How to Enroll
Helping you and your family reach and maintain good health is very important to Adventure Subaru. Adventure Subaru is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Action Required:
All elections must be submitted by 5:00pm on Monday, 05/12/2025.
This is an ACTIVE enrollment.
All employees will use a benefit election form to confirm or change benefit elections. You must take action no later than 5pm Monday, May 12, 2025. After this deadline you must wait until the next open enrollment period or experience a qualifying event in order to:
· Waive any benefits
· Change or drop the coverage of your current plan
· Participate, if you did not enroll during open enrollment or within the first 30 days of becoming eligible
Eligibility & Qualifying Events
Eligibility
Employee Eligibility: Employees are eligible for benefits the 1s day of the month following 30 days of employment.
If you do not enroll during the Open Enrollment period, you must wait until the next Open Enrollment period unless you experience a Qualifying Life Event. If you experience a Qualifying Life Event (i.e., birth or adoption of a child, marriage, divorce, loss of other coverage, etc.) you must enroll within 30 days of the Qualifying Event.
Dependent Eligibility: Eligible employees may enroll their dependents for coverage. Eligible dependents include:
- Your legal spouse:
- You MUST submit a copy of your Marriage License. If you have already submitted this documentation previously, you do not need to resubmit.
- Dependent children: up to the end of the month of 26th birthday regardless of student or marital status. The dependent child must meet the carrier’s legal definitions (i.e., child by birth, legal adoption or stepchildren that are required to be covered by court order).
- You MUST submit a copy of each child’s Birth Certificate. If you have already submitted this documentation previously, you do not need to resubmit.
Qualifying Life Events
If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.
Qualifying events include:
- Marriage
- Divorce
- Birth/Adoption of a child
- Death of a spouse or other enrolled dependent
- Change in spouse’s benefits or employment status
- A dependent becomes eligible for Medicare or Medicaid
Medical/Rx
Anthem will continue to be the medical and prescription carrier this year.

Dental Benefits through MetLife Dental provide comprehensive coverage to help you and your family maintain good dental health. Your coverage will be greater if you go to an in-network provider.

- Babies should receive their first professional eye exam at 6 months

Adventure Subaru offers vision coverage through Guardian which utilizes the VSP network to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in the VSP Network.

Basic Life & AD&D
Adventure Subaru provides all full-time employees with Basic Life/AD&D coverage.
Who's Your Beneficiary? Everyone needs to make sure your beneficiary is up to date when you enroll through.
Don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Life Policy Conversion is available if you want to keep this coverage after you leave employment with Adventure Subaru.

Short Term Disability
Short Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. This benefit is 100% voluntary. Benefits begin the 1st day for an Accident and 8th day for an Illness and is effective for up to 13 weeks.
