There are three medical plan options partially paid by Bushel Inc, through BlueCross and BlueShield of North Dakota. While you are free to select any provider you choose, it is more cost effective to stay within the network. All plans include preventive services paid at 100%. Omada This personalized program provides the tools and support, you need to reach your health goals, whether that’s losing weight or improving your overall health, all at no cost to you. Diabetes Prevention Program (DPP) services are a benefit covered at 100% to you or your adult family members who are enrolled in a Blue Cross Blue Shield of North Dakota group health plan and at risk for type two diabetes. If you wish to participate in a DPP program offered by your local health care provider, please check to see if these services are eligible for 100% coverage. DPP services provided by Omada are covered at 100%.

It is paired with a qualifying health insurance plan; typically, a high-deductible health plan (HDHP). An HDHP is a plan that offers lower monthly premiums in exchange for a higher deductible (the amount you pay out of pocket before insurance kicks in).
Flexible Savings Account (FSA) A Flexible Spending Account (FSA) allows you to aside pretax dollars for qualified expenses and reduces your taxable income. You can also enroll in a Dependent Care FSA to set aside pretax dollars to cover daycare expenses.

Voluntary Dental Plans with SunLife
The Sun Life Dental Plan is a voluntary benefit, and you have the option of electing either the Basic Plan or the Enhanced Plan. Both plans utilize the same Sun Life PPO network and include preventative services paid at 100% with participating providers. The Enhanced Plan provides for a higher annual maximum and features additional coverage for basic services, major services and orthodontic care.

Basic and Voluntary Life and AD&D with MetLife
Basic Life and AD&D Insurance - Company Paid Basic term life and accidental death and dismemberment coverage is provided for all benefit eligible employees. Employees must assign a beneficiary for the company provided benefit. This plan is portable when requested within 31 days of termination.
$50,000 Term Life + $50,000 AD&D
Age Reduction of Coverage: $32,500 term life for ages 70 up to age 75. $25,000 term life for ages 75 and over.
Voluntary Life and AD&D Insurance - Employee PaidMetLife Supplemental Term Life/AD&D Insurance is a voluntary benefit. Through this benefit you are able to purchase additional Life insurance to fit your individual needs.
Employees who elected coverage previously can increase their election by one increment of $10,000 without Evidence of Insurability and spouses are allowed the same in a $5,000 increment, if that increase does not exceed the guaranteed issue of $100,000 for employees, or $25,000 for spouses.
If an employee previously waived Supplemental Term Life/AD&D as a new hire, they do have the option during their 1st open enrollment to elect up to the guaranteed issue amount. After that 1st open enrollment, employees would need Evidence of Insurability for any amount elected.
New hires can elect up to the guaranteed issue amount for employee, spouse, and child without needing to complete evidence of insurability; any amounts over the guaranteed issue will require evidence of insurability to be completed and approved by MetLife to get the additional amount of life insurance.

Short and Long-Term Disability
Your family can count on your income while you are healthy and employed, but it is important to plan for your financial security in the event you become unable to work because of a disability. Disability coverage replaces a portion of your income when you are not able to work.
Short-Term Disability coverage is administered through Bushel by an internal Salary Continuation Program. Bushel will pay 100% of benefits for a sickness, illness or pregnancy for up to 12 weeks.
Long-Term Disability coverage is administered through MetLife.

MetLife Critical Illness Critical Illness coverage can help ease the financial burden of surviving a critical illness by providing financial resources to help with medical costs or ongoing living expenses. Coverage is guarantee-issue, and benefits are paid regardless of other medical insurance.
*Pre-Existing Condition Limitation: A 3/6 Pre-existing Condition Limitation applies. If you have an existing condition diagnosed or treated (including medication) 3 months prior to the effective date this condition will not be eligible for benefits for 6 months.

Retirement 401(k) through Bell Bank
Bushel offers eligible employees to participate in the Bushel Inc 401(k) Retirement plan. Eligible employees include full time and part time employees that have completed three months of service. Interns are not eligible to participate in the 401(k) program.
All newly hired employees will be automatically enrolled in the Bushel Retirement plan and will actively need to unenroll in the retirement program if there is no interest in participation. Automatic enrollment will be defaulted to a 5% pretax employee deferral. Employees under 5% are automatically increased 1% per year up to 5% unless opted out.
You can make two types of contributions:
Pre-Tax Contributions – Amounts you contribute, and any investment earnings are not taxed until you withdraw the money after you retire.
Roth After-Tax Contributions – Your contributions are taxed as regular income in the year you make the contributions. Future withdrawals of your contributions and investment earnings are tax free.
Bushel Inc. may make a matching contribution based on eligible compensation you contribute to the plan. The match is currently 100% of the first 3% of compensation you defer and 50% on next 2% of deferred compensation. This amount may be changed by the employer.
