We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Conveyor Solutions. When you enroll in the benefits program, you may also cover your eligible benefits. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.
As you know, Conveyor Solutions sponsors medical, dental and vision plans. This means you share in the cost of the plan(s) through your payroll deductions, copays and coinsurance, This year our medical plan ran well, meaning costs were well managed resulting in a good renewal. Some years we aren't so fortunate and increased costs must be shared. So given the personal and business challenges we have all experienced during the pandemic, we are happy to share this good news!
Conveyor Solutions remains committed to providing a competitive, cost-effective benefit program. We are implementing no changes for the 2021-2022 plan year. Please be sure to visit each section of this site, and if you wish to make any changes to your benefit elections for the upcoming year, please communication with HR.
Please download and review the "Annual Notices" packet below. Each year we are required to provide this information to you. Please sign and return the acknowledgement to Nichole by December 22.
Action Required:
All elections must be submitted by Noon on December 22. The benefits you elect during open enrollment will be effective from 12/1/2021 - 11/30/2021.
If you are not making any changes to your elections for the upcoming year, you do not need to do anything.
The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.
You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Humana will administer your medical and prescription drug benefits this year. There have been no changes to your medical benefits for the upcoming year.

Conveyor Solutions's dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Dental Care Plus network. You must see an in network provider to be eligible for benefits.
