Eligibility
Regular employees working at least 30 hours per week are eligible to participate in benefits offered by CLC. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and child(ren) up to age 26.

BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. The CLC dental plan provides comprehensive coverage to help you and your family maintain good dental health. Seeking care in the Delta Dental PPO Network will give you the lowest out of pocket expenses, however, you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher.

Employer Provided - $50,000 Life and Accidental Death & Dismemberment Insurance
Voluntary Life/AD&D: A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.

To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

$3,850 for a single contract
$7,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+
Maximums noted above are off-set by any employer provided HSA funding.

Your mental wellbeing is equally important as your physical wellbeing. CLC recognizes the importance of taking good care of yours and your families mental health. CLC provides all employees and their dependents access to the Employee Assistance Program with LFG. For those employees covered by the BCBSM medical plan, you and your covered dependents also have access to virtual mental health services with BCBSM Online Visits (registration link included in Telemedicine section).
