PACS has an electronic Benadmin enrollment system called Employee Navigator.
Gold EPO $1,000 $40/$50 - PARTNER NETWORK (Allegheny Employees - narrower network) - Class 1 Employees will pay 25% of employee only coverage and 100% for dependents. EMPLOYER will cover 75% of employee only coverage.
Gold EPO MCA $2,000 $35/$50 - PREMIUM NETWORK (Non-Allegheny Employees - broader network) - Class 2 Employees will pay 20% of employee only coverage and 100% of dependent coverage. EMPLOYER will cover 80% of employee only coverage.

Your employer will setup your profile in Employee Navigator and then will trigger an email for you to go into the ENAV system.
Once you receive that email from Employee Navigator you will go in and elect and/or waive any benefits which can't be changed until next open enrollment unless you have a qualifying life event. (example: divorce, married, have a baby, adopt a child or lose other coverage) You have 30 days from the date of the event to notify HR to make appropriate changes.
Please write down your username and password as every year at open enrollment you will be required to go in and re-elect or waive your benefits.

Summary of Benefits and Coverage (SBC) for each plan which are required to be given to all employees.
Employee Benefits Notices are attached per the ACA guidelines which are required for all employees.
Section 125 Form (this is your pre-tax form allowing your employer to take out of your pay your contributions pre-taxed - must be kept in your employee file and completed each year.)
