Eligibility

All full-time employees working at least 30 hours per week are eligible to participate in the benefit plans offered by AirBoss. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.


Children are eligible for benefits as follows:


  • Medical, Dental and Vision: Until the last day of the month in which the child turns 26
  • Child Life Insurance: Until the last day of the year in which the child turns 21, or 25 if the child is a full-time student
  • Child AD&D: Until the end of the year in which the child turns 26


If you are enrolling a spouse or a child, please note that AirBoss will be conducting a dependent audit in 2023. You will be required to provide documentation, at a later time, to cover any spouse or child under the plan. Acceptable forms of documentation include, but are not limited to: marriage license, birth certificate, adoption papers, judgments of divorce, etc.

ENROLLMENT INSTRUCTIONS:

  1. Go to www.flexible-productsconnection.com
  2. Enter your User ID: First Initial + Last Name
  3. Enter your Password: Last 4 digits of your SSN
  4. Follow instructions and enroll in your benefits
  5. Make sure to save your elections and print your confirmation statement


The elections you make can only be changed under certain circumstances, called "Qualifying Life Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes online within 30 days of the event.

Employee Contributions

Pre-Tax Contributions


You share in the cost of your Medical and Vision Benefits. Your employee contributions for these benefits are taken out of your pay on a pre-tax basis as set forth in each section below.

Medical

Both medical options are PPOs. A PPO option offers the freedom to see any provider when you need care. When you use providers from within the PPO network, you receive benefits at the discounted network cost. Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance. In order to comply with IRS regulations for high deductible health plans, the deductible on the PPO HSA will increase from $1,400/$2,800 to $1,500/$3,000.


Please note: if you do not make changes to your current medical election, you will still be able to use your current ID card in 2023. If you make changes, your new ID card may not arrive before April 1, 2023, but, rest assured, you will have benefits coverage.

Costs of Virtual Visits

Medical Visits

  • Non-HSA Plan: Equal to your Office Visit Copay
  • HSA Plan: 20% coinsurance applies after satisfying deductible


Mental Health Visits

  • Non-HSA Plan: Equal to your Office Visit Copay
  • HSA Plan:

-Therapy Visits: $85 (Masters Level) or $99 (Doctorates Level) – Once your deductible is satisfied, coinsurance will apply if applicable

-Psychiatry Visits: $199 for initial visit and $95 for follow up visits – Once your deductible is satisfied, coinsurance will apply if applicable Licensed


For all types of visits and both plan options, prescription copays apply if medication is prescribed.

Life and AD&D

Basic Life/AD&D

You are automatically enrolled in Basic Life and AD&D insurance benefits. Your Basic Life Insurance benefit is $50,000 (reduced if you are age 70 or older), and the amount of your AD&D benefit is equal to your Life insurance. You do not need to enroll in these benefits as they are provided at no cost to you.

Disability

Short Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. The amount of your Short Term Disability benefit depends on whether you are classified as Union (under 1 year) or Union (over 1 year). Please see below for information on your benefit.
