All employees should complete an election form whether you are enrolling or waiving benefits.
Enrolling with Sedera is a two step process. You must complete your election form and then enroll directly with Sedera with the link below by the 19th of the month to get a 1st of the month effective date. If you enroll after the 19th your membership may be pushed to the following month.
ASi MEC Plus
This is the part of your combo that covers your preventative care at 100%. When you enroll in Sedera you are automatically enrolled in this plan.
You will receive an ID card for this plan.
PHCS network search instructions:
●Go to www.multiplan.com
●Click on Find a Provider in the upper right corner
●Click on the green Select Network button & choose PHCS
●Click the green Select Network box
●Choose PHCS in the pop-up box
●Select Preventive Services Only
Services billed by a hospital are not covered on this plan.
