WELCOME

Helping you and your family reach and maintain good health is very important. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

ENROLLMENT

Annual Open Enrollment: November 6, 2024 - November 13, 2024

Remember: The benefit choices you make during the initial enrollment or annual open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event. Refer to your Employee Benefits Guide or contact Human Resources if you have questions about qualifying events.

MEDICAL/RX

BlueCross BlueShield of Texas

BCBSTX will continue to be the medical and prescription carrier this year. Three medical plans will be offered: two High Deductible Health Plans (HDHP) and a PPO Plan.

The PPO option offers the freedom to see any provider, when you need care. When you use providers from within the PPO network, you receive benefits at the discounted network cost. Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.

The HDHP plans are similar to the PPO Plan in that you have the option to choose any provider when you need care. However, in exchange for a lower per-paycheck cost, you must satisfy a higher deductible that applies to almost all health care expenses, including those prescription drugs. All expenses are your responsibility until the deductible is reached, with the exception of preventative care, which is covered at 100% when you visit a physician in the network. Once the deductible is met, you are responsible for coinsurance for medical expenses and a copay for prescription drug expenses.

If you are already enrolled in one of the Choice Health BCBS medical plans today and have not already done so, please be sure to register through the BCBS member portal where you can access important information pertaining to your medical plans with BCBS including participating providers, cost estimates for services, explanation of benefit/claims, and your accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the BCBS portal as of your effective date of coverage.

Helpful Rx Cost Saving Tools & Tips!

MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays, resulting in lower expenses for you.

GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside your plan (insurance will not be applied and costs with the coupon will not count towards your deductible and/or out-of-pocket maximums.)

ASK YOUR DOCTOR - Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.

DENTAL

Mutual of Omaha

Our dental plan has moved to Mutual of Omaha and provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Mutual of Omaha Dental Network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher.

Please be sure to register through the Mutual of Omaha member portal where you can access important information pertaining to your dental plan with Mutual of Omaha, including participating providers, coverage information, and explanation of benefits/claims.

Our vision coverage has moved to Mutual of Omaha using the EyeMed Network. We offer vision coverage to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider an In-Network provider.

Lively HSA

A Health Savings Account (HSA) is a tax-free savings account owned by you, is 100% vested from day one, and lets you build up savings for future needs. The funds may be used to pay for qualifying healthcare expenses not covered by insurance or any other plan for yourself, your spouse, or tax dependents. You decide how much you would like to contribute, when and how to spend the money on eligible expenses, and how to invest the balance.

HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and any employer provided funds to the HSA. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.

Basic Life/AD&D NEW!

Voluntary Life/AD&D
