Open Enrollment Window: November 12th - 19th
We will have no benefit changes for the 2023-2024 plan year. Your current benefit elections will rollover to next plan year if you do not take any action. If you would like to make a benefit change, please submit a paper enrollment/change form before
November 24, 2023.
Benefits Overview
Medical Plans:
- G735PFR Medical Plan Blue Advantage Network, $2,000 Deductible
- P8E1ADT Medical Plan Blue Advantage Network, $750 Deductible
Dental Plan:
- Delta Dental of Oklahoma PPO Plus Premier Network
Vision Plan:
- VSP Vision Care
Basic Life and AD&D:
- Dearborn/BCBS - $50,000

How To Enroll
Download and complete a change/enrollment form and submit to NFP before the open enrollment deadline of November 24, 2023.
If you experience a qualifying life event that requires a benefit change mid-plan year, please notify NFP within 30-days of the qualifying event. If you miss this 30-day window you will have to wait until next open enrollment to make your benefit changes and updates.
