Eligibility
Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Arthur Chapman. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.

We are pleased to offer our employees an easy to use, online enrollment tool called Ease. Please use the link below to access your new hire enrollment.
The benefit choices you make during your initial enrollment remain in effect for the entire year.
You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

Arthur Chapman offers two medical plans - the $1,500 Deductible plan with HRA and the $3,000 Deductible plan with HSA - both plans are available through Blue Cross Blue Shield of Minnesota. The medical plans offered to Arthur Chapman employees utilizes the Aware Network which is an open access provider network through Blue Cross Blue Shield of Minnesota (BCBS of MN).
Both plans include prescription drug coverage. Be sure to check the provider websites to find doctors, use the tools and get the latest information.

Health Reimbursement Account (HRA) Contributions
The HRA is employer-funded and available to you only while employed at the Firm and enrolled in the Firm’s BCBS HRA medical plan. Your HRA funds offset deductible expenses only. The funds are automatically applied to your deductible expenses by Further and paid directly to your provider. You do not need to file a claim to utilize HRA funds. Unused HRA funds will carry-over to the following plan year, but your funds will not exceed the plan year deductible maximum of $1,500 single and $3,000 family.
Doctor On Demand
With Doctor On Demand, Board-certified doctors are available 24/7, 365 days a year to treat many common medical conditions. You can also schedule next-day appointments to see licensed psychologists and psychiatrists between the hours of 7 a.m. to 10 p.m., local time.

Arthur Chapman's dental plan provides comprehensive coverage to help you and your family maintain good dental health. Providers are part of the Delta Dental network, or you may see an out-of-network dentist of your choice. Be aware if you go out-of-network, your costs will be higher.

Arthur Chapman provides you with Basic & Life/AD&D coverage through Unum at no cost to you.
Employees working 30 or more hours per week are eligible for 1.5x your annual salary to a maximum of $300,000 in group life and accidental death and dismemberment (AD&D) insurance. Arthur Chapman pays for the full cost of this benefit - meaning you are not responsible for paying any monthly premiums.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as divorce or birth of a child.
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.

Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. You are eligible for short term disability following 1 year of employment.

If you have enrolled in the Aware $3,000 High Deductible Health plan with HSA, you may be eligible to open a Health Savings Account (HSA) through Superior USA. Health Savings Accounts are a great way to save money and pay for qualified medical expenses.
HSA's are tax-advantaged savings accounts that accompany high deductible health plans (HDHPs). HDHPs offer lower monthly premiums in exchange for a higher deductible (the amount you pay before insurance kicks in).

Flexible Spending Accounts (FSAs) are voluntary accounts that allow you to set aside pre-tax money for certain health and dependent care expenses. These are separate accounts with separate purposes, and therefore the money in one account cannot be used to satisfy expenses in the other. As an Arthur Chapman employee, you can choose to participate in one or all of these accounts depending on your needs. The Flexible Spending plan is administered by Superior USA Benefits.
When you establish an FSA, you will be able to choose the annual amount you want to contribute to the account, up to certain plan limits. This amount is deducted from your paycheck in equal installments before Federal Security taxes are withheld. FSAs are a great way to save money, but keep in mind, they are "use it or lose it" provisions for each account, which means you will forfeit any balance that is left in your FSA at the end of the year.
You do not have to be enrolled in the Firm's medical, dental or vision plans to be eligible to participate in the FSA, however, if you are enrolled in the HRA medical plan through the Firm be sure to factor the employer-fund HRA dollars when estimating how much pre-tax dollars should be set aside.
Health Care Flexible Spending Account
Employees who are enrolled in the $1,500 deductible plan and not enrolled in a Health Savings Account (HSA) or High Deductible Health Plan (HDHP) may participate in the Health Care Flexible Spending Account.
You can elect up to $2,850 in the Health Care Flexible Spending Account for 2022. You can use this money to pay for eligible medical, dental, and vision expenses you or your dependents incur during the plan year.
Limited Health Care Flexible Spending Account
The Limited Health Care Flexible Spending Account is for all employees who contribute, or have a spouse who is contributing to a Health Savings Account or High Deductible Health Plan (includes expenses for dental, orthodontic and vision care only).
You can elect up to $2,850 in the Limited Health Care Flexible Spending Account for 2022.
Dependent Care Flexible Spending Account
This annual maximum applies to all contributions made by you and your spouse to a dependent care account. You can be reimbursed for any daycare expenses you have in a plan year if the expenses are necessary to allow you and your spouse to work.

Accident Insurance
Accident insurance can pay you money based on the injury and the treatment you receive, whether it’s a simple sprain or something more serious, like an injury from a car accident. The Accident Insurance Plan can pay you a benefit for an emergency room treatment, stitches, crutches, injury-related surgery and a list of other accident-related expenses. You can also purchase coverage for your spouse and dependent children. This plan includes a $50 wellness benefit per insured, per year.
Critical Illness
Critical Illness, including cancer, impacts so many lives. Critical Illness pays a lump-sum cash benefit that can help with expenses if you or a covered family member is diagnosed with a covered condition which includes:
This plan also includes a $50 wellness benefit per insurance, per year. The plan has a 12/12 pre-existing limitation. Lookback period is 12 months and must be treatment free for 12 consecutive months.
