Barker, Woltz and Lawrence is pleased to announce the 2021-2022 Open Enrollment period. This is the time each year when employees are encouraged to take the opportunity to re-evaluate current and future benefit needs. Understanding your 2021-2022 benefit plan option is important, so you are encouraged to review all information carefully. You may also want to visit the carrier websites to take advantage of the many tools and resources available to assist you in making a well informed decision.
This is the one time of year when you can select or make changes to your benefit choices. The selections you make during open enrollment go into effect January 15, 2021. Barker, Woltz and Lawrence benefit plan will continue in 2021-2022 with BCBS Plan 731CHC PPO 202 and the Blue Choice Network The following changes will go into effect January 15, 2021:
In Network Individual Deductible will increase to $2,500.
Out of Network Individual Deductible will increase to $5,000.
In Network Family Deductible will increase to $7,500.
Out of Network Family Deductible will increase to $15,000.
In Network Individual Out of Pocket will increase to $4,000.
In Network Family Out of Pocket will increase to $12,000.
In Network Primary Care Physician Copay will increase to $40 copay.
In Network Specialist Office Visit Copay will increase to $65 copay.
RX Changes
Non-Preferred Generic Drugs copay will increase to $20 Preferred and $30 Non-Preferred.
