Please review the Benefits Guide that includes benefit offerings and monthly rates. If you do not make any changes all your current elections will rollover to the next plan year. All benefit changes and updates need to be submitted by November 19th, for an effective date of December 1, 2021. You can find the enrollment forms in the “Enrollment Forms” folder in the “Employee Benefits” Channel in Employee Central.
