Helping you and your family reach and maintain good health is very important to Elevate Living.
Elevate Living is pleased to offer an extensive selection of benefits designed with your health and financial wellbeing in mind. We review our benefit program each year to ensure our offerings provide the best combination of benefit coverage, network access and affordability.
Eligibility
You are eligible to participate in the Benefit Program if you are a full-time employee or benefit eligible part-time employee scheduled to work at least 30 hours per week.
Coverage for most lines of coverage will begin on the 1st of the month following your date of hire. Please note, the disability plan has a different waiting period. Please refer to these sections for additional details regarding when coverage begins for those benefits.
Elevate Property Management closely monitors eligibility under the Company benefits program. If you are covering a dependent who does not meet the criteria above, coverage can be revoked retroactively and any claims paid on behalf of an eligible dependent will become the member’s liability. In addition, you may subject to fines and/or termination of employment. Enrollment is subject to an audit at any time.

Enrollment
Action Required
Read the information provided and review your benefit options carefully. Enrollment must be completed in Kronos/UKG within 30 days of your hire/eligibility date.
Dependents
When you enroll yourself in the benefit program, you may also choose to enroll your eligible dependents for medical, dental, vision and optional life insurance coverage.
Eligible dependents include:
- Legally married spouse; this does not include a common law spouse or an ex-spouse.
- Your (or your spouse’s) natural child, legally adopted child, stepchildren living with you, or children for whom you or your spouse is legal guardian and who are less than 26 years of age.
- Unmarried children incapable of self-support because of a total physical or mental disability (please see HR for details).
Election Changes
The benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.
Benefit Changes During the Year
Unless you experience a qualifying life event, you cannot make changes to your benefits until the next open enrollment period. Qualifying events include things like:
- Marriage, divorce or legal separation.
- Birth or adoption of a child.
- Change in child’s dependent status.
- Death of a spouse, child or other qualified dependent.
- Change in employment status or a change in coverage under another employer-sponsored plan.
If you have a life changing event throughout the year, please reach out to Human Resources within 30 days of the event.
Medical
Medical Plan Overview
Elevate Living offers three medical plan options for full-time employee or benefit eligible part-time employee scheduled to work at least 30 hours per week. All options provide high-quality, affordable medical care, including preventive care, doctor's visits, hospitalization, and emergency care. However, each plan has unique characteristics and advantages. The plans are offered through Medica.
Plan Choices
- Plan A: $2,500-$30-70% Copay Plan (also referred to as a PPO Plan).
- Plan B: $3,400-75% High Deductible Health Plan (HDHP).
- Plan C: $5,500-100% HSA + Rx Copays High Deductible Health Plan (HDHP).
Network Choices
- Medica Choice Passport with United Healthcare Choice Plus Network: In the Passport network you have access to Medica's largest selection of providers in Minnesota with coverage nationally under the United Healthcare Choice Plus national provider network.

Virtual Care (AmWell & Virtuwell)
Virtual care is a convenient way to get care for many common conditions. With a virtual care visit, you:
- Save time – avoid a trip to the doctor’s office and get care from the comfort of your home, work or wherever you are.
- Initiate the visit at your convenience – no appointment needed.
- Get care when you need it – visits are often available after clinic hours, sometimes even 24/7.
- May save money – a virtual care visit may cost less than a regular visit to the doctor’s office, depending on your plan.

Dental
Dental Insurance is available to full-time employee or benefit eligible part-time employee scheduled to work at least 30 hours per week.
In addition to protecting your smile, dental insurance helps pay for dental care and usually includes regular checkups, cleanings and X-rays. Several studies suggest that oral diseases, such as periodontitis (gum disease), can affect other areas of your body—including your heart. Receiving regular dental care can protect you and your family from the high cost of dental disease and surgery.
Elevate Living’s dental insurance is administered by Sun Life Financial. Log in to Sun Life Financial's website below to find a provider near you.
Vision
The Voluntary Vision plan is available to full-time employee or benefit eligible part-time employee scheduled to work at least 30 hours per week.
Driving to work, reading a news article and watching TV are all activities you likely perform every day. Your ability to do all of these activities depends on your vision and eye health. Elevate Living's vision insurance is administered by Sun Life Financial. The vision plan utilizes the Vision Service Plan (VSP) network, which is the largest network of private-practice eye care doctors in the industry.

Flexible Spending Account (FSA)
The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Eligibility
Full-time: Traditional Health Care FSA, Limited Care FSA, Dependent Care FSA
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
You may contribute as follows:
Traditional Health Care FSA
- Employees who are enrolled in the $2,500-$30-70% HSA plan may participate in the Traditional Health Care FSA.
- The maximum amount you can contribute to the Traditional Care FSA is $3,400.
- All benefit eligible can participate, unless you or your spouse are contributing to an HSA
- Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
Limited Care FSA used with an HSA
- Employees who enroll in the $3,400-75% HSA plan or the $5,500-100% HSA plan may participate in the Limited Health Care FSA that allows you to use the money for dental and vision expenses only.
- The maximum amount you can contribute to the Limited Care FSA is $3,400.
- Examples of eligible health care expenses under the limited health care FSA may include:
- Deductibles and coinsurance not covered by dental or vision insurance
- Uninsured expenses such as eyeglasses, contact lenses or certain eye surgeries, orthodontics
Dependent Care FSA
- The maximum amount you can contribute to the Dependent Care FSA is $7,500 per year (or $3,750 if married and filing separately).
- This annual maximum applies to all contributions made by you and your spouse to a dependent care account.
- You can be reimbursed for any daycare expenses you have in a plan year if the expenses are necessary to allow you and your spouse to work.
- Eligible expenses may include:
- Before and after school programs
- Daycare
- Nursery school
- Summer day camps
How the Plan Works
You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. Any funds after March 31 unclaimed will be forfeited. To learn more, visit the FSA carrier website below.
Health Savings Account (HSA)
Health Savings Accounts (HSAs) are a great way to save money and budget for qualified medical expenses. HSAs are tax-advantaged savings accounts that accompany the $3,400-75% HDHP or the $5,500-100% HDHP. HDHPs offer lower monthly premiums in exchange for a higher deductible (the amount you pay before insurance kicks in).
What are the benefits of an HSA?
There are many benefits of using an HSA, including the following:
- It is portable. The money in your HSA is carried over from year to year and is yours to keep, even is you leave the company.
- It is a tax-saver. HSA contributions are made with pre-tax dollars. Since your taxable income is decreased by your contributions, you'll pay less in taxes.
- It works for you. Once you accumulate a minimum balance of $2,000 in your account, you can choose to invest money in various funds offered by Optum Bank.
The maximum amount you can contribute to an HSA in 2026 is $4,400 for individual coverage and $8,750 for family coverage.
Additionally, if you are age 55 or older, you may make an additional “catch-up” contribution of $1,000. You may change your contribution amount at any time throughout the year as long as you don’t exceed the annual maximum.
In order to contribute to an HSA, you must not:
- Be claimed as a dependent on someone else’s tax return.
- Have other health plan coverage that provides benefits covered under the HDHP Plan (for example, a Health Care Flexible Spending Account, a separate prescription drug plan, or a spouse’s non-HDHP plan).
- Have a spouse with a Health Care Flexible Spending Account that could reimburse your medical expenses.
- Be enrolled in a government health plan, such as Medicare or Medicaid.
- You must re-enroll in your HSA each year during open enrollment to participate in the plan.

Basic Life/AD&D
Elevate Living provides full-time employee or benefit eligible part-time employee scheduled to work at least 30 hours per week with basic term life and accidental death and dismemberment (AD&D) coverage at no cost to you.
The life benefit is payable to your designated beneficiary in the event of your death equal to $25,000. AD&D benefits are payable to you in the event of a dismemberment or an additional benefit is payable to your beneficiary if your death is the result of an accident.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Voluntary Life/AD&D
While Elevate Living offers basic life insurance, some employees may want to purchase additional life insurance coverage to continue to provide for their dependents after their death. With voluntary life insurance through Sun Life Financial, you are responsible for paying the full cost of coverage through bi-weekly payroll deductions. You can purchase coverage for yourself, your spouse, and/or your dependent child(ren).
- Voluntary Life Insurance
- Voluntary Accidental Death & Dismemberment (AD&D)
Employee
- Available in $10,000 increments ($20,000 minimum) with a maximum of $500,000 or 5x's your basic annual earnings.
- Guarantee issue of $100,000, all new hires are eligible to elect coverage up to this amount without requiring evidence of insurability.
Spouse
- Available in $5,000 increments ($10,000 minimum) with a maximum of $100,000 or 50% of the employee’s election, whichever is less.
- Guarantee issue of $25,000, newly eligible spouses are eligible to elect coverage up to this amount without requiring evidence of insurability.
Dependent Child(ren)
- Age 14 days to 6 months: Available as a flat $250 amount.
- Age 6 months to age 26: Available as a flat $20,000 amount.
- Newly eligible dependents are eligible to elect coverage up to this amount without requiring evidence of insurability.

Disability
Short and Long Term Disability
Disability insurance provides partial income replacement in the event an eligible employee incurs an illness/ injury that keeps them out of work for at least 7 days. Elevate Living provides full-time employees with both short- and long-term disability coverage. Without disability coverage, you and your family may struggle to get by if you miss work due to an injury or illness.
At Elevate Living, we want to do everything we can to protect you and your family. That’s why we pay for the full cost of short‐term and long‐term disability insurance.
Please note that you are not eligible to receive short-term disability benefits if you are receiving workers’ compensation benefits.
Employees must work a minimum of 30 hours per week to be eligible for this benefit. Newly hired employees will have a 6 month wait period to enroll in the Short Term Disability (STD) or Long Term Disability (LTD) benefit plans.
Short Term Disability (STD)
- Coverage amount: 66 2/3 % of weekly salary
- Maximum weekly benefit, non-manager/nonexecutive: $1,200/week..
- Elimination period: 7 days.
- Maximum duration: 12 weeks.
Long Term Disability (LTD)
- Coverage amount: 60% of your monthly salary.
- Maximum monthly benefit: $6,000.
- Elimination period: 90 days.
- Maximum duration: normal social security retirement age or length of continued disability.

Your EAP allows you someone to talk to and resources to consult whenever and wherever you need them. See below for some of the resources you have access to:
- Confidential Emotional Support—Speak with highly trained clinicians to help you and your family members with any issues, including anxiety, depression, grief, loss, life adjustments and relationship or marital conflicts.
- Work-Life Solutions—Specialists provide qualified referrals and resources for just about anything on your to-do list, such as: finding child and elder care, hiring movers or home repair contractors, planning events and locating pet care.
- Legal Guidance—Talk to an attorney for practical assistance with your most pressing legal issues, including divorce, adoption, family law, wills, trusts and more.

If you or your family are traveling more than 100 miles from home and have a medical or personal emergency, help is just a phone call away. By calling Assist America’s 24/7 Emergency Travel Assistance call center, you can receive the following:
- Medical consultation, evaluation and referral.
- Hospital admission guarantee.
- Assistance with unattended minor children.
- Lost prescription assistance.

Identity Theft Protection
Identity theft is a serious crime. Every year, millions of Americans become victims of identity theft and spend significant time and money to restore their records. This service, provided through Assist America’s SecurAssist® Identity Protection program, provides the following:
- 24/7 telephone support and guidance by anti-fraud experts.
- A dedicated expert caseworker who will help notify credit bureaus, file paperwork to correct credit reports, cancel stolen cards and request new ones.
- Proactive protection and alerts for up to 10 credit or debit cards per employee, using sophisticated web crawling technology that monitors underground chat rooms, where thieves are selling and trading stolen personal information.
Online Will Preparation
A will is the cornerstone of any estate plan and can protect your assets and loved ones. Through an easy-to-use secure website, you and your spouse can now create and download a will in about 20 minutes. This service includes the following:
- Step-by-step guidance and customization for your unique situation.
- Glossary of legal definitions.
- Ability to name an executor to carry out your wished and a guardian(s) to care for your children.
- Ability to create a living will (for additional fees).
- Ability to create a final arrangements document (for an additional fee).
Claimant Support Services
Losing a loved one or becoming disabled can be overwhelming to say the least. With Claimant Support Services, you have access to no-cost, objective financial planning, legal information and emotional support, if you or your family member has filed a claim with us. You can receive the following:
- Up to five telephonic professional counseling sessions per claim for legal, financial and emotional assistance.
- Assistance with topics such as inheritance taxes, loss of income, creditors and probate.
- Support dealing with trauma, loss and adjusting to a reduced quality of life, and other concerns.

Company Holidays
Regular full-time employees working thirty (30) or more hours per workweek are eligible for holiday pay. Elevate Living observes the following holidays:
New Year’s Day – January 1
Memorial Day – May 25
Independence Day – July 3
Labor Day – September 7
Thanksgiving Day – November 26
Friday after Thanksgiving – November 27
Christmas Day – December 25
In general, holidays that fall on a Saturday will be observed on the preceding Friday. Holidays that fall on a Sunday will be observed the following Monday.
Non-exempt employees must work their regularly scheduled shift immediately preceding and immediately following the holiday in order to receive holiday pay.
Paid Time Off (PTO)
Paid Time Off (PTO) provides benefit eligible employees with a flexible bank of paid time away from work that can be used for vacation, personal time, personal illness, care for dependents, or emergency circumstances. PTO is earned on a biweekly basis immediately upon hire based on length of service as outlined in the table below.
Length of Service Annual Accrual Rate
- 0 - 4 years 15 days (120 hours)
- 5+ years 20 days (160 hours)
The accrual rates listed above are based on a forty (40) hour workweek. Employees that are scheduled to work less than 40 hours will accrue PTO on a pro-rated basis.
At any given time, employees can have a maximum of 1x their annual accrual rate in their PTO bank. Once the maximum is reached, no further PTO will be accrued.
