WELCOME

Helping you and your family reach and maintain good health is very important to CRDN. CRDN is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


CRDN remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees.

We are excited to provide the following benefit enhancements highlighted below:


  • Medical/Rx: Additional POS plan will be offered
  • Dental: MetLife will be the new 2022-23 carrier and includes an enhancement to the annual maximum of $1,750 and ortho lifetime maximum of $1,500
  • Life/AD&D: Enhanced benefit of $50k
  • STD: Enhanced benefit of a 7 day elimination period for accident and sickness

ENROLLMENT

All elections must be submitted by July 20th.


ENROLLMENT INSTRUCTIONS:

  1. Review the information in this guide and benefit plan summaries.
  2. You must complete and sign an enrollment form, even if you are waiving coverage.
  3. You will not be allowed to make changes after the open enrollment window closes, unless you experience a qualifying life event.


ELIGIBILITY: Regular employees working at least 35 hours per week are eligible to participate in benefits offered by CRDN. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and child(ren) up to the end of the calendar year your child turns age 26 for medical, dental, and vision coverage; and the date the child turns 26 for the voluntary life/add benefits.


The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

BCBSM will continue to provide your medical and prescription drug benefits this year. The plans that will be offered effective January 1, 2023 are as follows:

  • PPO HSA 1500
  • POS 500 - New!


The new Point of Service plan uses the BCN HMO network for those employee who reside in MI. You will be required to select a Primary Care Physician, however, you do not need a referral to see a specialist. When traveling outside of MI you will have access to the nation wide Blue Card Traditional network which is essentially the same as the BCBSM PPO network. For those employees who reside outside of MI, you will only use the Blue Card Traditional network and you do not need to select a Primary Care Physician.

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

  • Covered by a Qualified High Deductible Health Plan
  • You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
  • You cannot be covered by Medicare or Medicaid
  • You cannot be eligible to be claimed as a tax dependent
  • You have not received VA benefits in the past 90 days


Please take note that the HSA runs on a calendar year basis. Eligibility and contribution maximums are based on the period of Jan 1 - Dec 31. Calendar year maximums are shown below for both 2022 and 2023.


  • 2022 HSA IRS Maximums are as follows: $3,650 for a single contract / $7,300 for a two person or family
  • 2023 HSA IRS Maximums are as follows: $3,700 for a single contract / $7,400 for a two person or family
  • "Catch Up" Contribution: $1,000 "Catch Up" contribution for those age 55+

CRDN provides those employees who are covered by one of the BCBSM/BCN medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.


If you are enrolled in a CRDN standard (non HDHP/HSA plan) your cost sharing for using these services will be equal to your office visit copay. If you are enrolled in a CRDN HDHP/HSA plan these services will be subject to your deductible and coinsurance; applicable charges until you have satisfied your deductible are as follows: Virtual Medical Visit $59; Virtual Therapy $85-$99; Virtual Psychiatry $199 for initial visit and $95 for follow up visits. In all cases, if you receive a prescription during any of these visits, applicable prescription cost sharing will apply.

CRDN offers vision coverage through National Vision Administrators (NVA) to help pay for eye exams, prescription glasses and contact lenses. Eye exams can help detect serious eye and general health conditions sooner: high blood pressure, diabetes, heart disease, high cholesterol. Babies should receive their first professional eye exam at 6 months. 80% of learning in the first 12 years comes through the eyes.


You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Review your coverage details in your Benefit Guide.

CRDN provides you with financial protection options for life, accidental death & dismemberment, short term disability and long term disability with Unum.


Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. All eligible employees are covered by a Life and Accidental Death & Dismemberment (AD&D) policy paid for by CRDN. This policy includes an enhancement for 2022, your benefit will be increased to $50,000. In addition to this coverage, employees have the opportunity to enroll in Term Life and AD&D insurance which will further supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage and will be paid for by you through post-tax payroll deduction. If you choose to cover yourself by the supplemental coverage, you may also enroll your spouse and/or children


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. The 2022 plan will include a reduced elimination period of 7 days for an accident or illness.


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

  • All benefit eligible can participate
  • If you are enrolled in the HSA plan, you will be eligible for the Limited Purpose FSA only
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
