira

If you want the convenience of receiving everyday healthcare through low to no cost telehealth and home visits, Kavira is right for you. The goal of this offering is to save you time and money when addressing preventive needs and common acute illnesses within the comfort of your own home (virtually or in-person).


Kavira is available for all CSB/SJU/OSB medical plan members and their dependents.


NOTE: Dependents do not need to be on the medical plan to be covered as long as the employee is enrolled on the medical plan

This is an enhancement to your medical plan offering (however, entirely separate from HealthPartners).


You start with a virtual visit via text message, video on the Kavira mobile app, or on-line on your computer. Your care can be completed virtually; however, you can request an in-home visit as well, if desired!


Kavira is offered at no charge to you and visits are free to those on Core Plans A & B and only cost a $5 copay for those on HDHP (only for a non-preventive home visit) which is needed in order to remain compliant with IRS-related HSA regulations.

Dental

Provider: Delta Dental of MN


Your dental insurance is offered through Delta Dental. You have two networks to choose from -- Delta Dental PPO and Delta Dental Premier. Both networks offer discounted fees, but the greatest savings is available when you see a Delta Dental PPO dentist.

 

The annual benefit maximum is $1,500 per person and diagnostic and preventive services are covered at 100%. For other services, you share the cost as a percentage after you have met the calendar year deductible of $50 per person in-network. Orthodontia coverage is available for children between ages 8 and 18 with a $1,000 lifetime maximum benefit.

We strongly encourage you visit the Delta Dental of MN website and set up your member profile on Delta's secure member portal. 

Vision

Provider: EyeMed


The vision plan is 100% employee paid and is a comprehensive benefit meaning it covers materials (lenses and frames) in addition to an eye exam.


As a reminder, a preventive eye exam is also covered under your medical plan for children ages 6 and under.

HSA & FSA

Provider: Further


If you are enrolled in the qualified high deductible health plan (HDHP), you are eligible to contribute to a Health Savings Account (HSA). An HSA is a tax-advantaged savings account that you can use for medical, dental, vision, and hearing expenses. It is paired with a qualifying health insurance plan which offers lower monthly premiums in exchange for a higher deductible (the amount you pay out of pocket before insurance kicks in).

 

Your HSA is administered by Further by HealthEquity. For 2024, you can contribute up to $4,150 if you elect Employee Only coverage or up to $8,300 if you elect any other coverage level. In addition, if you are over age 55, you can make an additional $1,000 catch-up contribution. You can start, stop, or change your contribution at any time.

 

CSB/SJU/OSB also offers two Flexible Spending Account (FSA) options:

  • Healthcare FSA -- for employees enrolled in Core Plan A or B or employees not enrolled in CSB/SJU/OSB's medical plan. The Healthcare FSA allows you to set aside money on a pre-tax basis that can be used to cover out-of-pocket expenses for medical, dental, vision, and hearing costs that are not covered by the plan. You can contribute up to $3,200 for 2024.
  • Dependent Care FSA used to set aside pre-tax dollars to cover day-care/elder-care expenses for eligible dependents. You can contribute up to $5,000 for 2023 ($2,500 if married and filing separately).


You cannot change your FSA contribution amount during the year unless you experience a qualifying status change.

Life and AD&D

Provider: The Standard


As a CSB/SJU/OSB employee, you are provided with Basic Term Life and AD&D coverage at NO cost to you.


You are also eligible for additional Life insurance on a voluntary (100% employee paid) basis.
