Benefits Effective January 1, 2025 - December 31, 2025
Helping you and your family reach and maintain good health is very important to United Community Housing Coalition. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. United Community Housing Coalition remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Highlights of the 2025 benefit plans include:

All FULL-TIME employees working at least 30 hours per week are eligible to participate in the benefit plans offered by United Community Housing Coalition. Newly hired employees will be eligible the 1st of the month following 30 days of full-time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children are eligible for benefits as follows:
• Medical, Dental and Vision: Until the last day of the month in which the child turns 26
• Dependent Child Life Insurance & AD&D: Until child turns 26
Should your employment with United Community Housing Coalition end, your coverage will end on the last day of the month in which your employment ends, except STD and LTD, they terminate day of.
ENROLLMENT INSTRUCTIONS:
The Benefit choices you make during your initial enrollment remain in effect through December 31, 2025. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

United Healthcare (UHC) will be your medical and prescription carrier.
If you enrolled in one of the United Healthcare medical plans please be sure to register through the UHC member portal where you can access important information pertaining to your medical plans with UHC including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the UHC portal as of your effective date of coverage.

Helpful Rx Cost Saving Tools & Tips!
Viral Medication Program - The new UnitedHealthcare Vital Medication Program offers certain drugs at no additional cost.* This means there may be no out-of-pocket costs for preferred insulins and certain other medications, including: Insulin, Epinephrine, Glucagon, Naloxone, Albuterol. To see if you’re eligible for no out-of-pocket costs on sign in to myuhc.com/rx
MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. You can get more information on how to start receiving mail order drugs by your member portal under Rx > Refills & Home Delivery.
GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
UHC offers virtual visits so you can get the care you need when and where you need it the most!
24/7 Virtual Care
24/7 Virtual Visits are a way to schedule same-day, urgent care visits, so you can talk with a provider 24/7 for common urgent care needs or when your primary care provider (PCP) is not available.
Virtual Primary Care
Get regular health visits and checkups with a primary care provider (PCP), without the office visit. You can choose to see a network PCP regularly, so the same provider will take care of you over time.
Virtual Therapy
Get confidential behavioral health care by talking to a psychiatrist or therapist during a virtual home visit from the safety and comfort of your home. Check your benefits to learn more about the cost for Virtual therapy.
Virtual Specialty Care
Virtual specialty care allows you to more quickly access specialty care that's personalized for your specific care needs, such as migraine or back, joint and muscle care. Check your benefits to see if Virtual specialty care is available to you.
United Community Housing Coalition now offers a Health Savings Account (HSA) through Optum Bank that allows you to set aside pre-tax dollars to pay for eligible expenses.
HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and the employer funding ($1,000/$2,000). If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.
$3,300 for a single contract | $6,550 for a two person or family
$1,000 "Catch Up" contribution for those age 55+
United Community Housing Coalition's dental plans provide comprehensive coverage to help you and your family maintain good dental health. The dental plan include both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in the United Community Housing Coalition Ameritas Dental plan today and have not already done so, please be sure to register through the Ameritas Dental member portal where you can access important information pertaining to your dental plan with Ameritas Dental including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Ameritas Dental portal as of your effective date of coverage.

United Community Housing Coalition offers vision coverage through NVA to help pay for eye exams, prescription glasses and contact lenses. NVA also provides discounts for Hearing Care and Lasik.
If you are already enrolled in one of the United Community Housing Coalition's NVA vision plan today and have not already done so, please be sure to register through the NVA member portal where you can access important information pertaining to your vision plan with NVA including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the NVA portal as of your effective date of coverage.

United Community Housing Coalition understands the importance to you and your family to have strong financial protection. We provide all employees Basic Life/AD&D and Disability coverage at no cost to you. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.
United Community Housing Coalition provides both Short & Long Term Disability benefits at no cost to you!
Everyday illnesses or injuries can interfere with your ability to work, more serious issues can impact your ability to work indefinitely. Even a few weeks away from work can make it difficult to manage household costs. Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
Short-Term Disability
Benefits begin on the 1st day of a qualifying accident and the 7th day for a qualifying illness.
Benefits may last up to 11 weeks (90 days)
Benefit Amount 60% of your weekly earnings up to $650 per week
Long-Term Disability
Benefits begin on the 91st day of an accident or illness.
Benefits may last up to your Social Security Normal Retirement Age.
Benefit Amount is 60% of your monthly income to a maximum of $3,000
A sudden accident or death can leave you or your loved ones in a vulnerable position. United Community Housing Coalition provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage in the amount of $25,000 at no cost to you. Employees have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
United Community Housing Coalition realizes that your needs go beyond just the standard insurance. This is why when electing the Life and Disability carrier, they partnered with a carrier that offers additional value-added benefits at no additional cost to you and your families.
