Welcome

Helping you and your family reach and maintain good health is very important to us. Arthur Chapman is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.



Eligibility

Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Arthur Chapman. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

How To Enroll In Benefits

Open Enrollment: February 27th - March 10th, 2023


We are pleased to offer our employees an easy to use, online enrollment tool called Ease. Please use the link below to access your enrollment.


2023 Benefit Updates:

  • The medical and dental plans will be moving to HealthPartners.
  • No changes to rates or plan design on vision, life/AD&D and disability plans.
  • All benefits will begin on April 1, 2023 and go until March 31, 2024.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.


Refer to your 2023-24 Employee Benefits Guide or contact Human Resources if you have questions about qualifying events.

Medical

HealthPartners

Arthur Chapman offers two medical plans - the $1,500 Deductible plan with HRA and the $3,000 Deductible plan with HSA - both plans are available through HealthPartners. The medical plans offered to Arthur Chapman employees utilizes the Open Access Network which is an open access provider through HealthPartners. You are also covered by the Cigna network when you travel outside of Minnesota.


Both plans include prescription drug coverage. Be sure to check the provider websites to find doctors, use the tools and get the latest information.


HealthPartners Member Portal

Want to find a doctor in your network? Or wondering how much you’ll have to pay for that last visit? You can access this information all in one place. Have your member ID card available and sign up to get started by visiting the website below or use the myHP mobile app.


Membership allows you to find a doctor in your network, view claims, see benefits and coverage details, get support with your health goals and print member ID cards.


Health Reimbursement Account (HRA) Contributions

The HRA is employer-funded and available to you only while employed at the Firm and enrolled in the Firm’s HealthPartners HRA medical plan. Your HRA funds offset deductible expenses only. The funds are automatically applied to your deductible expenses by HealthPartners and paid directly to your provider. You do not need to file a claim to utilize HRA funds. Unused HRA funds will carry-over to the following plan year, but your funds will not exceed the plan year deductible maximum of $1,500 single and $3,000 family. 


Doctor On Demand

With Doctor On Demand, Board-certified doctors are available 24/7, 365 days a year to treat many common medical conditions. You can also schedule next-day appointments to see licensed psychologists and psychiatrists between the hours of 7 a.m. to 10 p.m., local time.

Doctor On Demand services include treatment for common medical conditions such as:

  • Cold & flu
  • Sore throat
  • Skin issues/rashes
  • Abdominal pain
  • Allergies
  • Travel illness
  • Ear infection
  • Migraines


Virtuwell

With Virtuwell you can receive 24/7 care from home, work or even in line for coffee. Visit the Virtuwell website below, answer a few questions and in 30 minutes or less a board-certified nurse practitioner emails and texts you your treatment plan, including any prescriptions.


You are only charged if they can treat you. Unlimited follow up calls are free. Visit the HealthPartners Virtuwell website below and use your member ID card to check your cost.

CareLine

When you need answers to health related questions or advice on how to best deal with an illness, injury or medical emergency, one phone call will link you to a registered nurse available 24 hours a day, 7 days a week. CareLine is free to all members. Call 612-339-3663 or 800-551-0859.


BabyLine Phone Services

BabyLine focuses on the special needs of expectant and new parents up to six weeks after delivery. Learn about healthy eating during pregnancy and which over-the-counter medications are safe. Available 24/7. Call 612-333-BABY (2229) or 800-845-9297.


HealthPartners Nurse Navigator Program

When you need help navigating complex health and insurance issues, call one of the HealthPartners Nurse Navigators. They can help guide you through difficult decisions to ensure you make the right choice while getting the most from your medical coverage. Wellbeats

Wellbeats is an on-demand fitness platform with 450+ workouts for all ages, abilities, and interests. No matter where you are in your fitness journey, Wellbeats has a starting place for you. With workouts for all ages from toddlers to seniors, Wellbeats can be used by all members of the family. Mindfulness courses, office breaks, and no equipment options are also included. Get started by signing in to your HealthPartners online account and selecting the Living Well tab.


Building Emotional Resilience Courses

These are short, three-session courses. Learn to replace negative self-talk, improve your listening skills and more. Find all the courses by visiting the link below.


Behavioral Health Navigators

Mental health and chemical health concerns are sensitive issues that you don’t want to discuss with just anyone. Our Behavior Health Navigators are staffed with professionals who can help answer questions and guide you through your behavioral health network, benefits and services. Discount Program

HealthPartners provides some free perks with their Healthy Discounts Program. Simply give your HealthPartners member ID card to participating retailers to receive Healthy Discounts on exercise equipment, classes, snowboard and ski equipment, spa, pet insurance and wellness services and much more. Condition Management—Personal Nurse Support

Living with a health condition is easier when you have a team of people to support you. Get support for conditions like asthma, depression, diabetes, heart disease and rheumatoid arthritis. Nurses can help with questions and resources about your condition, discuss how treatment is going, navigate the health system and your benefits, work with your doctor and more. Tobacco Cessation Program

Quitting tobacco and vape may be one of the hardest things you’ll ever do. Through HealthPartners you can get help from a health coach who will help you set goals and give you support and encouragement to reach those goals. Sign up with a health coach at 800-311-1052. In addition to a health coach, your health plan may help pay for medicines to help you quit. Superior Vision

Arthur Chapman offers vision coverage through Superior Vision to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the Superior Vision Network. Review your coverage details in your benefit guide.

Life and AD&D

Unum

Arthur Chapman provides you with Basic & Life/AD&D coverage through Unum at no cost to you.


Employees working 30 or more hours per week are eligible for 1.5x your annual salary to a maximum of $300,000 in group life and accidental death and dismemberment (AD&D) insurance. Arthur Chapman pays for the full cost of this benefit - meaning you are not responsible for paying any monthly premiums.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as divorce or birth of a child.

Voluntary Life and AD&D

Unum

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


Employee

  • Available in $10,000 increments with a maximum of $100,000 or 5x your basic annual earnings.
  • Guarantee issue of $100,000.


Spouse

  • Available in $10,000 increments with a maximum of $30,000.
  • Guarantee issue of $30,000.


Dependent Child(ren)

  • Birth to 6 months: $1,000
  • 6 months to age 19 or 26 is Full-Time Student: $10,000

Disability

Unum

Arthur Chapman provides you with Short Term and Long Term Disability coverage through Unum at no cost to you.


Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You are eligible for short term disability following 1 year of employment.


Benefits

  • Coverage Amount: 60% of your weekly earnings
  • Maximum Weekly Benefit: $500/week
  • Elimination Period: 14 days
  • Maximum Duration: 11 weeks


Long Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. 


Benefits

  • Coverage Amount: 60% of your monthly earnings
  • Maximum Monthly Benefit: $10,000
  • Elimination Period: 90 days
  • Maximum Duration: 2-year own occupation or 20% earnings loss

Employee Assistance Program (EAP)

Work/Life Balance: The EAP is provided to all employees through Unum. Employees have access to master level consultants 24 hours a day, 365 days a year. The EAP services are designed to help you be healthier and more productive by assisting you with problems both at work and at home.

Worldwide Emergency Travel Assistance

The Worldwide Emergency Travel Assistance program helps you obtain quality care when you have a health emergency while traveling 100 miles or more from home or in another country. Unum partners with Assist American to provide this service. The program can also assist with replacing lost or stolen prescriptions and passports, legal and interpreter referrals and much more. Before traveling, ask Human Resources for your travel assistance wallet card.

SuperiorUSA

If you have enrolled in the Aware $3,000 High Deductible Health plan with HSA, you may be eligible to open a Health Savings Account (HSA) through SuperiorUSA. Health Savings Accounts are a great way to save money and pay for qualified medical expenses.


HSA's are tax-advantaged savings accounts that accompany high deductible health plans (HDHPs). HDHPs offer lower monthly premiums in exchange for a higher deductible (the amount you pay before insurance kicks in).


What are the benefits of an HSA?

  • It saves you money. HDHPs have lower monthly premiums, meaning less money is being taken out of your paycheck.
  • It is portable. The money in your HSA is carried over from year to year and is yours to keep, even if you leave the company.
  • It is a tax-saver. HSA contributions are made with pre-tax dollars. Since your taxable income is decreased by your contributions, you'll pay less in taxes.


Important HSA Rules

To be eligible for our HSA, you must be enrolled in a High Deductible Health Plan through Arthur Chapman. In addition, in order to contribute, you must NOT:

  • Be claimed as a dependent on someone else's tax return.
  • Have other health plan coverage that provides benefits covered under the HDHP Plan (for example, a Health Care Flexible Spending Account, a separate prescription drug plan or a spouse's non-HDHP plan).
  • Have a spouse with a Health Care Flexible Spending Account that could reimburse your medical expenses.
  • Be enrolled in a government health plan, such as Medicare or Medicaid.
  • You must re-enroll in your HSA each year during open enrollment to participate in the plan.


If you are age 55 or older you may make an additional "catch-up" contribution of $1,000.


Please note: Dependent health care costs are qualified expenses under your HSA plan, as long as they are claimed as your tax dependent. After age 19, when they are no longer considered your tax dependent, their medical costs will no longer be considered a qualified expense.

SuperiorUSA

Flexible Spending Accounts (FSAs) are voluntary accounts that allow you to set aside pre-tax money for certain health and dependent care expenses. These are separate accounts with separate purposes, and therefore the money in one account cannot be used to satisfy expenses in the other. As an Arthur Chapman employee, you can choose to participate in one or all of these accounts depending on your needs. The Flexible Spending plan is administered by SuperiorUSA Benefits.


When you establish an FSA, you will be able to choose the annual amount you want to contribute to the account, up to certain plan limits. This amount is deducted from your paycheck in equal installments before Federal Security taxes are withheld. FSAs are a great way to save money, but keep in mind, they are "use it or lose it" provisions for each account, which means you will forfeit any balance that is left in your FSA at the end of the year.


You do not have to be enrolled in the Firm's medical, dental or vision plans to be eligible to participate in the FSA, however, if you are enrolled in the HRA medical plan through the Firm be sure to factor the employer-fund HRA dollars when estimating how much pre-tax dollars should be set aside.


Health Care Flexible Spending Account

Employees who are enrolled in the $1,500 deductible plan and not enrolled in a Health Savings Account (HSA) or High Deductible Health Plan (HDHP) may participate in the Health Care Flexible Spending Account. 


You can elect up to $3,050 in the Health Care Flexible Spending Account for 2023. You can use this money to pay for eligible medical, dental, and vision expenses you or your dependents incur during the plan year. 


Limited Health Care Flexible Spending Account

The Limited Health Care Flexible Spending Account is for all employees who contribute, or have a spouse who is contributing to a Health Savings Account or High Deductible Health Plan (includes expenses for dental, orthodontic and vision care only).


You can elect up to $3,050 in the Limited Health Care Flexible Spending Account for 2023. 


Dependent Care Flexible Spending Account

This annual maximum applies to all contributions made by you and your spouse to a dependent care account. You can be reimbursed for any daycare expenses you have in a plan year if the expenses are necessary to allow you and your spouse to work. 


Eligible expenses may include:

  • Before and after school programs
  • Daycare
  • Nursery school
  • Summer day camps



The maximum amount you can contribute to the Dependent Care FSA is $5,000 per year (or $2,500 if married and filing separately).

Voluntary Options

Arthur Chapman provides eligible employees with the opportunity to participate in a variety of voluntary benefits.


Accident Insurance

Accident insurance can pay you money based on the injury and the treatment you receive, whether it’s a simple sprain or something more serious, like an injury from a car accident. The Accident Insurance Plan can pay you a benefit for an emergency room treatment, stitches, crutches, injury-related surgery and a list of other accident-related expenses. You can also purchase coverage for your spouse and dependent children. This plan includes a $50 wellness benefit per insured, per year.


Critical Illness

Critical Illness, including cancer, impacts so many lives. Critical Illness pays a lump-sum cash benefit that can help with expenses if you or a covered family member is diagnosed with a covered condition which includes: 

  • Heart Attack
  • Major organ failure
  • End-stage kidney failure
  • Benign brain tumor
  • Coronary artery bypass surgery (pays 25% of lump sum)
  • Invasive cancer
  • Stroke
  • Paralysis


This plan also includes a $50 wellness benefit per insurance, per year. The plan has a 12/12 pre-existing limitation. Lookback period is 12 months and must be treatment free for 12 consecutive months.


Hospital Insurance

Hospital Insurance helps covered employees and their families cope with the financial impacts of a hospitalization. You can receive benefits you’re admitted the hospital for a covered accident, illness or childbirth. Coverage includes: 

  • $1,500 for each covered hospital admission - once per year
  • $100 for each day of your covered hospital stay, up to 60 days


You can also purchase coverage for your spouse and dependent children.

Identity Theft

Arthur Chapman offers you the opportunity to enroll in an Identity Theft Protection plan through IDShield.


The IDShield membership includes:

  • Privacy monitoring: Name, SSN, date of birth, email address, phone numbers, drivers license and passport numbers along with your medical ID numbers.
  • Security Monitoring: SSN, credit cards and bank account monitoring, sex offender search, financial activity alerts and quarterly credit score tracking.
  • Consultation: Includes 24/7/365 life support for covered emergencies, unlimited counseling, alerts and data breach notifications.
  • Full Service Restoration: Complete identity recovery services by Kroll Licensed Private Investigators, it will restore your identity to its pre-theft status.


The IDShield covers the employee and their family. A family rate covers the spouse and up to 8 dependents up to the age of 18.
