Inside, you will find detailed information about your employee benefits program including benefit summaries, and direct links to the insurance carriers. You will also find some helpful information regarding benefit options so you can choose a benefit package that's right for you and your family.
Eligibility
If you are regularly scheduled to work 30 or more hours per week, you are eligible for all of the benefits listed.
New Hires And Newly Eligible Employees
New hires are able to join the plan on the first of the month following date of hire. Retirement benefits start after three months. If you are a new hire or have recently become eligible for benefits you must complete the insurance enrollment even if you are waiving all coverage. You can change your benefit election during the year if you have a qualifying life event. Qualifying life events include, but are not limited to:
If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.
Paying For Your Benefits
You and Baldwin Supply Company share the cost of many of your benefits. Your contributions for medical, dental, vision and health savings contributions are paid on a pre-tax basis. In other words, your premium contributions are deducted from your paycheck before federal, state (if applicable) and FICA income taxes are withheld. This results in savings for you and your family. Your voluntary life contributions are paid for on an after-tax basis so you receive the full amount of the benefit should it be needed.
Benefit Eligibility Overview
During your initial enrollment period, you will make decisions on all of your benefits through Baldwin Supply.
If you are a new hire or have recently become eligible for benefits and do not enroll when you are first eligible, you will only receive coverage for certain benefits. In this case, you must wait until the next Open Enrollment period to enroll for the coverage you want and need, unless you experience a qualifying event.
When you have reviewed the enrollment materials and are ready to enroll in your 2025 benefits, you will need to login to ADP and submit your 2025 benefits elections. Reference the attachment below for instructions for ADP Enrollment.
Baldwin Supply Company offers comprehensive medical plan options through BlueCross BlueShield of MN (BCBSMN), utilizing the Aware provider network in Minnesota and the Blue Card PPO Network nationally.
The plans include two qualified High Deductible Health Plans (HDHP) which allow you to open a Health Savings Account (HSA). Refer to the below chart on a comparison of each plan.

Doctor On Demand
BCBSMN offers you a way to see a doctor or behavioral health professional from your smart phone, computer or tablet.
It’s available any time you need medical advice or treatment, including nights and weekends. Since a telehealth medical visit typically takes only about 10 minutes, you’ll have more time to spend doing the things that matter most to you! Cost is typically less than an in-person visit.

Doctor On Demand is an independent company providing telehealth services.
Make sure your doctor and clinic are in your network before receiving care. This will make sure you receiver the highest level of benefits.

Retail Pharmacy (Up to 31 Day Supply)
When you fill a prescription at a participating retail pharmacy, you may purchase up to a 31-day supply of covered drugs. At the participating pharmacy, you will need to present your BCBS of MN ID card and make the required payment. Most major pharmacies are in the plans pharmacy network. Please note, CVS and Target CVS Pharmacies are considered a non-participating pharmacy.
Home Delivery from Amazon Pharmacy
As a Blue Cross and Blue Shield of Minnesota member, you have access to MedsYourWay, a prescription drug discount card program. With MedsYourWay you’ll have up to 80 percent savings on brand and generic medicines and it’s seamlessly built into the Amazon Pharmacy experience. Specialty Medication
If you are taking a medication considered “specialty” you will want to use a Specialty Pharmacy. These pharmacies are able to purchase these medications at a reduced rate and pass the savings on to you. Visit bluecrossmn.com to see a complete listing of participating pharmacies.

Ask for a Generic Version
Generic drugs are chemically equal to the brand-name medications. This means they work the same way — the same dosage, safety, strength, quality, the way it works, the way it is taken and the way it should be used — they are just less expensive. Speak with your doctor about your options.

BlueCross BlueShield provides access to a member app called Blue Care Advisor. Blue Care Advisor is tool to help you connect to all your benefits, claims, programs, rewards and more. If you currently have a login for the BCBSMN Member app, this will be the same login for Blue Care Advisor. You will just need to download the Blue Care Advisor app or login with the link below. The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible for an HSA, you must be enrolled in a High Deductible Health Plan (HDHP).

Additionally, if you are age 55 or older, you may make an additional “catch-up” contribution of $1,000. You may change your contribution amount at any time throughout the year as long as you don’t exceed the annual maximum.
Employer Contribution
To encourage Wellness learning and participation in the Wellness Challenges, Baldwin will make company contributions into your personal Health Savings Account using the following eligibility policies.
Baldwin will contribute $400 bi-annually into employees enrolled in Baldwin medical coverage at the employee only level.
Baldwin will contribute $800 bi-annually to employees enrolled in Baldwin medical coverage at the employee + children or spouse or family levels
Participation is encouraged for all employees regardless of medical coverage enrollment, but participating will not have an alternative financial reward.

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.

How the Plan Works
You must incur your eligible expenses during the plan year — January 1 to December 31. An expense is considered to be incurred when the service is performed, not when you are billed or pay for the service. You do, however, have until March 31st annually to file your claims. If you do not use up all the FSA Medical dollars you elected to have deducted for the year, you will be able to carry over up to $660 from 2024 into 2025, all other remaining balance will be forfeited. However, FSA Dependent Care still has the “use it or lose it” rule and unspent dollars cannot be carried over into the next plan year.
Baldwin Supply Company partners with BlueCross Blue Shield of Minnesota to bring you a dental plan. The plan provide coverage for exams, X-rays, cleanings, fillings and major dental work such as crowns and bridges. You may use any dental provider, but your out-of-pocket cost will be less if you use an in-network provider.

*When you receive services from an out-of-network provider, you are responsible for the difference between what BlueCross BlueShield will reimburse and what the provider bills. BlueCross BlueShield will reimburse 90% of what providers in your area will charge.

Baldwin Supply Company provides you with Basic & Voluntary Life/AD&D coverage as well as Short Term Disability and Voluntary Long-Term Disability through Sun Life.
Basic Life and AD&D Insurance
Employees working 30 or more hours per week are provided with $20,000 in Basic Life and Accidental Death and Dismemberment coverage. Benefits reduce to 67% at age 70 and to 50% at age 75.
Make sure that your beneficiary is up to date. If you are unsure of who you currently have as your beneficiary, or need to update, contact Human Resources.
Voluntary Life and AD&D Insurance
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Voluntary Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Voluntary Life:
Employee: During your initial eligibility, you may elect up to $200,000 without Evidence of Insurability or proof of good health, in increments of $10,000. If you enroll at a later date for Voluntary Life, you may be subject to Evidence of Insurability.
This benefit may not exceed 5 times your Basic Annual Earnings.
Spouse: During your initial eligibility, you elect coverage for yourself, you may also elect up to $50,000 without Evidence of Insurability or proof of good health, in increments of $5,000. If you enroll your spouse at a later date for voluntary spouse life, you may be subject to Evidence of Insurability.
This benefit may not exceed 100 percent of your employee voluntary life election.
Children: You may elect up to $10,000 in benefits for your dependent children.
AD&D
Employee: You may elect up to $500,000 in AD&D insurance for yourself, in increments of $10,000.
This benefit may not exceed 5 times your Basic Annual Earnings.
Spouse: If you elect coverage for yourself, you may also elect up to $250,000 in AD&D benefits for your spouse, in increments of $5,000.
This benefit may not exceed 100 percent of your employee voluntary AD&D election.
Children: You may elect up to $10,000 in benefits for your dependent children.

Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Salary Continuation
Baldwin Supply Company provides Full Time employees with a Salary Continution Income benefit and pays the full cost of this coverage.

Voluntary Long Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.

Voluntary Critical Illness Insurance
You may have medical insurance– But that doesn’t mean you’re covered for all of the expenses resulting from a serious illness. Critical Illness, including cancer, impacts so many lives. One of the greatest and most easily forgotten effects is the financial burden associated with such diagnosis. In addition to the cost of treatment, the time away from work can deplete your savings account in no time. Identity Theft Protection and Pre-Paid Legal Services are provided by LegalShield. The legal plan covers the employee, spouse and dependent children. For more information on these plans, please refer to the Benefit Summary posted below.
