WELCOME

Helping you and your family reach and maintain good health is very important to AVI. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

ENROLLMENT

Employee Eligibility: You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective on the 1st of the month following your date of hire or newly eligible date. You may also enroll eligible dependents for benefits coverage. A ‘dependent’ is defined as the legal spouse and/or ‘dependent child(ren)’ of the plan participant or the spouse.


Action Required: All elections must be submitted within 15 days of eligibility


Remember: The benefit choices you make during your initial enrollment or annual open enrollment remain in effect for the entire year.    

You can, however, modify your elections under certain circumstances, called “Qualifying Life Event”. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child. Refer to your Employee Benefits Guide or contact Human Resources if you have questions about qualifying events.

TELEMEDICINE

Ross provides those employees who are covered by the BCBS medical plan access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.


How Much Does It Cost?

Standard Plan (Non HDHP/HSA): Equal to your office visit copay.

Prescriptions: If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

FLEXIBLE SPENDING ACCOUNT

The Health Care, Limited Purpose and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to an FSA you reduce your taxable income, so you pay less in taxes — which saves you money.


HEALTH CARE FSA

  • You may contribute up to $3,050 annually, you can rollover only $610, the rest you will lose if you don't use it.
  • All benefit eligible can participate, unless you or your spouse are contributing to an HSA
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


LIMITED PURPOSE FSA

  • You may contribute up to $3,050 annually, you can rollover only $610, the rest you will lose if you don't use it.
  • If you are enrolled in an HDHP HSA – you are only able to participate in a Limited Purpose FSA
  • Limited Purpose FSA can over cover the costs for qualified dental and vision expenses for yourself and dependents – not able to cover any qualified medical expenses


DEPENDENT CARE FSA

  • You may contribute up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns
  • Eligible expenses include care for your dependent child, spouse, parent or grandparent who are incapable of caring for themselves

LIFE & DISABILITY

Mutual of Omaha

We provide all team members with Basic & Voluntary Life/AD&D coverage as well as Short- and Long-Term Disability through Mutual of Omaha (MOO).


Basic Life/AD&D

Employees working 30 or more hours per week are eligible for 1.5 your salary, to a maximum of $375,000, in coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident.


Voluntary Life/AD&D

A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


  • At Open Enrollment you can opt to increase up to one increment of $10,000 without EOI, if the amount you request exceed GI, you will be required to fill out an EOI. 
  • When you are first eligible (at hire) for Voluntary Life and AD&D, you may purchase up to $100,000 GI for yourself and up to $20,000 for your spouse without providing EOI.
  • If you elect optional life coverage, and are required to complete an EOI form, it is your responsibility to complete the form and to submit it to the carrier within 30 days of your election.


Short-Term Disability

Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your benefits begin on the 15th day of accident or illness and will pay 60% of your income up to a maximum of $2,000 per week, for 11 weeks.


Long-Term Disability

Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long-Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. Benefits begin after a 90-day elimination period and are equal to 60% of covered monthly earnings up to a maximum of $7,000 per month.
