Welcome
Helping you and your family reach and maintain good health is very important to CER Group NA. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. CER Group NA remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Highlights of the 2026-2027 benefit plans include:
Medical moving from BCN to HAP
- BCN HMO $3000 moving to HAP HMO $3000
- BCN HMO HSA $6350 moving to HAP HMO HSA $6350
- BCN POS HSA $3300 to HAP PPO HSA $3400
- BCN POS $3000 moving to PPO $4000
- No changes to dental, vision, STD, LTD, Life/AD&D

OPEN ENROLLMENT REMINDER!
Open Enrollment is from March 11, 2026 through March 18, 2026. This is the time of year where you can elect new plans with CER Group NA. You may switch between plans and make changes without being required to have a qualifying life event. Please also keep in mind the following when making your elections for April 2026:
- If you are currently covered by the Voluntary Life/AD&D, you may increase your benefit up to the guaranteed amount of $50,000 for the employee and $25,000 for your spouse with no evidence of insurability!
Eligibility
All FULL-TIME employees working at least 30 hours per week are eligible to participate in the benefit plans offered by CER Group NA. Newly hired employees will be eligible the 1st of the month following 30 days of full-time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children are eligible for benefits as follows:
• Medical, Dental and Vision: Until the last day of the year in which the child turns 26
• Dependent Child Life Insurance & AD&D: Until the last day of the year in which the child turns 23, or 25 if the child is a full-time student
Should your employment with CER Group NA end, your coverage will end as of the date of termination.

Action Required: All elections must be submitted by March 18, 2026!
ENROLLMENT INSTRUCTIONS:
- Review your benefit information included on this site
- Log into: Login (ease.com)
- Follow instructions and enroll in your benefits - you must complete open enrollment even if you are waiving benefits
- Make sure to save your elections and save/print your confirmation statement.
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

Medical & Rx

Health Alliance Plan (HAP) will to be your NEW medical and prescription carrier. You have the choice between four plans for the 2026-2027 plan year. T. View the Providers & Networks for more information on how these plans networks differ from each other as well as buttons to locate participating providers.
Please be sure to register through the HAP member portal where you can access important information pertaining to your medical plans with HAP including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. You will be able to register in the HAP portal as of your effective date of coverage.
Additional requirements under this plan:
You must select a Primary Care physician (PCP) who will determine the care/treatment you need. In the HMO, in order to see a specialist, you must obtain a referral from your PCP. You may self-refer yourself to a specialist if you are in the POS plans. PPO plans do not require a designated PCP or referrals.

Rx Information
Helpful Rx Cost Saving Tools & Tips!
MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. You can get more information on how to start receiving mail order drugs by doing to the medications tab of your portal and then hitting mail order.
GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
Providers & Networks
The two medical plans function differently when it comes to networks and access. The POS plans provide out-of-network coverage for a variety of services, whereas the HMO only provides out-of-network coverage in the event of a true emergency. If you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing.

You have the opportunity to contribute to a variety of savings and/or spending accounts on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by CER Group NA.
Health Savings Account (HSA)
The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. The HMO HSA 3400 and the HMO HSA 6350 Medical Plans are qualified for an HSA. By contributing to an HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and any employer provided funds to the HSA. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.
$4,400 for a single contract | $8,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+
If you have an HSA with another bank or custodian, you can transfer your funds to the CER Group NA HSA's with Health Equity by completing this form.

Dental

CER Group NA dental plans provide comprehensive coverage to help you and your family maintain good dental health. The dental plans include both in and out-of-network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in one of the CER Group NA Guardian Dental plans today and have not already done so, please be sure to register through the Guardian Dental member portal where you can access important information pertaining to your dental plan with Guardian Dental including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Guardian Dental portal as of your effective date of coverage.
Dental Plans Overview
Dental Benefits through Guardian provide comprehensive coverage to help you and your family maintain good dental health. You can elect the PPO or DHMO plan. Your coverage will be greater if you go to an in-network provider if you select the PPO.
Vision

CER Group NA offers vision coverage through EyeMed to help pay for eye exams, prescription glasses and contact lenses. EyeMed also provides discounts for Hearing Care and Lasik.
If you are already enrolled in one of the CER Group NA EyeMed vision plan today and have not already done so, please be sure to register through the EyeMed member portal where you can access important information pertaining to your vision plan with EyeMed including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the EyeMed portal as of your effective date of coverage.

Network & Resources
Your vision network with EyeMed is the INSIGHT network. You may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services out-of-network, you may be required to pay that provider at time of service and will need to submit a claim to EyeMed for the applicable reimbursement based on the out-of-network reimbursement schedule.

Financial Protection
CER Group NA understands the importance to you and your family to have strong financial protection. We provide you with the opportunity to purchase Supplemental Life and AD&D, Short-Term Disability, and Long-Term Disability. All of these coverages are provided to protect your income and out-of-pocket expenses.
Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. CER Group NA provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage in the amount of $25,000 at no cost to you.
Employees have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death.
Voluntary Life
When you are first eligible (at hire) for Optional Life coverage, you may purchase up to the Guaranteed Issue (GI), $175,000 for yourself and up to $25,000 for your spouse, without providing proof of good health through the insurance provider’s EOI process.
During Open Enrollment:
- Current participants can increase Employee coverage by up to $50,000 and Spouse coverage by on level, not to exceed the $25,000 GI, without proof of good health. Any amount above these increase levels will be subject to medical underwriting.
- Non-participants will have to provide proof of good health for any dollar amount.
If you elect optional life coverage, and are required to complete EOI form, it is your responsibility to complete the form and to submit it to Guardian within 30 days of your election.
Accidental Death and Dismemberment
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

CER Group NA provides Voluntary Short & Long-Term Disability benefits!
Everyday illnesses or injuries can interfere with your ability to work, more serious issues can impact your ability to work indefinitely.
