This portal is designed to provide you with an introduction to the company and help make your transition a smooth one.

First Day Paperwork

This section contains links to your First Day Paperwork. Please review this information and bring complete documents with you when you report to work.


You will also need to provide two forms of identification for I9 purposes; the I9 form contains a list of acceptable items. 


Forms should be scanned and emailed back to Human Resources. We are required to keep originals on file, these should be mailed to:

Benefits

As a new employee, you have the opportunity to enroll in Alvogen’s benefit programs. Any coverage you elect will take effect on the first day of the month following your date of hire. You may also enroll your eligible dependents which includes your legal spouse or domestic partner, your dependent children who are under age 26, and unmarried children of any age if they are mentally or physically disabled.

 

It is important that you carefully consider your options and elect the plans that will meet the needs of you and your family. The elections you make now, as a new employee, will be in place for the remainder of 2023. You cannot change these elections during the year unless you experience a qualifying life event.
