Welcome to Benefits Enrollment!

Bargain Center is pleased to announce the open enrollment period will begin February 14, 2023 and will end on February 21, 2023. This is the time each year when employees are encouraged to take the opportunity to re-evaluate current and future benefit needs. Understanding your 2023-2024 benefit plan options is important, so you are encouraged to review all information carefully. You may also want to visit the carrier websites to take advantage of the many tools and resources available to assist you in making a well-informed decision.

Thank you!

2023 Bargain Center Enrollment Forms

2023 Mutual of Omaha Enrollment/Change Form

2023 Mutual of Omaha Evidence of Insurability Questionnaire

Please print the 2023 Bargain Center Enrollment form and complete, sign, date and return to your HR Department no later than February 21, 2023. Please note, if you wish to not make any election changes for you and your dependents, you can mark the box that states "I want to keep my current (Medical, Dental, Vision or Life) enrollment the same." This is available for each line of coverage. Please note, if you choose to keep you and your dependents current enrollment the same, we will not access the file, the enrollment will continue as is.

If you wish to enroll in Voluntary Life & AD&D and did not enroll yourself or dependents when you were first eligible for coverage or are increasing yours or your dependents VTL & AD&D benefit, you will need to complete a Mutual of Omaha Enrollment Form and a Mutual of Omaha's Evidence of Insurability questionnaire via Mutual of Omaha's website. The location and directions to Mutual of Omaha EOI website is included in the Mutual of Omaha EOI.  It is your responsibility to complete the questions and submit to Mutual of Omaha's Underwriting Team for review and either approve or decline. If you do not submit the information, the coverage will not be issued. Please be sure to include all information requested on the form.

If you are currently enrolled in the Voluntary Life/AD&D and wish to increase your benefits, you can increase your benefit by 4 increments of $10,000 each ($40,000 total), up to the guaranteed issue amount of $100,000 without having to complete an Evidence of Insurability Form. This benefit is for employees only. It is not available for dependents. If you wish to purchase more than the guaranteed issue amount, an Evidence of Insurability Form must be completed and submitted to Mutual of Omaha for review and approval.

Please turn the completed enrollment form into Human Resources.
