Baldwin Supply Company provides you with Basic & Voluntary Life/AD&D coverage as well as Short Term Disability and Voluntary Long-Term Disability through Unum.
Basic Life and AD&D Insurance
Employees working 30 or more hours per week are provided with $50,000 in Basic Life and Accidental Death and Dismemberment coverage. Benefits reduce to 67% at age 70 and to 50% at age 75.
Make sure that your beneficiary is up to date. If you are unsure of who you currently have as your beneficiary, or need to update, contact Human Resources.
Voluntary Life and AD&D Insurance
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Voluntary Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Voluntary Life:
Employee: For this 2026 enrollment only, you may elect up to $200,000 without Evidence of Insurability or proof of good health, in increments of $10,000. If you enroll at a later date for Voluntary Life, you may be subject to Evidence of Insurability. This benefit may not exceed 5 times your Basic Annual Earnings.
Spouse: For this 2026 enrollment only, if you elect coverage for yourself, you may also elect up to $50,000 without Evidence of Insurability or proof of good health, in increments of $5,000. If you enroll your spouse at a later date for voluntary spouse life, you may be subject to Evidence of Insurability. This benefit may not exceed 100 percent of your employee voluntary life election.
Children: For children under 6 months of age, the life coverage you may elect is $1,000. For children older than 6 months of age, you may elect up to $10,000, in increments of $1,000.
AD&D
Employee: You may elect up to $500,000 in AD&D insurance for yourself, in increments of $10,000. This benefit may not exceed 5 times your Annual Earnings.
Spouse: If you elect coverage for yourself, you may also elect up to $250,000 in AD&D benefits for your spouse, in increments of $5,000. This benefit may not exceed 100 percent of your employee voluntary AD&D election.
Children: For children under 6 months of age, the life coverage you may elect is $1,000. For children older than 6 months of age, you may elect up to $10,000, in increments of $1,000.

Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Salary Continuation
Baldwin Supply Company provides Full Time employees with a Salary Continution Income benefit and pays the full cost of this coverage.
How it works:
- Employees file a Salary Continution claim with Unum, who approves the claim and provides updates. The weekly benefit will be calculated by Human Resources at Baldwin Supply Company.
- Salary Continution payments will be on the normal bi-weekly Baldwin Supply payroll with normal benefit and tax deductions taken out.
- There is a 7-day waiting period before Salary Continution payments begin, then Salary Continution pay will be issued for up to a maximum of 11 weeks.
- Employee must first use available PTO to cover that waiting period or unpaid time.
- Salary Continution leave runs concurrently with any FMLA approved leave.
Voluntary Long Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.
- LTD is a voluntary employee paid benefit through Unum.
- LTD is intended to protect your income for a long duration after you have depleted any Short Term Disability benefit.
- The plan replaces 60% of your salary, up to $7,500 per month (unless otherwise noted in your classification) after you have been disabled for 90 continuous days.
- Benefits continue until your Social Security Normal Retirement Age (SSNRA)*
*Depending on your age at the time of disability, your benefit may be subject to a different schedule. Refer to your plan documents for more specific details on benefit duration.
Long Term Disability Rates are based on employee age. Please refer to ADP Enrollment Event for detailed information on your cost for this coverage.
