  • Coverage for medical, dental, vision, and life insurance begins on the first day of the month following your date of hire.
  • Long-Term Disability coverage becomes effective after one year of employment.

Please note: Employees hired on or after April 2, 2019 are eligible for the High Deductible Health Plan (HDHP) with a Health Savings Account (HSA) only.

For additional eligibility details, please refer to the New Hire Information tab or contact your HR or Benefits Team.

Dependent Eligibility

Eligible dependents typically include:

• Your legal spouse

• Your children up to age 26 (natural, adopted, or stepchildren)

• Children over age 26 who are unable to support themselves due to a physical or mental disability

⚠️ Please Note: Enrolling someone who does not qualify as a dependent is considered insurance fraud and may result in penalties.

📄 Dependent Verification:

City of Covingtonmay require verification of dependent eligibility. This could include submitting documents such as a marriage certificate, birth certificate, or legal guardianship papers. Until these documents are submitted and approved, dependent benefits may be placed on hold or pending.

For questions about the verification process, please contact the NFP Call Center to speak with a Client Advocate.

New Hire Info

Select your benefits through the enrollment portal (see links below).

You’ll have 30 days from your hire date to enroll in your benefits.

After you make your elections, your benefits will become effective on 1st of the month following hire date.

For example:

💡 Payroll Deductions

Payroll deductions will begin after your benefits go into effect.

Timing may vary — please check with your HR Manager for confirmation.

📌 Reminders

You are required to provide full details for any dependents or beneficiaries, including:

  • Full Name
  • Date of Birth
  • Social Security Number

⚠️ Missing your enrollment window may result in delayed coverage or limited plan options.

Qualifying Life Events

When Can I Change My Coverage?

Your benefit elections typically remain in place for the entire plan year—unless you experience a Qualifying Life Event (QLE).

Common QLEs include:

  • 🧑‍🤝‍🧑 Changes in marital status

Marriage, divorce, legal separation, annulment, or death of a spouse.

  • 👶 Change in dependent status

Birth, adoption, placement of a foster child, legal guardianship, or death of a covered child.

  • 👔 Employment changes

Gain or loss of employment for you or your spouse, changes in work schedule (full-time/part-time), or return from unpaid leave.

  • 📍 Change in residence

A move that affects benefit eligibility for you, your spouse, or dependents.

  • 🩺 Eligibility for other coverage

Gaining or losing coverage through Medicare, Medicaid, CHIP, PeachCare, or a spouse’s employer plan.

⚠️ Important:

You must report your Qualifying Life Event within 30 days of the event.

Missing this deadline may result in delayed or denied changes—you’ll have to wait until the next Open Enrollment period.

📎 Supporting Documentation

To process your change, you’ll need to provide proof of the event, such as:

  • Marriage certificate
  • Divorce decree
  • Birth certificate or hospital record
  • Legal adoption/guardianship paperwork
  • Death certificate
  • COBRA or loss of coverage letter
  • State Medicaid/PeachCare documentation

City of Covingtonmay have specific documentation requirements. Please contact your HR or Benefits Team or refer to the Contacts tab for guidance.

📲 How to Declare a QLE

Log into the enrollment portal or contact NFP for help submitting your change:

📞 (844) 626-8435

We recommend keeping records of any confirmations or receipts you receive during the change process.

💡 Note: A newborn is typically covered for the first 30 days after birth. To continue coverage, you must enroll your child within the QLE window. Otherwise, coverage may be terminated retroactively to the date of birth.

Post-Employment Coverage

What happens to your benefits when your employment ends?

When your employment ends, it’s important to understand what happens to your benefits. This section provides a general overview of what to expect after separation, including how long your coverage may remain active and options that may be available to continue your benefits.

Coverage typically remains in effect for a defined period of time following separation, but this timing can vary by employer and benefit type. Be sure to review the Coverage End Dates section for details specific to your situation.

In many cases, you may also have the opportunity to continue certain benefits through COBRA or another continuation program. Additional information can be found in the COBRA & Continuation section.

Coverage End Dates

The date your benefits end depends on your employer’s policy and the type of coverage. In most cases, benefits may remain active for a set period of time following your separation date.

Coverage typically ends on one of the following:

  • The same day your employment ends
  • The last day of the month in which your employment ends
  • Another specified date defined by the employer

Please refer to the details below for your group-specific rules:

  • Medical Coverage Ends: End of Month
  • Dental Coverage Ends: End of Month
  • Vision Coverage Ends: End of Month
  • Curalinc Coverage Ends: End of Month
  • Life & Disability Insurance Coverage Ends: Last day of employment
  • Other Benefits: Last day of employment

If you’re unsure about your coverage end dates, contact your HR or benefits team for clarification.

Cobra & Continuation of Coverage

Medcom

Group Number: City of Covington | (800) 523-7542 option 3 | COBRA/Retiree & Direct Billing Login | Medcom Benefits | premiumbilling@medcombenefits.com

COBRA (Consolidated Omnibus Budget Reconciliation Act) provides eligible employees and their dependents the option to continue certain health benefits after employment ends or following a qualifying event.

Key points about COBRA continuation coverage:

  • Eligibility depends on your employer’s group size and the type of benefits offered.
  • Coverage is typically available for a limited time period, which varies by plan and event.
  • You may be responsible for paying the full premium cost, plus any administrative fees.
  • Enrollment and payment deadlines apply, so it’s important to review all COBRA notices carefully.

For specific details on COBRA eligibility, coverage duration, and costs for your group, please refer to your benefits administrator or HR contact.

Health accounts help you manage out-of-pocket healthcare and dependent care costs using pre-tax dollars. City of Covington offers the following accounts to help you save on eligible expenses.

  • Health Savings Account (HSA): An HSA pairs with a high-deductible health plan and lets you set aside pre-tax dollars for qualified medical, dental, and vision expenses. Funds roll over each year and remain yours, even if you change jobs or retire.

  • Health Reimbursement Arrangement (HRA): An HRA is funded by your employer to reimburse you for eligible medical expenses, depending on the plan design. Unlike FSAs or HSAs, only your employer can contribute.

Visit each tab to explore plan features, contribution limits, and eligible expenses.

Health Savings Account (HSA)

💰 HSA Bank - HSA

The City of Covington automatically enrolls employees on the HDHP Medical Plan in a Health Savings Account (HSA) through HSA Bank. Your HSA lets you set aside funds tax-free to pay for qualified medical expenses—now or in the future—including deductibles, copays, coinsurance, and certain long-term care or Medicare expenses. Funds roll over year to year, can be invested, and remain yours even if you change jobs or retire.

💡 Why Consider an HSA?

  • Contributions outside of payroll are 100% deductible from your federal taxable income
  • Earnings grow tax-free, and withdrawals for qualified expenses are tax- and penalty-free
  • Pairs with a High-Deductible Health Plan (HDHP) to help lower monthly premiums
  • You own the account—funds remain yours even if you leave your employer

  • Employee Only: $1,200
  • Employee + Spouse: $1,500
  • Employee + Child(ren): $1,500
