Eligibility
Eligibility
If you are regularly scheduled to work 30 or more hours per week, you are eligible for all of the benefits listed.
New Hires And Newly Eligible Employees
New hires are able to join the plan on the first of the month following date of hire. Retirement benefits start after three months. If you are a new hire or have recently become eligible for benefits you must complete the insurance enrollment even if you are waiving all coverage. Company provided benefit coverage you will receive:
- Basic Life & AD&D and Short Term Disability Coverage
Eligible Dependents
You may elect certain coverage options for your eligible dependents. Eligible dependents include:
- Your legal spouse. This does not include a common law spouse or ex-spouse.
- Your (or your spouse’s) natural child, legally adopted child, stepchildren living with you, or children for whom you or your spouse is legal guardian and who are less than 26 years of age.
- Unmarried children incapable of self-support because of a total physical or mental disability.
Qualifying Events
You can change your benefit election during the year if you have a qualifying life event. Qualifying life events include, but are not limited to:
- Marriage, divorce or legal separation
- Birth or adoption of a child
- Change in child’s dependent status
- Death of a spouse, child or other qualified dependent
- Change in employment status or a change in coverage under another employer-sponsored plan
If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.
Paying For Your Benefits
You and Baldwin Supply Company share the cost of many of your benefits. Your contributions for medical, dental, vision and health savings contributions are paid on a pre-tax basis. In other words, your premium contributions are deducted from your paycheck before federal, state (if applicable) and FICA income taxes are withheld. This results in savings for you and your family. Your voluntary life contributions are paid for on an after-tax basis so you receive the full amount of the benefit should it be needed.
Enrollment
During your initial enrollment period, you will make decisions on all of your benefits through Baldwin Supply.
If you are a new hire or have recently become eligible for benefits and do not enroll when you are first eligible, you will only receive coverage for certain benefits. In this case, you must wait until the next Open Enrollment period to enroll for the coverage you want and need, unless you experience a qualifying event.
When you have reviewed the enrollment materials and are ready to enroll in your 2026 benefits, you will need to login to ADP and submit your 2026 benefits elections. Reference the attachment below for instructions for ADP Enrollment.
Medical
Baldwin Supply Company offers comprehensive medical plan options through BlueCross BlueShield of Minnesota (BCBSMN), utilizing the Aware provider network in Minnesota and the Blue Card PPO Network nationally.
The plans include two qualified High Deductible Health Plans (HDHP) which allow you to open a Health Savings Account (HSA). Refer to the below chart on a comparison of each plan.

Doctor On Demand
BCBSMN offers you a way to see a doctor or behavioral health professional from your smart phone, computer or tablet.
It’s available any time you need medical advice or treatment, including nights and weekends. Since a telehealth medical visit typically takes only about 10 minutes, you’ll have more time to spend doing the things that matter most to you! Cost is typically less than an in-person visit.
- Medical: up to $65.56

- Psychiatry: Up to $314.71 for first visit, up to $143.15 for follow-up visit

Retail Pharmacy (Up to 31 Day Supply)
When you fill a prescription at a participating retail pharmacy, you may purchase up to a 31-day supply of covered drugs. At the participating pharmacy, you will need to present your BCBS of MN ID card and make the required payment. Most major pharmacies are in the plans pharmacy network. Please note, CVS and Target CVS Pharmacies are considered a non-participating pharmacy.

The Preventive Rx Pack from BCBS offers you your maintenance medication at no cost to you. Examples of maintenance medication are, but not limited to:

BlueCross BlueShield of Minnesota (BCBSMN) provides access to a member app called Blue Care Advisor. Blue Care Advisor is tool to help you connect to all your benefits, claims, programs, rewards and more. If you currently have a login for the BCBSMN Member app, this will be the same login for Blue Care Advisor. You will just need to download the Blue Care Advisor app or login with the link below. Health Savings Account (HSA)
The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible for an HSA, you must be enrolled in a High Deductible Health Plan (HDHP).
You may contribute as follows:
- $4,400 for a single contract
- $8,750 for a two person or family
Additionally, if you are age 55 or older, you may make an additional “catch-up” contribution of $1,000. You may change your contribution amount at any time throughout the year as long as you don’t exceed the annual maximum.
Limits include both employee and employer contributions.
Employer Contribution
To encourage Wellness learning and participation in the Wellness Challenges, Baldwin will make company contributions into your personal Health Savings Account using the following eligibility policies.
- Must participate in BOTH Quarter 1 and Quarter 2 Wellness Challenges for a July contribution.
- Must participate in BOTH Quarter 3 and Quarter 4 Wellness Challenges for a January contribution.
- Must be an active employee on the date of contributions.
- New Hires only are eligible for a 50% contribution level if they missed the opportunity to participate in Quarter 1 or Quarter3 because of their hire date.
Baldwin will contribute $400 bi-annually into employees enrolled in Baldwin medical coverage at the employee only level.
Baldwin will contribute $800 bi-annually to employees enrolled in Baldwin medical coverage at the employee + children or spouse or family levels
Participation is encouraged for all employees regardless of medical coverage enrollment, but participating will not have an alternative financial reward.
What are the Benefits of an HSA?
- It is a Personal and Portable Account! Both your HSA contributions and Baldwin’s contributions become your personal funds to keep, even if you leave the company. All funds rollover year to year in the account.
- It’s a TRIPLE Tax-Saver! HSA contributions are made with pre-tax dollars. HSA funds are not taxed when you withdraw to pay medical expenses, and if you invest your HSA funds, the investment earnings are not taxed.
- Easy Payments of Medical Expenses! Pay your medical expenses directly with the Health Equity Visa Debit card issued to you.
- You can Invest your HSA Funds! Should you choose, Health Equity offers options to invest your funds. (as with any investments, there is no guarantee of performance) See HSA Investment Guide below or request one from HR.
Qualified HSA Expenses:

- Dental (including Orthodontia)
- Vision (including Lasik)
- Premiums: At 65 (Medicare), long-term care insurance, and COBRA insurance
- Dependent expenses, even if not covered by your insurance
- Over-the-counter medicines (allergy meds, ibuprofen, cough suppressant)
Health Equity Employee Portal Instructions:
- Click the "My Health Equity Employee Portal" link below.
- Click “Create username and password”.
- Enter the information requested on the “Find your account” screen.
- Enter the information asked for on the “Verify your identity” screen.
- On the “Set up your login” screen.
- Pick a user/login name of at least six characters with numbers and letters on the “Set up your login” screen.
- Choose a password of at least eight characters with an uppercase letter, a lowercase letter and a number.
- Follow password creation recommendations as listed in the log in screen.
- On the “Your email settings” screen, enter your email address.
- Click the box to agree to the terms of the web site and save the agreement.
HSA Investment Options:
Health Equity makes it easy to invest your HSA dollars. If you have a minimum of $2,000 in your HSA account, you may start investing your dollars. Here's how to access the HSA Investment Desktop:
- Log into your HealthEquity Member Account.
- Hover over "My Account" in the navigation bar.
- Select "Investments" from the dropdown menu.
Once inside, you have several options to choose and manage your investments.
- View portfolio performance and allocation

- Research fund options and historical performance
- Buy, sell and trade funds
- Automatically reinvest earnings and rebalance investments

2026 Wellness Challenges
Baldwin Supply Company is partnering with NFP to offer Wellness Challenges for 2026. NFP’s online challenges offer meaningful wellness initiatives that support the well-being of our employees. The best part is these challenges are suitable for all employees, and you can earn HSA dollars by participating.
Flexible Spending Account (FSA)
The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
You may contribute as follows:
Health Care FSA
- Up to $3,400 annually
- All benefit eligible can participate, unless you or your spouse are contributing to an HSA
- Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
Limited Care FSA
- Up to $3,400 annually
- Employees who enroll in either of Baldwin Supply Company's medical plans may participate in the Limited Health Care FSA
- Reimbursements allowed for unreimbursed dental, and vision expenses
Dependent Care FSA
- Up to $7,500 annually
- Limited to $3,750 if you are married and file separate tax returns
How the Plan Works
You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. If you do not use up all the FSA Medical dollars you elected to have deducted for the year, you will be able to carry over up to $660 from 2025 into 2026, all other remaining balance will be forfeited. However, FSA Dependent Care still has the “use it or lose it” rule and unspent dollars cannot be carried over into the next plan year.
Dental
Baldwin Supply Company partners with BlueCross Blue Shield of Minnesota to bring you a dental plan. The plan provide coverage for exams, X-rays, cleanings, fillings and major dental work such as crowns and bridges. You may use any dental provider, but your out-of-pocket cost will be less if you use an in-network provider.

*When you receive services from an out-of-network provider, you are responsible for the difference between what BlueCross BlueShield will reimburse and what the provider bills. BlueCross BlueShield will reimburse 90% of what providers in your area will charge.

Life & Disability
Baldwin Supply Company provides you with Basic & Voluntary Life/AD&D coverage as well as Short Term Disability and Voluntary Long-Term Disability through Unum.
Basic Life and AD&D Insurance
Employees working 30 or more hours per week are provided with $50,000 in Basic Life and Accidental Death and Dismemberment coverage. Benefits reduce to 67% at age 70 and to 50% at age 75.
Make sure that your beneficiary is up to date. If you are unsure of who you currently have as your beneficiary, or need to update, contact Human Resources.
Voluntary Life and AD&D Insurance
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Voluntary Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Voluntary Life:
Employee: For this 2026 enrollment only, you may elect up to $200,000 without Evidence of Insurability or proof of good health, in increments of $10,000. If you enroll at a later date for Voluntary Life, you may be subject to Evidence of Insurability. This benefit may not exceed 5 times your Basic Annual Earnings.
Spouse: For this 2026 enrollment only, if you elect coverage for yourself, you may also elect up to $50,000 without Evidence of Insurability or proof of good health, in increments of $5,000. If you enroll your spouse at a later date for voluntary spouse life, you may be subject to Evidence of Insurability. This benefit may not exceed 100 percent of your employee voluntary life election.
Children: For children under 6 months of age, the life coverage you may elect is $1,000. For children older than 6 months of age, you may elect up to $10,000, in increments of $1,000.
AD&D
Employee: You may elect up to $500,000 in AD&D insurance for yourself, in increments of $10,000. This benefit may not exceed 5 times your Annual Earnings.
Spouse: If you elect coverage for yourself, you may also elect up to $250,000 in AD&D benefits for your spouse, in increments of $5,000. This benefit may not exceed 100 percent of your employee voluntary AD&D election.
Children: For children under 6 months of age, the life coverage you may elect is $1,000. For children older than 6 months of age, you may elect up to $10,000, in increments of $1,000.
Rates are based on employee age. Please refer to ADP Enrollment Event for detailed information on your cost for this coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Salary Continuation
Baldwin Supply Company provides Full Time employees with a Salary Continution Income benefit and pays the full cost of this coverage.
How it works:
- Employees file a Salary Continution claim with Unum, who approves the claim and provides updates. The weekly benefit will be calculated by Human Resources at Baldwin Supply Company.
- Salary Continution payments will be on the normal bi-weekly Baldwin Supply payroll with normal benefit and tax deductions taken out.
- There is a 7-day waiting period before Salary Continution payments begin, then Salary Continution pay will be issued for up to a maximum of 11 weeks.
- Employee must first use available PTO to cover that waiting period or unpaid time.
- Salary Continution leave runs concurrently with any FMLA approved leave.
Voluntary Long Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.
- LTD is a voluntary employee paid benefit through Unum.
- LTD is intended to protect your income for a long duration after you have depleted any Short Term Disability benefit.
- The plan replaces 60% of your salary, up to $7,500 per month (unless otherwise noted in your classification) after you have been disabled for 90 continuous days.
- Benefits continue until your Social Security Normal Retirement Age (SSNRA)*
*Depending on your age at the time of disability, your benefit may be subject to a different schedule. Refer to your plan documents for more specific details on benefit duration.
Long Term Disability Rates are based on employee age. Please refer to ADP Enrollment Event for detailed information on your cost for this coverage.

Voluntary Benefits
Voluntary Critical Illness Insurance
Critical Illness, including cancer, impacts so many lives. Critical Illness can pay a lump-sum benefit at the diagnosis of a covered illness. Below are the benefit option available:
- Employees: You can choose $10,000, $20,000, or $30,000 of coverage, in increments of $10,000.
- Spouse: If you elect coverage for yourself, your spouse can elect 50% coverage of your coverage amount.
- Children: If you elect coverage for yourself, your child(ren) can elect 50% coverage of your coverage amount. An eligible child is defined as your child from birth to age 26.
*The Critical Illness plan has a 12/12 pre-existing conditions limitation. There will be no benefit payment for anything you have been treated for 12 months prior to coverage until you have been on the policy and treatment free for 12 consecutive months. Please see the benefit summary for full details of this provision.

Voluntary Accident Insurance
As we all know, accidents can happen at any time. Having an accident doesn’t just hurt you– it can also damage your finances. Your medical insurance will cover some of the expenses, but you’ll be left to foot the bills for your deductible. Accident insurance is designed to help meet out-of-pockets expenses and extra bills that can follow an accidental injury, whether minor or catastrophic. Coverage includes payments for emergency treatment and hospitalizations, as well as treatment for broken bones, lacerations, and burns. Our group accident insurance is an affordable way to make sure you can cover the gap between what your medical insurance covers and what you’d owe out-of-pocket if you or a family member were to get injured.
