We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Arthur Chapman. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

Open Enrollment
Open Enrollment: March 5th, 2026 - March 13th, 2026
2026 Benefit Updates:
- On HDHP with HSA, the single and family deductible is increasing ($3,400/$6,800) due to IRS regulations.
- IRS increased HSA Limits: Single increased to $4,400. Family Increased to $8,750.
- IRS increased FSA Limits: Healthcare FSA increased to $3,400. Dependent care increased to $7,500.
- Changes to Dental Premiums effective 4/1/2026.
- Termination of STD plan due to MN Paid Leave private plan through Unum effective 1/1/2026.
- Virtual Care Updates
- Virtuwell is covered at 100% on both medical plans.
- Teladoc Health is a virtual care program newly offered in 2026 on both medical plans.

You can modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

Medical
Arthur Chapman offers two medical plans - the $1,500 Deductible plan with HRA and the $3,400 Deductible plan with HSA - both plans are available through HealthPartners. The medical plans offered to Arthur Chapman employees utilizes the Open Access Network which is an open access provider through HealthPartners. You are also covered by the Cigna network when you travel outside of Minnesota.
Both plans include prescription drug coverage. Be sure to check the provider websites to find doctors, use the tools and get the latest information.
HealthPartners Member Portal
Want to find a doctor in your network? Or wondering how much you’ll have to pay for that last visit? You can access this information all in one place. Have your member ID card available and sign up to get started by visiting the website below or use the myHP mobile app.
Membership allows you to find a doctor in your network, view claims, see benefits and coverage details, get support with your health goals and print member ID cards.
Health Reimbursement Account (HRA) Contributions
The HRA is employer-funded and available to you only while employed at the Firm and enrolled in the Firm’s HealthPartners HRA medical plan. Your HRA funds offset deductible expenses only. The funds are automatically applied to your deductible expenses by HealthPartners and paid directly to your provider. You do not need to file a claim to utilize HRA funds. Unused HRA funds will carry-over to the following plan year, but your funds will not exceed the plan year deductible maximum of $1,500 single and $3,000 family.
Doctor On Demand
With Doctor On Demand, Board-certified doctors are available 24/7, 365 days a year to treat many common medical conditions. You can also schedule next-day appointments to see licensed psychologists and psychiatrists between the hours of 7 a.m. to 10 p.m., local time.
Doctor On Demand services include treatment for common medical conditions such as:
- Cold & flu
- Sore throat
- Skin issues/rashes
- Abdominal pain
- Allergies
- Travel illness
- Ear infection
- Migraines

Virtuwell
With Virtuwell you can receive care from home, work or even in line for coffee. Visit the Virtuwell website below, answer a few questions and in 30 minutes or less a board-certified nurse practitioner emails and texts you your treatment plan, including any prescriptions. Virtuwell visits are covered 100% for those enrolled in either health plan.

- Monday through Friday, 7 a.m. to 8 p.m.
- Saturday, Sunday, and holidays, 8 a.m. to 5 p.m.
- Available anywhere in the U.S. to residents of AZ, CA, CO, CT, IA, MI, MN, NY, ND, PA, SD, VA and WI.
Teladoc
Fill out a brief medical history to connect with medical experts by phone, video or mobile app. Whether it's a prescription sent to the pharmacy of your choice, the guidance to move forward or a review of a preexisting condition, they're ready to help.
Pharmacy
Employees who have elected medical coverage receive prescription drug coverage through HealthPartners Pharmacy Program. You can purchase prescriptions through a national network of retail pharmacies or through the mail order program. CVS and Target pharmacies are in the HealthPartners retail pharmacy network.
Please note, as health plan, HealthPartners will no longer cover weight loss drugs. For the specific formulary list, please log on to your secure member portal to check your pharmacy benefits.

Kavira
Kavira offers a better healthcare experience for you and your family. Kavira's experienced team of clinicians provide same day primary and urgent care via home visits and telehealth. No more needing to leave your home to see a doctor, all care is provided through house visits or through the mobile app.
Members can do everything online or in the app, including messaging, scheduling house visits, requesting prescription refills, and videochatting with Kavira providers. A Kavira provider will work with you to assess your symptoms and answer any questions you may have.

- Kavira services are covered for spouses and dependents to age 26. The Employee must be enrolled in the medical plan to receive Kavira services, but the spouse and dependent do not have to be enrolled in the medical plan.

When you need help navigating complex health and insurance issues, call one of the HealthPartners Nurse Navigators. They can help guide you through difficult decisions to ensure you make the right choice while getting the most from your medical coverage. Wellbeats
Wellbeats is an on-demand fitness platform with 450+ workouts for all ages, abilities, and interests. No matter where you are in your fitness journey, Wellbeats has a starting place for you. With workouts for all ages from toddlers to seniors, Wellbeats can be used by all members of the family. Mindfulness courses, office breaks, and no equipment options are also included. Behavioral Health Navigators
Mental health and chemical health concerns are sensitive issues that you don’t want to discuss with just anyone. Our Behavior Health Navigators are staffed with professionals who can help answer questions and guide you through your behavioral health network, benefits and services. HealthPartners provides some free perks with their Healthy Discounts Program. Simply give your HealthPartners member ID card to participating retailers to receive Healthy Discounts on exercise equipment, classes, snowboard and ski equipment, spa, pet insurance and wellness services and much more. Condition Management—Personal Nurse Support
Living with a health condition is easier when you have a team of people to support you. Get support for conditions like asthma, depression, diabetes, heart disease and rheumatoid arthritis. Nurses can help with questions and resources about your condition, discuss how treatment is going, navigate the health system and your benefits, work with your doctor and more. Tobacco Cessation Program
Quitting tobacco and vape may be one of the hardest things you’ll ever do. Through HealthPartners you can get help from a health coach who will help you set goals and give you support and encouragement to reach those goals. Sign up with a health coach at 800-311-1052. In addition to a health coach, your health plan may help pay for medicines to help you quit. Dental
Arthur Chapman offers a dental plan administered by HealthPartners. The plan provides coverage for exams, X-rays, cleanings, fillings and major dental work such as crowns and bridges. You may use any dental provider, but your out-of-pocket cost will be less if you use an in-network provider. Dental Summary
All deductibles and out-of-pocket maximums will accumulate on a Calendar Year basis (January 1 to December 31). This is a summary of benefits only. Complete details, limitations and exclusions will be provided in the certificate of coverage provided when enrolled. In the event of a conflict between this benefit summary and the contract, the contract will apply.
Benefit Level 1: Features highly respected clinics within the Twin Cities and St. Cloud that deliver quality and cost savings. This tier includes more than 300 dentists, including the HealthPartners Dental Group.
Benefit Level 2: Includes more than 2,600 dentists throughout Minnesota and bordering communities and more than 130,000 PPO providers nationwide.
Little Partners Dental Benefit
Children 12 and younger are covered 100% at in-network dentists, with no deductibles, no coinsurance and no annual maximum.

Vision
Arthur Chapman offers vision coverage through Superior Vision to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the Superior Vision Network. Review your coverage details in your benefit guide.

Life and AD&D
Arthur Chapman provides you with Basic & Life/AD&D coverage through Unum at no cost to you.
Employees working 30 or more hours per week are eligible for 1.5x your annual salary to a maximum of $300,000 in group life and accidental death and dismemberment (AD&D) insurance. Arthur Chapman pays for the full cost of this benefit - meaning you are not responsible for paying any monthly premiums. Benefits reduce to 50% of your original amount at age 70.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as divorce or birth of a child.

Voluntary Life and AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Employee
- Available in $10,000 increments with a maximum of $100,000 or 5x your basic annual earnings.
- Guarantee issue of $100,000.
Spouse
- Available in $10,000 increments with a maximum of $30,000.
- Guarantee issue of $30,000.
Dependent Child(ren)
- Birth to 6 months: $1,000
- 6 months to age 19 or 26 is Full-Time Student: $10,000

Disability
Arthur Chapman provides you with Long Term Disability coverage through Unum at no cost to you.
New for 2026: Arthur Chapman no longer offers the Short-Term Disability plan due to the implementation of MN Paid Leave private plan through Unum.
Long Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.
Benefits
- Coverage Amount: 60% of your monthly earnings
- Maximum Monthly Benefit: $10,000
- Elimination Period: 90 days
- Maximum Duration: 2-year own occupation or 20% earnings loss

Minnesota Paid Leave
Arthur Chapman offers Minnesota Paid Leave through an equivalent private plan insured by Unum. This is in place of the state program. This plan provides benefits that meet or exceed Minnesota’s Paid Leave requirements and offers eligible Minnesota employees paid time off for qualifying medical and family leave events, including bonding, caregiving, and personal medical needs.
What can I use Paid Leave for?
Medical Leave:
- To care for your own serious health condition, including care related to pregnancy, childbirth, and recovery
Family Leave:
- Bonding Leave – to care for and bond with a new child welcomed through birth, adoption, or foster placement
- Caring Leave – to care for a family member with a serious health condition
- Military Family Leave – to support a family member called to active duty
- Safety Leave – to respond to issues related to domestic violence, sexual assault, or stalking for yourself or a family member
Generally, conditions must last at least seven days and be certified by a healthcare provider or other professional.
Am I covered by Paid Leave?
Most workers in Minnesota are covered by Paid Leave. You are covered no matter the size of your employer, or the hours or days you work. Independent contractors and self-employed individuals are not automatically covered but may opt in. You may qualify for payments if you’ve been paid a minimum amount for work in Minnesota in the last year ($3,900 for the start of Paid Leave in 2026).
How long can I take leave?
Arthur Chapman's equivalent plan through Unum must offer leave time that is equal to or greater than what is offered under the state plan. You may qualify to take up to 12 weeks of family or medical leave per benefit year. If you need both family and medical leave in the same benefit year, you may qualify for up to 20 weeks in total.
How much will I get paid?
Arthur Chapman's equivalent plan must offer payments that are equal to or greater than what is offered under the state plan. You will be paid up to 90% of your wages, based on your income level, with a maximum weekly amount set at the state’s average weekly wage. This amount changes each year and is $1,423 for the start of Paid Leave in 2026.
What are my employment protections?
- Job protections: Generally, you must be restored to your job or an equivalent position when returning from leave. Job protections take effect 90 days after your date of hire.
- Health insurance continuation: Generally, employers must continue to fund their portion of healthcare insurance premiums while you are on leave.
- No retaliation or interference: Employers must not interfere with or retaliate against you if you apply for or use Paid Leave. Employers cannot take your Paid Leave payments.

Your Personalized Legal Center also contains state specific templates for power of attorney, trusts, estate planning, and more as well as access to a legal library. You will be able to create your legal form or document instantly and ready it for printing. Educational content, definitions, and help prompts are available for each form or document. Your documents can be stored on a secure server and accessible 24.7 via a unique username and password.

FSA
Flexible Spending Accounts (FSAs) are voluntary accounts that allow you to set aside pre-tax money for certain health and dependent care expenses. These are separate accounts with separate purposes, and therefore the money in one account cannot be used to satisfy expenses in the other. As an Arthur Chapman employee, you can choose to participate in one or all of these accounts depending on your needs. The Flexible Spending plan is administered by SuperiorUSA Benefits.
When you establish an FSA, you will be able to choose the annual amount you want to contribute to the account, up to certain plan limits. This amount is deducted from your paycheck in equal installments before Federal Security taxes are withheld. FSAs are a great way to save money, but keep in mind, they are "use it or lose it" provisions for each account, which means you will forfeit any balance that is left in your FSA at the end of the year.
You do not have to be enrolled in the Firm's medical, dental or vision plans to be eligible to participate in the FSA, however, if you are enrolled in the HRA medical plan through the Firm be sure to factor the employer-fund HRA dollars when estimating how much pre-tax dollars should be set aside.
Health Care Flexible Spending Account
Employees who are enrolled in the $1,500 deductible plan and not enrolled in a Health Savings Account (HSA) or High Deductible Health Plan (HDHP) may participate in the Health Care Flexible Spending Account.
You can elect up to $3,400 in the Health Care Flexible Spending Account for 2026. You can use this money to pay for eligible medical, dental, and vision expenses you or your dependents incur during the plan year.
The plan includes a carryover provision, that allows employees to carry up to $680 of unused Health Care FSA dollars into 2027. Any balance remaining will be forfeited.
Limited Health Care Flexible Spending Account
The Limited Health Care Flexible Spending Account is for all employees who contribute or have a spouse who is contributing to a Health Savings Account or High Deductible Health Plan (includes expenses for dental, orthodontic and vision care only).
You can elect up to $3,400 in the Limited Health Care Flexible Spending Account for 2026.
The plan includes a carryover provision, that allows employees to carry up to $680 of unused Health Care FSA dollars into 2027. Any balance remaining will be forfeited.
Dependent Care Flexible Spending Account
This annual maximum applies to all contributions made by you and your spouse to a dependent care account. You can be reimbursed for any daycare expenses you have in a plan year if the expenses are necessary to allow you and your spouse to work. Dependent care expenses such as day care and after school programs for children under age 13 or elder care expenses, so you and your spouse can work or attend school fulltime.
Eligible expenses may include:
- Before and after school programs
- Daycare
- Nursery school
- Summer day camps
The maximum amount you can contribute to the Dependent Care FSA is $7,500 per year (or $3,750 if married and filing separately).
HSA
If you have enrolled in the Aware $3,400 High Deductible Health plan with HSA, you may be eligible to open a Health Savings Account (HSA) through SuperiorUSA. Health Savings Accounts are a great way to save money and pay for qualified medical expenses.
HSA's are tax-advantaged savings accounts that accompany high deductible health plans (HDHPs). HDHPs offer lower monthly premiums in exchange for a higher deductible (the amount you pay before insurance kicks in).
What are the benefits of an HSA?
- It saves you money. HDHPs have lower monthly premiums, meaning less money is being taken out of your paycheck.
- It is portable. The money in your HSA is carried over from year to year and is yours to keep, even if you leave the company.
- It is a tax-saver. HSA contributions are made with pre-tax dollars. Since your taxable income is decreased by your contributions, you'll pay less in taxes.
The maximum amount you can contribute to an HSA in 2026 is $4,400 for individual coverage and $8,750 for family coverage.
Important HSA Rules
To be eligible for our HSA, you must be enrolled in a High Deductible Health Plan through Arthur Chapman. In addition, in order to contribute, you must NOT:
- Be claimed as a dependent on someone else's tax return.
- Have other health plan coverage that provides benefits covered under the HDHP Plan (for example, a Health Care Flexible Spending Account, a separate prescription drug plan or a spouse's non-HDHP plan).
- Have a spouse with a Health Care Flexible Spending Account that could reimburse your medical expenses.
- Be enrolled in a government health plan, such as Medicare or Medicaid.
- You must re-enroll in your HSA each year during open enrollment to participate in the plan.
If you are age 55 or older you may make an additional "catch-up" contribution of $1,000.
Please note: Dependent health care costs are qualified expenses under your HSA plan, as long as they are claimed as your tax dependent. After age 19, when they are no longer considered your tax dependent, their medical costs will no longer be considered a qualified expense.

Voluntary Options
Arthur Chapman provides eligible employees with the opportunity to participate in a variety of voluntary benefits.
Accident Insurance
Accident insurance can pay you money based on the injury and the treatment you receive, whether it’s a simple sprain or something more serious, like an injury from a car accident. The Accident Insurance Plan can pay you a benefit for an emergency room treatment, stitches, crutches, injury-related surgery and a list of other accident related expenses. You can also purchase coverage for your spouse and dependent children. This plan includes a $50 wellness benefit per insured, per year.
Critical Illness
Critical Illness, including cancer, impacts so many lives. Critical Illness pays a lump sum cash benefit that can help with expenses if you or a covered family member is diagnosed with a covered condition which includes:
- Heart Attack
- Major organ failure
- End-stage kidney failure
- Benign brain tumor
- Coronary artery bypass surgery (pays 25% of lump sum)
- Invasive cancer
- Stroke
- Paralysis
This plan also includes a $50 wellness benefit per insurance, per year. The plan has a 12/12 pre-existing limitation. Lookback period is 12 months and must be treatment free for 12 consecutive months.
If you elect the Accident and/or Critical Illness insurance, you can earn an incentive for taking care of your health. Each of your covered family members are also eligible for this benefit. Under the Wellness Benefit you can receive a $50 check for having important tests and screenings each year. Many of these tests are routine, so it is easy to take advantage of this benefit. - Go to unum.com/access.
- If filling for the first time, click “Create an account.” We recommend using a personal email address that is easily accessible when away from work. If you already have an account, enter your email.
- Once you’re logged in, begin with “Start a Claim or Leave” to provide initial details of what happened.
- Add claimant information as requested, including:
- Your employment information
- Medical care resulting
- Medical providers visited: physicians, hospitals, other medical professionals

Hospital Insurance
Hospital Insurance helps covered employees and their families cope with the financial impacts of a hospitalization. You can receive benefits you’re admitted the hospital for a covered accident, illness or childbirth. Coverage includes:
- $1,500 for each covered hospital admission - once per year
- $100 for each day of your covered hospital stay, up to 60 days
You can also purchase coverage for your spouse and dependent children.
Pet Insurance

The IDShield membership includes:
