How to Enroll

The Benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.

You can, however, modify your elections under certain circumstances, called "Qualifying Events". These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Full time and Part-time employees are eligible to participate in the plan. Aztec will contribute up to $275 a month per full time employee and up to $150 a month per part time employee. Your coverage will be effective on the 1st of the month, following 60 days from your date of hire.

All employees should complete an election form to confirm their benefit elections that will be effective 12-1-2022. CLICK TO DOWNLOAD THE ELECTION FORM (on the right).

Preventative Care

ASi MEC Plus (Minimum Essential Coverage)

This is the part of your combo that covers your preventative care at 100%. When you enroll in Sedera you are automatically enrolled in this plan.

You will receive an ID card for this plan.

PHCS network search instructions:

●Go to

●Click on Find a Provider in the upper right corner

●Click on the green Select Network button & choose PHCS

●Click the green Select Network box

●Choose PHCS in the pop-up box

●Select Preventive Services Only

  • In order to make sure that everything is coded for Preventative, do not use this time to ask about a recent back issue or other problem. Reserve that for a separate doctor's appointment or the code will shift to "diagnostic" and will result in balance billing.
  • Remember that this plan does not cover any services billed by a hospital. Mammograms, colonoscopies, and labs need to be done in a freestanding facility.
