We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Northgate. For new employees, benefits are effective on the 1st of the month following 60 days from your date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
Should your employment with Northgate end, your coverage will end as of the date of termination.
ENROLLMENT INSTRUCTIONS:
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Blue Cross Blue Shield of MI (BCBSM) will continue to be your medical and prescription carrier. You have the choice between three plans for the 2025 plan year. The three plan options include a PPO plan, a POS plan, and an HMO plan. View the Providers & Networks for more information on how these plans networks differ from each other as well as buttons to locate participating providers.
If you are already enrolled in one of the Northgate BCBSM medical plans today and have not already done so, please be sure to register through the BCBSM member portal where you can access important information pertaining to your medical plans with BCBSM including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the BCBSM portal as of your effective date of coverage. Information on how to register can be found here.

Look up your prescriptions by viewing the BCBSM Drug List. Use the Control F feature to type in the name of your prescriptions and determine which level the drug is and whether a Prior Authorization (PA) or Quantity Limits (QL) apply. You can also determine whether there may be lower cost alternatives for your prescriptions.
Helpful Rx Cost Saving Tools & Tips!
MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
The three medical plans function differently when it comes to networks and access. The PPO and POS plans provide out-of-network coverage for a variety of services, whereas the HMO only provides out-of-network coverage in the event of a true emergency. If you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing. Virtual Urgent Care
When you have an urgent care need, Teladoc has you covered 24/7 from wherever you are, whether it be at home or while traveling! Common conditions where Virtual Care can be used for urgent care needs include colds, sore throats, urinary tract infections, pink eye, sinus infections, rashes, and more! Visits are available for adults and children.
Mental Health Services
Through the Mental Health option, you can connect with a licensed therapist or U.S. board-certified psychiatrist when you’re dealing with stressful situations or issues such as grief, anxiety and depression. Mental health visits require an appointment, but many therapists and psychiatrists have evening and weekend availability.

You have the opportunity to contribute to a variety of savings and/or spending accounts on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by Northgate.
A Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for Qualified Medical Expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
2024 HSA IRS Maximums are as follows:
$4,150 for a single contract
$8,300 for a two person or family
2024 Employer Contribution:
Single: $250
Two Person/Family: $500
$1,000 "Catch Up" contribution for those age 55+
If you have an HSA with another bank or custodian, you can transfer your funds to the Northgate HSA's with Health Equity by completing this form.

Northgat's dental plans provide comprehensive coverage to help you and your family maintain good dental health. The dental plans include both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in one of the Northgate Ameritas Dental plans today and have not already done so, please be sure to register through the Ameritas Dental member portal where you can access important information pertaining to your dental plan with Ameritas Dental including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Ameritas Dental portal as of your effective date of coverage.
You have the choice of two dental plans - The Core Plan includes a deductible (for basic and major services only), no ortho coverage, higher cost sharing for your basic and major services, and a lower annual maximum - OR - The Buy-Up Plan. The Buy-Up plan has no deductible, richer benefits for basic and major services, orthodontia coverage is included for dependents (through age 19), and a higher annual maximum

Northgate offers vision coverage through BCBSM partnered with VSP to help pay for eye exams, prescription glasses and contact lenses. VSP also provides discounts for Hearing Care and Lasik.
If you are already enrolled in one of the Northgate's VSP vision plan today and have not already done so, please be sure to register through the VSP member portal where you can access important information pertaining to your vision plan with VSP including participating providers, coverage information, and explanation of benefits/claims. Northgate understands the importance to you and your family to have strong financial protection. We provide all employees Basic Life/AD&D and Disability coverage at no cost to you. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.
Northgate provides you with financial protection options for life, accidental death & dismemberment, short-term disability and long-term disability with Prudential.
Company Paid Life/AD&D
Northgate provides all eligible employees with Life Insurance to provide your loved ones some financial relief. Accidental Death and Dismemberment pays in addition to Life if your death is deemed an accident and dismemberment pays a partial amount to you. Coverage amount is 1x annual salary up to $50,000, see Benefit Summary for more details.
Voluntary Life/AD&D
Employees also have the opportunity to enroll in Voluntary Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in this employee paid coverage, this will be in addition to your employer provided Basic Life/AD&D described above. See Benefit Summary for details and ADP for rates.
Northgate provides both Short & Long Term Disability benefits at no cost to you!
Short-Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short-Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Benefits begin on the 8th day of an accident or illness, will be paid for up to 12 weeks and can cover 60% of your weekly income up to the max amount. Long-Term Disability
Illnesses or accidents can disrupt your life, and your ability to work for months – even years. Long-Term Disability provides financial protection for you by paying a portion of your lost income, so you have financial support to manage your disability and your household. Benefits begin after 90 days and can cover 60% of your monthly earnings up to the max amount, for as long as you are disabled or until you reach your Social Security Retirement Age. ACCIDENT INSURANCE
Our coverage pays you cash benefits that correspond with hospital and intensive care confinement. Your plan may also include coverage for a variety of occurrences, such as: dismemberment; dislocation or fracture; ambulance services; physical therapy and more. The cash benefits can be used to help pay for deductibles, treatment, rent and more.
CRITICAL ILLNESS INSURANCE
You choose benefits to protect yourself and any family members if diagnosed with a critical illness. Then, if diagnosed with a covered critical illness, you will receive a cash benefit based on the percentage payable for the condition.
HOSPITAL INDEMNITY INSURANCE
Our Hospital Indemnity insurance pays a cash benefit for hospital confinements. This benefit is payable directly to you and can keep you from withdrawing money from your personal bank account or your Health Savings Account (HSA) for hospital-related expenses. It is increasingly important to not only protect your finances if faced with an unexpected illness, but also to empower yourself to seek the necessary treatment.
