How To Enroll
Open Enrollment is happening today through June 2nd, 2021. Any changes you make will be effective July 1st, 2021.
Please note, all employees MUST log into your Ease online portal to complete your enrollment.

We have partnered with NFP to provide a live webinar to review our benefit plans and offerings. NFP will also assist in answering questions in real time.

2021-2022 Benefits
- MEDICAL: We will be staying with Humana. In an effort to keep your premiums as low as possible, we are introducing 2 additional options! Staff can continue to enjoy our enhanced copay options, or they can enroll in our new High Deductible Plans! Please review all four plan options to decide which option is best for you and your family.
- HEALTH SAVINGS ACCOUNT (HSA): We are excited to announce that, for those enrolled in one of the new High Deductible Plans, you will now have access to contribute to an HSA! Individuals can contribute up to $3,600 annually and employees covering dependents can contribute up to $7,200 annually. For any employees over the age of 55, you can also contribute an additional $1,000 as a catch up contribution.
- DENTAL & VISION: This year we will be keeping our Dental & Vision coverages with Humana! There will be NO CHANGES to the plan costs or benefits.
- LIFE/AD&D AND DISABILITY: We will be keeping our Group Life/AD&D, Voluntary Life/AD&D, Short Term Disability and Long Term Disability benefits with AXA/Equitable this year. Audubon Schools provides Group Life/AD&D, Short Term Disability and Long Term Disability coverages to all full time employees. Enrollment in these plans are automatic. If you currently have Voluntary Life/AD&D coverage, you will keep your benefits. As a new hire, you can enroll in the Voluntary Life/AD&D coverages up to $100,000 with no medical questions asked. However, if you would like to increase your life coverage or have previously waived coverage and would like to newly enroll, you will need to complete medical questions (Evidence of Insurability EOI) to request the coverage(s).
- VOLUNTARY BENEFITS: We will continue to offer Accident, Cancer, Critical Illness and Hospital Indemnity coverages through Guardian! These benefits pay cash benefits directly to YOU.
Benefit Notices
SBC: I hereby acknowledge receipt of the Summary of Benefits and Coverage (SBC) found in Ease. I have read the SBC and am familiar with its contents. If I have any questions concerning the information, I will contact the carrier or my Human Resource Department to have my questions answered. I understand this SBC is not a contract, and the material represents guidelines subject to change.
Offer of Coverage: I certify that I have received enrollment and plan information for health care coverage that Audubon Schools offers, representing that it meets ACA requirements for affordable, minimum essential coverage, offering minimum value to me. I have been given the opportunity to enroll in or decline that coverage.
Pre-Tax Payroll Deductions: I authorize any payroll deductions required for the elections made in this online enrollment portal. I understand that these deductions will be made on a pre-tax basis unless I elect otherwise on this form. I understand that I cannot change my elections until the next open enrollment period, but I may change coverage for the dependents I am insuring or add new dependents if there is a "qualified" change in status. I have read the plan description and understand the post-tax deduction option.
