Plan Year Dates

Columbus Consolidated Government's benefit plan year begins on January 1st, 2026, and ends on December 31st, 2026. This timeframe defines the period during which your benefits are effective.

Please refer to this timeframe when making benefit elections, submitting claims, or requesting changes to your coverage.

Username: first initial + last name + last 4 of SSN (ex: jdoe4567)

Welcome to your eligibility overview!


This section helps you understand who qualifies for benefits and important timelines for enrolling or making changes. Check your plan details below to confirm specific rules that apply to you and your family.

Employee Eligibility

Active full-time employees of Columbus Consolidated Government are eligible for benefits. Employees under age 65 may enroll in the Anthem/BCBS medical, dental, and vision plans (see New Hire Information tab). Employees 65 and older who are retired are eligible for the UHC Medicare Advantage plan (see Medicare Advantage Plan tab). For any eligibility questions, contact your HR or Benefits Team.

Dependent Eligibility

Eligible dependents typically include:

• Your legal spouse

• Your children up to age 26 (natural, adopted, or stepchildren)

• Children over age 26 who are unable to support themselves due to a physical or mental disability

⚠️ Please Note: Enrolling someone who does not qualify as a dependent is considered insurance fraud and may result in penalties.

📄 Dependent Verification:

Columbus Consolidated Government requires verification of dependent eligibility. This could include submitting documents such as a marriage certificate, birth certificate, or legal guardianship papers. Until these documents are submitted and approved, dependent benefits may be placed on hold or pending.

New Hire Info

Select your benefits through the enrollment portal (see links below).

You’ll have 31 days from your hire date to enroll in your benefits.

After you make your elections, your benefits will become effective the 1st of the month following your eligibility window.

For example:

If you're hired on June 18, your benefits would begin August 1.

💡 Payroll Deductions

Payroll deductions will begin after your benefits go into effect. Timing may vary—please check with your HR or payroll contact for confirmation.

Qualifying Life Events

When Can I Change My Coverage?

Your benefit elections typically remain in place for the entire plan year—unless you experience a Qualifying Life Event (QLE).

Common QLEs include:

  • 🧑‍🤝‍🧑 Changes in marital status

Marriage, divorce, legal separation, annulment, or death of a spouse.

  • 👶 Change in dependent status

Birth, adoption, placement of a foster child, legal guardianship, or death of a covered child.

  • 👔 Employment changes

Gain or loss of employment for you or your spouse, changes in work schedule (full-time/part-time), or return from unpaid leave.

  • 📍 Change in residence

A move that affects benefit eligibility for you, your spouse, or dependents.

  • 🩺 Eligibility for other coverage

Gaining or losing coverage through Medicare, Medicaid, CHIP, PeachCare, or a spouse’s employer plan.

⚠️ Important:

You must report your Qualifying Life Event within 30 days of the event.

Missing this deadline may result in delayed or denied changes—you’ll have to wait until the next Open Enrollment period.

💡 Note: A newborn is typically covered for the first 30 days after birth. To continue coverage, you must enroll your child within the QLE window. Otherwise, coverage may be terminated retroactively to the date of birth.

What to Expect While You’re Still Working

If you are actively employed and turning 65, you will receive a Medicare Enrollment Kit 60–90 days before your birthday. This kit includes information about:

  • Medicare Part A (Hospital Insurance)
  • Medicare Part B (Medical Insurance)
  • Medicare Part D (Prescription Drug Coverage)

📌 Important

While you are actively employed and covered under your Anthem/BCBS medical plan, your employer coverage remains primary. Medicare acts as secondary coverage.

💡 No Late Enrollment Penalties

Your employer plan is considered Creditable Coverage for Medicare Parts B and D. You will not face late enrollment penalties if you delay enrolling in Medicare until retirement.

Your health matters, and CCG’s Wellness Program supports employees in staying healthy and compliant with program requirements.

The Personal Health Assessment (PHA) includes a Health Risk Assessment and biometric screening. Based on results, some employees may be required to participate in health coaching.

📅 Important Dates & What to Expect

  • The 2025 PHA event ran from June 2 through September 25, 2025.
  • Employees requiring health coaching were encouraged to self-schedule during November and December.
  • Health coaches are completing outreach calls through mid-February.
  • Employees who have not scheduled coaching will receive a mailed notice by March 1.
  • All required coaching must be completed by April 1.

  • Tobacco Cessation compliance was submitted in October 2025.

This comprehensive clinic offers primary, urgent, and preventive care (for ages 3 and up), management of chronic conditions (such as high blood pressure, diabetes, asthma, and more), laboratory testing, flu shots, limited onsite pharmacy services, referrals to diagnostic imaging at no cost, and wellness programs like smoking cessation and weight management. Employees and dependents also have access to home delivery of prescriptions (with free shipping) within 5–7 business days. Telemedicine services are available 24/7/365.

⚠️ Important: Participation in the CareATC Wellness Clinic and completion of the required Health Risk Assessment (HRA) is mandatory to avoid a 22% surcharge on medical premiums per pay period (applies to both employees and spouses).
