Alvogen offers three medical plans options through Aetna. All three plans utilize a network of physicians, hospitals and other health care providers. Referrals are not required. All three plans include a prescription drug plan. These plans have different coverage levels and contributions so you can select the plan that best meets your needs.

If you are enrolled in the Aetna Core medical plan, you may be eligible to contribute money to a Health Savings Account (HSA). An HSA allows you to save money pre-tax through payroll deductions and use those funds to pay for you and your family's qualified healthcare expenses. To determine if you are eligible to contribute to an HSA, please review the eligibility requirements in the Alvogen Benefits Guidebook.
Alvogen partners with Fidelity to offer this HSA.
Alvogen will also make an annual contribution to your HSA based on your medical enrollment tier.
The IRS sets annual limits on the amount of money which may be contributed to a Health Savings Account. For 2023, the maximum amount for single coverage is $3,850 and $7,750 for all other tiers. If you are age 55 or older, you may contribute an additional $1,000 annually in catch-up contributions.
These maximum contributions include both the employee and employer contributions.

Alvogen is pleased to partner with Cigna Healthcare to offer dental coverage. The Cigna Passive PPO Dental Plan provides preventive, restorative and orthodontic coverage. While you may see any provider, you will receive greater benefits (less out-of-pocket costs) when a participating dentist performs covered dental services.

Alvogen provides employees with an opportunity to enroll in VSP Vision Care, one of the nation’s largest providers of eye care coverage.
VSP provides you with access to affordable, quality vision care coverage and allows you to receive a complete eye examination and materials every 12 months.
As a VSP member, you also have access to savings on additional services such as prescription sunglasses or Lasik vision correction. Check out the VSP benefit summary below for additional details.
Flexible Spending Accounts (FSAs) allow you to set aside money on a pre-tax basis to pay for certain out-of-pocket health care and dependent care expenses. Alvogen offers three types of FSAs:
General Purpose Health Care FSA - This FSA is an option for employees who are enrolled in either the Aetna Traditional PPO or Premium PPO plans (or to employees who have waived medical with Alvogen).
Limited Purpose Health Care FSA - This FSA is only offered to employees who are enrolled in the Aetna Core medical plan. This type of FSA can be paired with a Health Savings Account.
Dependent Care FSA - This FSA is available to all benefit eligible employees regardless of what (if any) medical coverage you have with Alvogen.

Commuter Benefits are separate FSA accounts which allow you to set aside a portion of your income on a pre-tax basis to pay for public transportation and/or parking that you incur on your commute to and from work.
Please Note - This benefit is available to Alvogen employees working in the state of New Jersey.
Alvogen’s Commuter Benefits program is administered by EdenRed. You can sign up for Commuter Benefits at any time during the year and enrollment is easy! Log on to Eden Red's Commuter Check website via the link below. Click on ‘New User Sign Up’ and be sure to use Alvogen’s company code (CCO15465) to complete your enrollment.
As a benefit eligible employee, you are provided with Basic Life Insurance at no cost to you. You are also provided with an equal amount of Basic AD&D coverage based on the same Class.
The amount of your basic life and AD&D insurance benefit is dependent on your job type: Salaried, Hourly or Executive.
If you need life or AD&D insurance in addition to the coverage that Alvogen provides to you at no cost, you can elect Supplemental Life and AD&D Insurance. The cost of this coverage is paid for by you via paycheck deductions. Supplemental insurance may be purchased in amounts as follows:
You must elect Supplemental Life/AD&D coverage for yourself if you want to purchase this coverage for your spouse or child(ren).
The disability benefits provided by Alvogen work together to help you pay your household expenses if you become disabled and cannot work. You are provided with both Short-Term and Long-Term Disability at no cost.
Short-Term disability benefits protect your income in the event you are unable to work due to an accident or injury for a short duration of time.
Long-Term Disability benefits are provided to employees who remain disabled for 180 calendar days or more. Long-Term Disability coverage provides a continued source of income equal to 60% of your monthly pre-disability earnings to a maximum monthly benefit as described below:
Effective with your first paycheck, Alvogen will add the value of the Long-Term Disability premium to your paycheck (subject to taxes), then deduct the cost of coverage, thus allowing you to pay the tax on the value of the premium and collect a tax-free LTD benefit.
Alvogen has established a 401(k) Savings Plan through Fidelity to assist you in meeting your long-term financial goals. This plan allows you to contribute pre-tax or post-tax (Roth) dollars from each paycheck.
The 401(k) plan has an automatic enrollment feature. Unless you elect otherwise, approximately 30 days after hire, you will be automatically enrolled at 6% of eligible pay. You may change the contribution amount or opt out of the plan if you so choose.
Alvogen matches your contribution at 100% up to 6% of eligible pay. To be eligible for matching contributions, you are required to make employee pre-tax contributions or Roth 401(k) contributions. The Plan will match on the combined total of these contributions up to the annual IRS limit.
Voluntary benefits are coverages that are designed to complement the Alvogen benefits offering. The costs for these plans are 100% employee paid. You have the opportunity to elect the plans below:

Employee Assistance Program (EAP)
The NexGen EAP offers specialized life enhancement services, referrals, and individualized guidance at no cost to you. You and your eligible family members have access to 8 counseling sessions per year at no cost.
Education Assistance Plan
You may be reimbursed for expenses incurred for continuing education for up to two courses per semester. The plan would reimburse 100% of tuition expenses for a passing grade of “C” or above (up to a maximum of $5,250 per year) as well as 100% of costs for books and registration fees. You must have at least six months of continuous full-time employment with the Company to be eligible, and plan participation must be approved in advance. Other restrictions may apply.
AIG Benefits Travel Assist Program
This company paid benefit provides out-of-country accident and sickness medical coverage with 24/7 assistance services.
Health Advocate
Health Advocate provides a wide variety of services that can help employees and their families with health care and insurance matters. Services are available to all Alvogen employees and to their spouses, dependent children, parents and parents-in-law. Health Advocate services are free and completely confidential.
Medicare Assistance
When you have questions about Medicare eligibility and Medicare Supplement Insurance, HTA’s Medicare Services are there to help. They are a telephone support team for employees who are approaching retirement and/or age 65 and may have questions about how Medicare works with their group coverage, whether to enroll in COBRA vs Medicare, Medicare Supplement Insurance, etc. Their service is free.
