Upon your termination of employment or employee status change, your Basic Life, Accidental Death & Dismemberment (AD&D), as well as any Supplemental Life/AD&D that you may have purchased, will be ending. You have the right to continue these policies either through Portability or Conversion. Please see the grid below which will provide links to applications and various resources. Please be sure to reach out to NY Life right away if you wish to continue these policies post termination. You only have 30 days from your date of termination to elect the policy provision that suits you and make the applicable payment.
