2025-2026 Benefits
Medical: We will continue to partner with Louisiana Blue (previously known as Blue Cross Blue Shield of Louisiana) for the 2025-2026 plan year. You can choose between a copay plan that gives you predictable costs for doctor visits and prescriptions with a lower deductible, or a high‑deductible health plan with lower premiums that can be partnered with a Health Savings Account (HSA). HSAs allow you save tax‑free dollars help cover your medical expenses.
We have partnered with Sun Life to offer a fully comprehensive benefits package for all other coverages.
- Dental: This dental plan offers an extensive dental network and fully covers cleanings and checkups and helps pay for fillings, major dental work, and child orthodontia.
- Vision: This vision plan offers low‑cost eye exams and lenses, plus an allowance to help you pay for frames or contacts.
- Basic Life & AD&D: This employer‑paid benefit provides financial protection for your loved ones with life and accidental death coverage equal to twice your annual salary.
- Voluntary Life & AD&D: You can choose to purchase additional life insurance for yourself, your spouse, and your children for extra peace of mind.
- Long‑Term Disability: If you’re ever unable to work due to a long‑term illness or injury, this employer-paid plan replaces 60% of your income after 90 days to help support you and your family.
- Accident Insurance: This plan pays you cash benefits if you’re injured in a covered accident to help with expenses like deductibles, copays, or everyday bills.
- Critical Illness Insurance: This plan provides a lump‑sum cash payment if you’re diagnosed with a covered serious illness, including cancer, giving you financial support when you need it most.
- Hospital Indemnity Insurance: This plan pays you a set cash benefit if you’re admitted to the hospital, helping offset the extra costs that come with a hospital stay.
Benefit Notices
SBC: I hereby acknowledge receipt of the Summary of Benefits and Coverage (SBC) found in Ease. I have read the SBC and am familiar with its contents. If I have any questions concerning the information, I will contact the carrier or my Human Resource Department to have my questions answered. I understand this SBC is not a contract, and the material represents guidelines subject to change.
Offer of Coverage: I certify that I have received enrollment and plan information for health care coverage that Advantage Capital offers, representing that it meets ACA requirements for affordable, minimum essential coverage, offering minimum value to me. I have been given the opportunity to enroll in or decline that coverage.
Pre-Tax Payroll Deductions: I authorize any payroll deductions required for the elections made in this online enrollment portal. I understand that these deductions will be made on a pre-tax basis unless I elect otherwise on this form. I understand that I cannot change my elections until the next open enrollment period, but I may change coverage for the dependents I am insuring or add new dependents if there is a "qualified" change in status. I have read the plan description and understand the post-tax deduction option.
