Annual Open Enrollment: November 6, 2023 - November 13, 2023
Remember: The benefit choices you make during the initial enrollment or annual open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event. Refer to your Employee Benefits Guide or contact Human Resources if you have questions about qualifying events.

BCBSTX will continue to be the medical and prescription carrier this year. Three medical plans will be offered: two High Deductible Health Plans (HDHP) and a PPO Plan.
The PPO option offers the freedom to see any provider, when you need care. When you use providers from within the PPO network, you receive benefits at the discounted network cost. Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.
The HDHP plans are similar to the PPO Plan in that you have the option to choose any provider when you need care. However, in exchange for a lower per-paycheck cost, you must satisfy a higher deductible that applies to almost all health care expenses, including those prescription drugs. All expenses are your responsibility until the deductible is reached, with the exception of preventative care, which is covered at 100% when you visit a physician in the network. Once the deductible is met, you are responsible for coinsurance for medical expenses and a copay for prescription drug expenses.

Our dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Ameritas Dental Network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher.
If you are already enrolled in the Choice Health Standard dental plan today and have not already done so, please be sure to register through the Standard member portal where you can access important information pertaining to your dental plan with the Standard including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Standard portal as of your effective date of coverage.

We offer vision coverage to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider an In-Network provider.
A Health Savings Account (HSA) is a tax-free savings account owned by you, is 100% vested from day one, and lets you build up savings for future needs. The funds may be used to pay for qualifying healthcare expenses not covered by insurance or any other plan for yourself, your spouse, or tax dependents. You decide how much you would like to contribute, when and how to spend the money on eligible expenses, and how to invest the balance.
Voluntary Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death.
