WELCOME


Helping you and your family reach and maintain good health is very important to CLC. CLC is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.




Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by CLC. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and child(ren) up to age 26.

All team members have access to our online benefits enrollment platform 24/7 where you have the ability to enroll, select or change your benefits online during the annual open enrollment period, new hire orientation, and for qualifying events.

                                  3. Verify all of your personal information.

 

After making your elections, you have the option to print out an Enrollment Summary Statement that contains all of your benefit elections and beneficiary designations. Make changes to beneficiary designations and more.

BCBSM Online Visits

CLC provides those employees who are covered by the BCBSM medical plan access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

Delta Dental Plan

The CLC dental plan provides comprehensive coverage to help you and your family maintain good dental health. Seeking care in the Delta Dental PPO Network will give you the lowest out of pocket expenses, however, you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher.

DID YOU KNOW?

LIFE & DISABILITY

Lincoln Financial Group (LFG)

CLC provides you with financial protection options for Life/AD&D, Voluntary Life/AD&D, Long Term Disability with LFG.


Life/AD&D

Employer Provided - $50,000 Life and Accidental Death & Dismemberment Insurance


Voluntary Life/AD&D: A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.

Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Long Term Disability - Employer Paid

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.

Flexible Spending Accounts

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. Contributions

You may contribute as follows:

Health Care FSA

  • Up to $3,050 annually
  • All benefit eligible can participate
  • If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns

HSA

Health Savings Account

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.


To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:

  • Covered by a Qualified High Deductible Health Plan
  • You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
  • You cannot be covered by Medicare or Medicaid
  • You cannot be eligible to be claimed as a tax dependent
  • You have not received VA benefits in the past 90 days


HSA IRS Maximums are as follows:

$3,850 for a single contract

$7,750 for a two person or family


$1,000 "Catch Up" contribution for those age 55+


Maximums noted above are off-set by any employer provided HSA funding.

Your mental wellbeing is equally important as your physical wellbeing. CLC recognizes the importance of taking good care of yours and your families mental health. CLC provides all employees and their dependents access to the Employee Assistance Program with LFG. For those employees covered by the BCBSM medical plan, you and your covered dependents also have access to virtual mental health services with BCBSM Online Visits (registration link included in Telemedicine section).
