All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Northgate. For new employees, benefits are effective on the 1st of the month following 60 days from your date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
- Medical, Dental & Vision: End of the calendar year in which the child turns 26
- Voluntary Life: Age 19, or 25 if full time student
Should your employment with Northgate end, your coverage will end as of the date of termination.

The three medical plans function differently when it comes to networks and access. The PPO and POS plans provide out-of-network coverage for a variety of services, whereas the HMO only provides out-of-network coverage in the event of a true emergency. If you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing. View the comparison below for additional details on the differences between the PPO, POS, and HMO plans when it comes to networks and access.

- PPO PROVIDERS Enter your zip code and select "Yes this is Correct" - then in the upper right corner select your plan "PPO Plans"
- POS MI PROVIDERS Use this button if you are a MI Resident electing a POS Plan or a non-MI resident traveling to MI. Enter your zip code and select "Yes this is Correct" - then in the upper right corner select your plan "Blue Care Network HMO & POS Plans"
- POS NON-MI PROVIDERS Use this button if you live outside of MI or are a MI resident traveling outside of MI. Enter your zip code and select "Yes this is Correct" - then rather than entering your card information, select "Browse a list of plans" and select "BlueCard Traditional"
- HMO PROVIDERS Available to MI residents only. Enter your zip code and select "Yes this is Correct" - then in the upper right corner select your plan "Blue Care Network HMO & POS Plans"

Virtual Urgent Care
When you have an urgent care need, Teladoc has you covered 24/7 from wherever you are, whether it be at home or while traveling! Common conditions where Virtual Care can be used for urgent care needs include colds, sore throats, urinary tract infections, pink eye, sinus infections, rashes, and more! Visits are available for adults and children.
Mental Health Services
Through the Mental Health option, you can connect with a licensed therapist or U.S. board-certified psychiatrist when you’re dealing with stressful situations or issues such as grief, anxiety and depression. Mental health visits require an appointment, but many therapists and psychiatrists have evening and weekend availability.
Family members ages 18 and older will need to create their own Virtual Care accounts. When updating or creating an account, choose your plan name and enter your member ID so your coverage is applied correctly. Call 1-800-835-2362 with any questions about your account or to arrange a telephone visit
You have the choice of two dental plans - The Core Plan includes a deductible (for basic and major services only), no ortho coverage, higher cost sharing for your basic and major services, and a lower annual maximum - OR - The Buy-Up Plan. The Buy-Up plan has no deductible, richer benefits for basic and major services, orthodontia coverage is included for dependents (through age 19), and a higher annual maximum

What are Basic Services? Periodontics, endodontics (root canals), oral surgery, fillings, prosthetic maintenance

Vision
Northgate offers vision coverage through BCBSM partnered with VSP to help pay for eye exams, prescription glasses and contact lenses. VSP also provides discounts for Hearing Care and Lasik.
If you are already enrolled in one of the Northgate's VSP vision plan today and have not already done so, please be sure to register through the VSP member portal where you can access important information pertaining to your vision plan with VSP including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the VSP portal as of your effective date of coverage.
