ASi MEC Plus


This is the part of your combo that covers your preventative care at 100%.  When you enroll in Sedera you are automatically enrolled in this plan.


You will receive an ID card for this plan.

 

PHCS network search instructions:


Go to www.multiplan.com

Click on Find a Provider in the upper right corner

Click on the green Select Network button & choose PHCS

Click the green Select Network box

Choose PHCS in the pop-up box

Select Preventive Services Only

There are two steps to enrolling:


All employees should complete their election form by DATE.

Insert instructions to enrollment portal if applicable.


Anyone enrolling with Sedera must complete their online enrollment with Sedera through the new portal by Thursday, August 20th, to ensure they will have a September 1st effective date.


  • Current members will follow their unique renewal link. This would have been sent to your email address on file. You can also receive this link from HR.


  • NEW members should follow INSERT LINK. You will also need to enter the Employer Code: 0000
