WELCOME
Helping you and your family reach and maintain good health is very important to Active Dynamics. Active Dynamics is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Active Dynamics remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Please review your 2023 Enrollment materials for more information regarding your plans offerings.

ENROLLMENT
All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Active Dynamics. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
- Medical, Dental & Vision: End of the calendar year in which the child turns 26
- Voluntary Life: Age 21, or 25 if full time student
Action Required: All elections must be made within 30 days of eligibility date.
ENROLLMENT INSTRUCTIONS:
- Go to www.vconverterconnection.com
- Enter your User ID: First Initial + Last Name e.g. John Smith = JSmith
- Enter your Password: Last 4 of your SSN
- Follow instructions and enroll in your benefits
- Make sure to save your elections and print your confirmation statement.
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child. You must provide proof of Life Event to Lucy Keith.

HAP Medical/Rx
HAP will continue to be the medical and prescription carrier this year. You may view participating providers at the link below or consult with your existing providers.
HSA
Health Equity
The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- Covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You have not received VA benefits in the past 90 days
2023 HSA IRS Maximums are as follows:
$3,850 for a single contract
$7,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the FSA's you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
You may contribute as follows:
Health Care and Limited Purpose FSA
- Up to $3,050 annually, $610 rollover
- All benefit eligible can participate
- If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
- Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
Dependent Care FSA
- Up to $5,000 annually
- Limited to $2,500 if you are married and file separate tax returns
VIRTUAL VISITS
HAP
Active Dynamics provides those employees who are covered by one of the HAP medical plans access to virtual visits with AmWell. AmWell provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

DENTAL

Active Dynamics offers eligible employees a dental plan detailed below. For more information see attached Dental Summary. To find out if your provider is in-network, please visit the website below.
VISION
NVA
Active Dynamics offers vision coverage through NVA to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. SAFETY GLASSES
NVA
Eligible members are entitled to receive one (1) pair of lenses and a frame once every plan year. To verify your benefit eligibility prior to calling or visiting your eye care provider, please contact NVA, see details below.
Safety eyeglasses are an important component of employee protection in the workplace. They are a requirement of OSHA for all employees who work in a hazardous environment and the responsibility of the employer to ensure compliance. In order to assist clients with providing the appropriate prescription safety eyewear, NVA has created the following safety eyeglass
program:
- Frames: All frames will meet the ANSI Z87.5 standard for manufacturing and carry the Z87 logo on the front and temples. Manufacturers, including On-Guard, Titmus (or similar), may be utilized, and providers must agree to make available an adequate selection of zyl (plastic) and metal frames in appropriate sizes and colors with both skull and cable temples available. Safety shields (fixed or removable may be allowed, depending on occupational hazard), when required, will also be available.
Frames must be new, and only complete frames (2 temples and 1 front) may be provided. Members may choose to supply their own safety frame. However, it must be a new frame and carry the required ANSI Z87 markings.
Repairs to existing frames may be made as long as the frame is less than 2 years old.
- Lenses: All lenses supplied must be fabricated from Polycarbonate material (unless the required lens type is not available in Polycarbonate, i.e., Double D Segment Lenses) and meet the Z87.1 standard for manufacturing of ophthalmic lenses. They must carry the logo (stamped in one of the corners of each lens) of the fabrication laboratory. Lenses must be available in standard single vision, bifocal or trifocal designs including flat tops (28 and 35 mm and executive). Progressive Addition Lenses may be supplied at member’s expense, if safety compliant according to the fixed fee schedule below.
When lenses are not available in Polycarbonate, other materials may be utilized if they meet the ANSI Standards for Industrial Lenses including thickness.
Lens replacements may be made for change of prescription or breakage, subject to restriction on frame age.
LIFE & DISABILITY

Active Dynamics provides you with financial protection options for life, accidental death & dismemberment, short-term disability and long-term disability with Lincoln Financial Group.
Employer Paid Life/AD&D
Employees working 30 or more hours per week are eligible for employer paid Basic Life/AD&D in the amount of $25,000, payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.
Voluntary Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Voluntary Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in this employee paid coverage, this will be in addition to your employer provided Life/AD&D coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Guaranteed Issue (GI) and Evidence of Insurability (EOI)
When you are first eligible (at hire) for Optional Life and AD&D coverage, you may purchase up to the Guaranteed Issue (GI), $100,000 or 5x BAE for yourself and up to $25,000 for your spouse, without providing EOI.
During Open Enrollment:
Current participants: Employee can increase coverage by one increment ($25,000) not exceeding the GI without proof of good health. If GI ($100,000) is exceeded EOI will be required. Any increase in spousal coverage would require EOI.
Non-participants will have to provide EOI for any dollar amount.
Employer Paid Short-Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Benefits begin on the 1st day of an accident and 8th day of an illness, for 60% of your weekly income up to $500 per week, benefits will pay for up to 13 weeks.
Employer Paid Long-Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long-Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits being after a 90-day elimination period and cover 60% of your monthly income up to a maximum of $5,000 per month for as long as you are disabled or up to Social Security Normal Retirement Age (SSNRA). Pre-existing limitations apply.
