Introduction

We are pleased to present this guide, which highlights the comprehensive coverage available to you. Our benefits program is designed to offer employees substantial coverage to meet both individual and family needs. This booklet includes only highlights. The specific terms of coverage, exclusions and limitations are contained in the plan documents and insurance certificates. All coverage and coverage costs are subject to change at any time in the future. If you have any questions about a specific service or treatment, please contact the appropriate insurance carrier.

Remember: The benefit choices you make during the initial enrollment or annual open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event. Refer to your Employee Benefits Guide or contact Human Resources if you have questions about qualifying events.

  • Visit Paylocity to review your current coverage and enroll and modify your elections for 2025.
  • Please make all changes or new elections by December 8th.
  • To participate in a Health Savings Account in 2025, you must make an active election, even if you're already participating in 2024.

We're excited to share that our dental plan is moving to Cigna beginning January 1, 2025!

Our dental plan provides comprehensive coverage to help you and your family maintain good dental health. You have the freedom to select the dentist of your choice; however, when you visit a participating in-network dentist, you will have lower out-of-pocket costs, no balance billing, and claims will be submitted by your dentist on your behalf. Be aware if you go out-of-network, your costs will be higher.

  • Eye exams can help detect serious eye and general health conditions sooner: high blood pressure, diabetes, heart disease, high cholesterol
  • Babies should receive their first professional eye exam at 6 months
  • 80% of learning in the first 12 years comes through the eyes


We offer vision coverage to help pay for eye exams, prescription glasses and contact lenses. Under this plan, you may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services from an out-of-network provider, you will be required to pay that provider at the time of service and submit a claim form for reimbursement.





HSA (Health Savings Account)

A Health Savings Account (HSA) is a tax-free savings account owned by you, is 100% vested from day one, and lets you build up savings for future needs. The funds may be used to pay for qualifying healthcare expenses not covered by insurance or any other plan for yourself, your spouse, or tax dependents. You decide how much you would like to contribute, when and how to spend the money on eligible expenses, and how to invest the balance.

Life insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. AD&D insurance is equal to your Life benefit in the event of your death being a result of an accident, and may also pay benefits for certain injuries sustained.

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months - even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.


Employee Assistance Program (EAP)

The EAP is included - at no additional cost - with the Long-Term Disability benefit. The GuidanceResources program MGIS Complete offers someone to talk to and resources to consult whenever and whenever you need them. Highly trained clinicians will listen to your concerns and help you or your family members with any issues, including confidential emotional support, work-life solutions, legal guidance, financial resources or free online will preparation.

We realize you make benefit choices based on what's important to you and the needs of your changing lifestyle. Therefore, along with our core benefits, we offer these additional benefits which help to provide financial support to employees. Additional benefits include:

Regarding Disability and Voluntary Life Insurance:

If you are a new hire, you are able to enroll in the coverage up to the Guarantee Issue amount. If you are a current employee making Open Enrollment elections, you are able to increase your coverage by $10,000 without completing an Evidence of Insurability (EOI). If you are a current employee and would like to increase your coverage by more than $10,000, you will need to complete and submit an EOI for the additional coverage amount. EOIs can be completed electronically, and the links can be found in Paylocity, according to employee class.

Submitting an EOI is the employee's responsibility.

The increase in coverage over the GI amount will not go into effect until your EOI has been approved by the insurance carrier.
