Introduction

The Broadway National Annual Open Enrollment is your opportunity to learn about your Benefit Programs for the 2021-2022 plan year. We know healthcare is very important so we want you to take the opportunity to learn about the latest Health Care Program, review your current coverage and choose the best options for you and your family.

 

You will have the ability to view programs available to you and your family, and select plan options that will provide coverage from December 1, 2021 to November 30, 2022.

 

Enrollment starts on November 12th and you must make your Annual Enrollment elections before November 18, 2021. Review the Benefit Guide to learn more. We hope you and your family are staying safe and healthy.

Brief Summary of Medical Benefit Changes Effective December 1, 2021:


High PPO Plan

  • Increasing the in-network Out of Pocket Maximum (Individual/Family) to $6,000/$12,000 from $5,500 / $11,000
  • Increasing the in-network Coinsurance to 20% after the Deductible from 10% after the Deductible
  • Increase the Prescription Drug Retail copays (Tier 1 / Tier 2 / Tier 3) to $15/$40/$80 from $10/$35/$60
  • Increasing the out of network Deductible (Individual/Family) to $6,000/$12,000 from $5,000/$10,000
  • Increasing the out of network Out of Pocket Maximum to $15,000/$30,000 from $10,000/$20,000  


Mid EPO Plan

  • Increasing the in-network Out of Pocket Maximum to $6,500/$13,000 from $6,250/$12,500
  • Increasing the in-network Coinsurance to 20% after the Deductible from 10% after the Deductible
  • Increase the Prescription Drug Retail copays (Tier 1 / Tier 2 / Tier 3) to $15/$40/$80 from $10/$35/$60

 

Low HDHD EPO HSA Plan

  • Increase the Prescription Drug Retail copays (Tier 1 / Tier 2 / Tier 3) after the Medical Deductible to $15/$40/$80 from $10/$35/$60


No Benefit Changes to the Dental, Vision, or Life/AD&D plans.

How To Enroll

  • Actively enroll by November 18, 2021
  • If you'd like to change your existing coverage or make new elections, you must do so by November 18th. Your current medical, dental and vision elections will carry forward to the new plan year if you don't make a new election
  • Complete the PPI Enrollment form if you are enrolling for the first time or adding/dropping any dependents or now waving coverage
  • You must complete the election form located below and return it to Kayla Graffia by November 18th
