Henry Doneger Annual Enrollment Information 2021-2022

Annual enrollment is your opportunity to learn about the 2021-2022 Benefits Program, view or update your current coverage options or elect new coverage options for you and your family. You can access and review our 2021-2022 Benefits Guide in "Additional Resources" below.

Benefits Overview

Summary of Medical Benefit Changes Effective November 1, 2021:

Buy Up PPO Plan

-      Increasing the Deductible from $1,000/$2,000 to $1,500/$3,000 for in-network services

-      Changing the Virtual Visit copay from $10 to No Charge

-      Increasing the Tier 2 & Tier 3 Prescription Drug Deductible from $100 to $150

-      Increasing the Tier 2 Prescription Drug copy from $35 to $40

-      Increasing the Tier 3 Prescription Drug copay from $75 to $80

 

Core EPO Plan

-      Increasing the Deductible from $2,500/$5,000 to $3,000/$6,000 for in-network services

-      Increasing the Out of Pocket Maximum from $8,150/$16,300 to $8,550/$17,100 for in-network services

-      Changing the Virtual Visit copay from $20 to No Charge

-      Decreasing the Tier 1 Prescription Drug Retail copay from $15 to $10

-      Increasing the Tier 2 Prescription Drug Retail copay from $45 to $50

-      Increasing the Tier 3 Prescription Drug Retail copay from $75 to $90

 

Buy-Down HSA EPO Plan

-      Increasing the Deductible from $4,000/$8,000 to $5,750/$11,500 for in-network services

-      Increasing the Out of Pocket Maximum from $6,750/$13,500 to $7,000/$14,000 for in-network services

-      Changing Virtual Visit coverage from a $10 copay after the deductible is met to No Charge after the deductible is met

-      Changing the Emergency Room benefit from 30% coinsurance after the deductible is met to 50% coinsurance after the deductible is met


Dependent Eligibility: Children will be eligible for coverage until the end of the month in which they turn 26 years of age.

How To Enroll

How to Enroll for the 2021-2022 Plan Year
  • Access the Henry Doneger Benefit Guide to review your current coverage and enroll and modify your elections for 2021-2022.
  • If you'd like to change your existing coverage or make new elections, you must do so by October 22nd.
  • You can download your election form below. Once you have completed your election form, review it for accuracy, and return it to Lauren Trotta by October 22nd.
  • HSA Plan Members, decide if you want to make a contribution to your Health Savings Account.
