All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Active Dynamics. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -

ENROLLMENT INSTRUCTIONS:
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child. You must provide proof of Life Event to Lucy Keith.
HAP Medical/Rx
HAP will continue to be the medical and prescription carrier this year. You may view participating providers at the link below or consult with your existing providers.
Health Equity
The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
2023 HSA IRS Maximums are as follows:
$3,850 for a single contract
$7,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+

AmWell provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

NVA
Active Dynamics offers vision coverage through NVA to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Eligible members are entitled to receive one (1) pair of lenses and a frame once every plan year. To verify your benefit eligibility prior to calling or visiting your eye care provider, please contact NVA, see details below.
Safety eyeglasses are an important component of employee protection in the workplace. They are a requirement of OSHA for all employees who work in a hazardous environment and the responsibility of the employer to ensure compliance. In order to assist clients with providing the appropriate prescription safety eyewear, NVA has created the following safety eyeglass
program:
Frames must be new, and only complete frames (2 temples and 1 front) may be provided. Members may choose to supply their own safety frame. However, it must be a new frame and carry the required ANSI Z87 markings.
Repairs to existing frames may be made as long as the frame is less than 2 years old.
When lenses are not available in Polycarbonate, other materials may be utilized if they meet the ANSI Standards for Industrial Lenses including thickness.
Lens replacements may be made for change of prescription or breakage, subject to restriction on frame age.

Active Dynamics provides you with financial protection options for life, accidental death & dismemberment, short-term disability and long-term disability with Lincoln Financial Group.
Employer Paid Life/AD&D
Employees working 30 or more hours per week are eligible for employer paid Basic Life/AD&D in the amount of $25,000, payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.
Voluntary Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Voluntary Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in this employee paid coverage, this will be in addition to your employer provided Life/AD&D coverage.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Guaranteed Issue (GI) and Evidence of Insurability (EOI)
When you are first eligible (at hire) for Optional Life and AD&D coverage, you may purchase up to the Guaranteed Issue (GI), $100,000 or 5x BAE for yourself and up to $25,000 for your spouse, without providing EOI.
During Open Enrollment:
Current participants: Employee can increase coverage by one increment ($25,000) not exceeding the GI without proof of good health. If GI ($100,000) is exceeded EOI will be required. Any increase in spousal coverage would require EOI.
Non-participants will have to provide EOI for any dollar amount.
Employer Paid Short-Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Benefits begin on the 1st day of an accident and 8th day of an illness, for 60% of your weekly income up to $500 per week, benefits will pay for up to 13 weeks.
Employer Paid Long-Term Disability
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long-Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits being after a 90-day elimination period and cover 60% of your monthly income up to a maximum of $5,000 per month for as long as you are disabled or up to Social Security Normal Retirement Age (SSNRA). Pre-existing limitations apply.
