Convive Brands is proud to offer a comprehensive program of benefits designed to serve the diverse needs of our unique workforce, and we are committed to continually enhancing and expanding our offerings.
Your compensation at Convive Brands includes more than just your paycheck.
Eligibility & Enrollment
Convive Brands offers a comprehensive benefit program to help protect your health and income, and provide resources and discounts to help you get healthier and save money. During Open Enrollment, you have the opportunity to review and select the benefits that will best meet your personal situation.
All full-time employees working an average of at least 30 hours per week are eligible to enroll themselves and their eligible dependents on the 1st of the month following 30 days of employment. After making your coverage selection, you may not change your plan options until the next open enrollment period unless you experience a change in family status.

In order to continue eligibility throughout the year, you must maintain at least 30 eligible hours per week. Your eligibility for benefits will be reviewed on a quarterly basis.
Eligible Dependents May Include:
- Your legal spouse
- Your child who is less than age 26
- Court ordered eligible dependents with legal guardianship
Changing Benefit Elections:
You have a variety of benefits that are offered to you. Be sure to consider your choices before you make your benefit decisions.
The reason is that once you make your elections, most will remain in effect until the next open enrollment period, unless you have incurred a life event.
Convive Brands annual enrollment period generally occurs around December 1st of each year. Changes are effective on January 1st.
Before Enrollment:
Before enrollment begins, take the time to educate yourself on all of the benefit options that are available to you. Convive Brands provides a variety of tools and resources to help you make your benefits decisions. These tools can be accessed online on the Workday Portal.
•Review this Benefits Guide carefully as you consider your plan choices.
•Attend Annual Benefits Enrollment Meeting to hear benefits consultant explain the details of each plan and give you an overview of all benefit programs.
During Enrollment:
•If you wish to continue with your current elections for 2026, no further action is required. Your elections will automatically rollover into the new plan year.
•If you do not wish to enroll, be sure to waive.
•Once you have completed your enrollment online, save or print a copy of your confirmation, review it for accuracy and retain it for your record.
For Existing Employees (During the annual enrollment period)
•If you are currently enrolled and would like to make a change to your benefit elections, you will need to make your new elections online.
•If you are an existing employee and wish to enroll for the first time, you will need to complete your elections online.
After Enrollment:
•Verify your benefits elections by visiting the carrier’s member services portal. If you notice any errors, notify the Convive Brands Benefits Team immediately at benefits@convivebrands.com. Most elections cannot be changed except within 30 days of a qualifying event.
- Download Anthem’s mobile app named Sydney. Once downloaded onto your phone, you will have access to your digital ID card, plan details, claims and so much more. This robust tool will also assist with finding care, wellness goals, view costs, online appointment scheduling and one-click access to Live Health Online and Anthem.com.
Medical

Convive Brands offers 3 medical plans, insured by Anthem BC. All of the medical plans include prescription drug coverage, preventive care and wellness resources. All of the plans offer 100% coverage for in-network preventive care, resources to help you get and stay healthy, and financial protection from catastrophic expenses.
•EPO Low Plan – Access to Anthem Blue Access -There are no out of network benefits.
•EPO High Plan –Access to Anthem Blue Access - There are no out of network benefits.
•PPO Plan– Access to Anthem’s PPO network - You can visit any provider; however, your costs are lower when you visit a network provider. When you use out-of-network providers, your deductible and coinsurance are higher and you are responsible for any amounts that exceed the UCR, in addition to the deductible and coinsurance.

Rx Cost Saving Tools & Tips!
MAIL ORDER -
Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you.
GOOD Rx -
There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).

Out-of-network services are not covered under the EPO plan. To receive plan benefits and discounted rates, you must use participating Anthem / Kaiser providers.

Convive Brands' dental plans encourage early detection of dental problems by paying the most toward diagnostic and preventive services, such as routine check-ups and cleanings.

Anthem offers a large national dental network. You will not be balance billed if you see a PPO dentist. If you choose to visit a non-participating dentist, you may be balance billed and could incur higher out-of-pocket costs.

Vision

Convive Brands offers vision coverage through Anthem to help pay for eye exams, prescription glasses and contact lenses.

You may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services out-of-network, you may be required to pay that provider at time of service and will need to submit a claim to Anthem for the applicable reimbursement based on the out-of-network reimbursement schedule.
Voluntary Life and AD&D
You have the freedom to select adequate levels of life insurance coverage to protect the well-being of your family. Convive Brands offers Voluntary Life and Voluntary Accidental Death & Dismemberment through Unum. These premiums are paid for by the employee.
You must enroll in Unum Life Insurance when you’re first eligible or during open enrollment. Once enrolled, you cannot make changes outside of open enrollment unless you experience a qualified life event.
Voluntary Disability
As a Convive Brands employee, you have access to comprehensive disability insurance coverage. Your ability to work and earn an income is your most valuable asset. Disability insurance provides a portion of your income if you can't work because of an illness or non-job-related injury.
You have the option to purchase long-term disability coverage 100% employee paid.

Additional Voluntary Benefits
Convive Brands realizes that your needs go beyond just the standard insurance. This is why we also provide all employees and their families access to personal service and assistance resource. Here you will find information on our MetLife Accident, Critical Illness, and Hospital Indemnity. Additionally, we are happy to offer ARAG Legal, Pets Best Insurance, and Life Lock to our employees. All to give you peace of mind and support when you need it the most!
Accident
Accident
Accident insurance provides cash benefits to employees who have been injured in a covered accident. Benefits do not coordinate with existing medical insurance so they can be used toward anything that the insured sees fit. A sample of treatments and diagnosis included in the accident plan are shown. Employees are required to have major medical in place in order to enroll in the accident plan.
Critical Illness
Critical Illness
Critical Illness insurance provides a lump sum benefit paid upon the first diagnosis of a covered condition. Benefits do not coordinate with existing medical insurance so they can be used toward anything that the insured sees fit.
