Helping you and your family reach and maintain good health is very important to Media Loft.


Media Loft is pleased to offer an extensive selection of benefits designed with your health and financial wellbeing in mind. We review our benefit program each year to ensure our offerings provide the best combination of benefit coverage, network access and affordability.


Eligibility

You are eligible for benefits if you are a regular, full-time employee who works 30 hours or more per week. If you are eligible, you may enroll in benefits the first of the month following your date of employment. If you previously waived any benefits, you have the option during Open Enrollment to enroll or make benefit changes and add or drop dependent coverage without having a qualifying event.

Your Employee Benefit Guide and this web page provides general information as an overview. If there is any discrepancy between the insurance carrier's certificate of coverage or Summary Plan Description and this web page or Benefit Guide, the insurance plan documents will prevail.

ENROLLMENT

How To Access Your Benefits

You can access your current benefits by logging into G&A Partners using your individually provided credentials as shown below:

If you are enrolling in benefits, follow the process as outlined below:

  1. Log into the system using your unique username and password as described above.
  2. Click on the Benefits tile.
  3. You will land on the Welcome Page. Click "Next" at the bottom of the page to advance through the enrollment.
  4. Your available medical plan options will appear on the first screen. Use the toggle buttons to add dependents to the coverage. Click "Select" on the medical plan you wish to enroll in. You may click "Compare" to see additional plan information. When you have finished selecting or waiving the medical coverage, click the "Next" button at the bottom of the page.
  5. Continue through each screen either selecting or waiving the coverage.
  6. After completing all coverage options and clicking the "Next" button, you will see a Benefit Summary list of all enrollments and waived plans. Scroll to the bottom and click the "Submit" button.

ELECTION CHANGES

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.


Refer to your Employee Benefits Guide or contact G&A Partners if you have questions about qualifying events.

MEDICAL

Blue Cross Blue Shield MN

Media Loft offers three medical plan options for full-time employees. Each option provides high-quality, affordable medical care, including preventive care, doctor's visits, hospitalization, and emergency care. However, each plan has unique characteristics and advantages. The plans are offered through Blue Cross Blue Shield of Minnesota.


PLAN CHOICES

  • Plan T23046: $1,000 deductible with 20% co-insurance after deductible and office visit/Rx co-pays
  • Plan T23051: $2,500 deductible with 20% co-insurance after deductible and office visit/Rx copays
  • Plan T23069: $4,500 deductible with 0% co-insurance - Health Savings Account compatible

HSA

Health Savings Account

If you choose to enroll in the $4,500 HDHP, you may also contribute to a Health Savings Account (HSA). Media Loft will make a one-time contribution of $100 into your account.


A Health Savings Accounts (HSA) is a tax-advantaged savings account which is paired with a high deductible health plan exclusively for the purpose of paying qualified medical, dental, and vision expenses for you, your spouse and your dependents. Over-the-counter (OTC) drugs and medicines are also eligible.


An HSA is tax-free times three. When the funds are used to pay for qualified medical expenses, you do not pay taxes on:

  • Contributions
  • Earnings
  • Withdrawals


At enrollment, you select how much money you want deducted from each paycheck on a pre-tax basis to meet your medical needs. You may change your election throughout the year. Unlike flexible spending accounts, you own this account and the money contributed rolls over from year-to-year - allowing you to save for future medical expenses. If you leave employment, you can take your HSA with you.

FSA

Flexible Spending Accounts

Full-time employees have the option to enroll in a flexible spending account through Benefit Extras.


The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

Health Care FSA

  • Up to $3,050 annually (or in accordance with IRS 2023 limits)
  • All benefit eligible can participate, unless you or your spouse are contributing to an HSA.
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses. Other the counter drugs and medicines are also eligible.


Limited Health FSA

  • Up to $3,050 annually (or in accordance with IRS 2023 limits)
  • Only participants who contribute to an HSA can participate
  • Reimbursements allowed for unreimbursed dental and vision expenses ONLY.


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns


How the Plan Works

You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. Any funds after March 31 unclaimed will be forfeited.

DENTAL

Delta Dental Plan

Media Loft provides two dental plan options to full-time employees through Delta Dental of Minnesota.


  • Plan #T02413: provides care through the larger Delta Dental Premier network of dentists. Your deductible is $50 single and $150 family with an annual benefit maximum of $1,000 per person.


  • Plan #T02414: allows you to choose between the smaller PPO network and the larger Premier network. Obtaining care within the PPO network provides you with the greatest benefits of higher reimbursement percentages and deeper provider discounts - thereby saving you money in out-of-pocket costs. The PPO Network provides 100% coverage for preventive services, no deductible and a $2,000 per person benefit maximum. Care within the Premier network provides 80% coverage for preventive services, an annual deductible of $25 single and $75 family.


There is no coverage on either plan for orthodontia services.

BASIC LIFE/AD&D

Media Loft provides full-time eligible employees with basic term life and accidental death and dismemberment (AD&D) coverage at no cost to you.


The life benefit is payable to your designated beneficiary in the event of your death equal to 1x annual base salary. AD&D benefits are payable to you in the event of a dismemberment or an additional benefit is payable to your beneficiary if your death is the result of an accident.

Overview of Voluntary Life Coverage for You and Your Family

Voluntary Term Life and Accidental Death & Dismemberment (AD&D) Insurance through Unum can provide money for your family if you die or are diagnosed with a terminal illness.

SHORT & LONG TERM DISABILITY

Short and Long Term Disability

The long-term and short-term disability insurance are designed to provide protection for your paycheck should you become disabled and unable to work, as a result of an injury or illness. These plans are paid for by Media Loft on behalf of all benefit eligible employees. The short-term disability benefit is payable for 12 weeks, at which time long-term disability benefits would take over should your disability continue past the 12 weeks.
