Helping you and your family reach and maintain good health is very important to Job Site Services. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. Job Site Services remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees.
Open Enrollment is from April 11, 2025 through April 21, 2025. This is the time of year where you can elect new plans with Job Site Services. You may switch between plans and make changes without being required to have a qualifying life event. Please also keep in mind the following when making your elections for 2025:
All full-time employees working at least 20 hours per week are eligible to participate in the benefit plans offered by Job Site Services. Newly hired employees will be eligible on the first day of active full-time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.
Children are eligible for benefits as follows:
• Medical, Dental and Vision: Until the last day of the year in which the child turns 26
• Dependent Child Life Insurance & AD&D: Until the last day of the year in which the child turns 26
Should your employment with Job Site Services end, your coverage will end as of the date of termination.
ENROLLMENT INSTRUCTIONS:
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event.

Blue Cross Blue Shield of MI (BCBSM) will continue to be your medical and prescription carrier. You have the choice between three plans for the 2025 plan year. View the Providers & Networks for more information on how these plans networks differ from each other as well as buttons to locate participating providers.
If you are already enrolled in one of the Job Site Services BCBSM medical plans today and have not already done so, please be sure to register through the BCBSM member portal where you can access important information pertaining to your medical plans with BCBSM including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. Helpful Rx Cost Saving Tools & Tips!
GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
The three medical plans function differently when it comes to networks and access. The two PPO and POS plans provide out-of-network coverage for a variety of services. If you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing.

Job Site Services provides those employees who are covered by one of the BCBSM medical plans access to virtual care with Teladoc Health. Teladoc Health provides members the opportunity to receive medical and mental health care from the comfort of your own home.
All eligible members will need to download the Teladoc Health app and create a new account with Teladoc Health.
Virtual Care:
Have a virtual visit with a U.S. board-certified doctor for minor illnesses such as colds, sore throats, urinary tract infections and pink eye. Visits are available for adults and children 24/7. Average wait time is 10 minutes. Prescriptions can be called in to your preferred pharmacy if needed.
Virtual Primary Care: (Available to PPO members only)
See your PCP virtually, from the convenience and comfort of your home. You will receive, at no cost to you, a digital blood pressure monitor before your first visit. Appointments are available from 7 a.m. to 6 p.m. Monday through Friday, and 8 a.m. to 12 p.m. on Saturday.

Virtual Mental Health Services:
Through the Mental Health option, you can connect with a licensed therapist or U.S. board-certified psychiatrist when you’re dealing with stressful situations or issues such as grief, anxiety and depression. Appointments are available from 7 a.m. to 9 p.m., seven days a week. Therapy is available for members ages 13 and up. Psychiatry is available for members ages 18 and up.
Livongo:
A resource for those with hypertension, prediabetes, or diabetes. Some may qualify for a glucose monitor or scale, at no cost. See flyers for details.
myStrength:
A resiliency and behavioral health resources available to you for health needs including; depression, anxiety, substance abuse, chronic pain, and much more.

Iris Cancer is difficult to navigate, emotionally and physically. That’s why we’re working with OncoHealth® to introduce Cancer Support, a navigational program through OncoHealth’s virtual platform, Iris. Cancer Support is part of your health plan at no cost for you and your employees.

Sempre Rx This discount program focuses on certain drugs treating common chronic conditions where following doctors’ orders can improve your health and prevent serious complications.
You may be contacted by BCBSM via phone or mail to sign up for PillarRx, it's important you don't ignore these calls or letters. The team will only contact you when a drug you're taking is now under a 'co-pay assistance program' called PillarRx. If you don't register for PillarRx, which is free to do, you will be asked to pay a much higher copay than you are used to.
Tobacco Coaching Quitting tobacco is difficult. BCBSM has a program to help you through this journey.
AbleTo Virtual appointments with a licensed therapist who will guide you through a personalized 8-week program over phone or video to teach you skills and tools you can use to enjoy more of life.
Job Site Services dental plan provide comprehensive coverage to help you and your family maintain good dental health. The dental plans include both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in the Job Site Services Unum Dental plan today and have not already done so, please be sure to register through the Unum Dental member portal where you can access important information pertaining to your dental plan with Unum Dental including participating providers, coverage information, and explanation of benefits/claims. Job Site Services offers vision coverage through BCBSM partnered with VSP to help pay for eye exams, prescription glasses and contact lenses. BCBSM also provides discounts for Hearing Care and Lasik.

Job Site Services understands the importance to you and your family to have strong financial protection. We provide employees Basic Life/AD&D and Short-Term Disability coverage at no cost to you! In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.
A sudden accident or death can leave you or your loved ones in a vulnerable position. Job Site Services provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage in the amount of 1 time's your annual salary up to $200,000 at no cost to you. Employees have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Job Site Services provides Short-Term Disability at no cost to you!
Everyday illnesses or injuries can interfere with your ability to work, more serious issues can impact your ability to work indefinitely. Even a few weeks away from work can make it difficult to manage household costs. Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
Long-term disability is available on a voluntary basis, with this coverage you would be eligible for 60% of your monthly income up to $5,000 per month after a 90-elimination period. Benefits may last for as long as you are disabled or reach SSNRA, rates are based on age, please see Employee Navigator for the cost.
Job Site Services provides a competitive benefits package for all employees as part of our total compensation package. We realize you make benefit choices based on what’s important to you and the needs of your changing lifestyle. These benefits are paid for through payroll deduction on a post-tax basis so that the benefits are tax free.
Accident & Hospital coverage is provided through Unum Insurance. Hopefully you are never in the position to use it, but if you were to experience an accident or hospitalization, it could end up costing you a lot of money. With these plans, you will be paid money for qualifying situations/services to cover out of pocket expenses or you may use the funds for whatever you like.
Documents & Forms
Retirement Plan Summary- Please review the rules and features of the plan. The company does offer a matching contribution of 100% of the first 4% you defer.
Beneficiary Designation form- Please complete and sign to name a beneficiary for your account. The Spousal Consent section on the back of the form is only needed if you are married and naming someone other than your spouse as the primary beneficiary. Once completed, please return this form to your HR Contact.
Investment Details
Please review the investment options available to you. You can either choose one actively managed risk-based model portfolio, or any of the individual mutual funds listed. If you are uncertain which model is best for you, please complete the Risk Assessment Questionnaire. You can also contact Avantax for personal investment guidance. If you do not enroll and choose an investment option, your balance will be invested in the Moderate Portfolio by default.
Online Enrollment Instructions
Enrolling will involve choosing how much you would like to contribute from your paycheck and how your balance is invested. To enroll in the plan, you can do this online by going to www.avantax.com/retire and clicking on New User. Instructions and screen shots for enrolling online are attached.
