How To Enroll

ENROLLMENT INSTRUCTIONS:

  1. Review your Benefit Information
  2. Print or e-fill the attached Election Form (Election Form must be complete even if you are waiving coverage)
      1. Complete the Election Form
      2. Save the Form
      3. Provide to Sue Gerkey

The Benefit choices you make during your initial enrollment remain in effect for the entire plan year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Blue Cross Blue Shield of MI (BCBSM) & Blue Care Network (BCN) will continue to be your medical and prescription carrier. You have the choice between three plans for the 2025 plan year. The three plan options include a PPO plan, and two HMO plans. View the Providers & Networks for more information on how these plans networks differ from each other as well as buttons to locate participating providers.

If you are already enrolled in one of the DePorre Veterinary Hospital BCBSM medical plans today and have not already done so, please be sure to register through the BCBSM member portal where you can access important information pertaining to your medical plans with BCBSM including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the BCBSM portal as of your effective date of coverage. Information on how to register can be found here.

Helpful Rx Cost Saving Tools & Tips!

MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. You can get more information on how to start receiving mail order drugs by the member portal under medications.

GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).

ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.

Providers & Networks

The three medical plans function differently when it comes to networks and access. The PPO plan provide out-of-network coverage for a variety of services, whereas the HMO only provides out-of-network coverage in the event of a true emergency. If you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing. Virtual Care

Deporre Veterinary Hospital provides those employees who are covered by one of the BCBSM medical plans access to virtual care with Teladoc Health (formerly known as BCBSM Online Visits). Teladoc Health provides members the opportunity to receive medical and mental health care from the comfort of your own home.

All eligible members will need to download the Teladoc Health app and create a new account with Teladoc Health.

Virtual Care:

Have a virtual visit with a U.S. board-certified doctor for minor illnesses such as colds, sore throats, urinary tract infections and pink eye. Visits are available for adults and children 24/7. Average wait time is 10 minutes. Prescriptions can be called in to your preferred pharmacy if needed.

Virtual Primary Care (Only available to PPO Members): 

See your PCP virtually, from the convenience and comfort of your home. You will receive, at no cost to you, a digital blood pressure monitor before your first visit. Appointments are available from 7 a.m. to 6 p.m. Monday through Friday, and 8 a.m. to 12 p.m. on Saturday.

Virtual Mental Health Services:

Through the Mental Health option, you can connect with a licensed therapist or U.S. board-certified psychiatrist when you’re dealing with stressful situations or issues such as grief, anxiety and depression. Appointments are available from 7 a.m. to 9 p.m., seven days a week. Therapy is available for members ages 13 and up. Psychiatry is available for members ages 18 and up.

Livongo:

A resource for those with hypertension, prediabetes, or diabetes. Some may qualify for a glucose monitor or scale, at no cost. See flyers for details.

myStrength:

A resiliency and behavioral health resources available to you for health needs including; depression, anxiety, substance abuse, chronic pain, and much more.

Iris Cancer is difficult to navigate, emotionally and physically. That’s why we’re working with OncoHealth® to introduce Cancer Support, a navigational program through OncoHealth’s virtual platform, Iris. Cancer Support is part of your health plan at no cost for you and your employees.

Sempre Rx This discount program focuses on certain drugs treating common chronic conditions where following doctors’ orders can improve your health and prevent serious complications.


You may be contacted by BCBSM via phone or mail to sign up for PillarRx, it's important you don't ignore these calls or letters. The team will only contact you when a drug you're taking is now under a 'co-pay assistance program' called PillarRx. If you don't register for PillarRx, which is free to do, you will be asked to pay a much higher copay than you are used to.


Tobacco Coaching Quitting tobacco is difficult. BCBSM has a program to help you through this journey.


AbleTo Virtual appointments with a licensed therapist who will guide you through a personalized 8-week program over phone or video to teach you skills and tools you can use to enjoy more of life.

You have the opportunity to contribute to a Health Savings Account (HSA) on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by DePorre Veterinary Hospital.

Health Savings Account (HSA)

The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. The BCN HMO 6350 is qualified for an HSA. By contributing to an HSA you reduce your taxable income, so you pay less in taxes — which saves you money.

HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and any employer provided funds to the HSA. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.

$4,300 for a single contract | $8,550 for a two person or family

$1,000 "Catch Up" contribution for those age 55+

If you have an HSA with another bank or custodian, you can transfer your funds to the DePorre HSA's with Health Equity by completing this form.

DePorre Veterinary Hospital's dental plans provide comprehensive coverage to help you and your family maintain good dental health. The dental plans include both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.

Discussing what might happen to your family if you were not around to provide for them isn't always the easiest conversation, but it is necessary.

To help with the task of planning for your family’s security, DePorre Veterinary Hospital provides all eligible employees with Basic Group Life Insurance and Accidental Death & Dismemberment (AD&D) coverage through Guardian.

BASIC LIFE INSURANCE

Life insurance is an important part of your financial security. Life insurance helps protect your family from financial risk and sudden loss of income in the event of your death. If you have life insurance, chances are you can take comfort in knowing that those who depend on you will be provided for.

BENEFICIARY(IES)

  • It’s very important to designate beneficiaries. Taking a few minutes to designate your beneficiaries now will help ensure that your assets will be distributed according to your direction. 
  • Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
  • It is also important that you name a Primary and Contingent Beneficiary. A contingent beneficiary will receive the benefits of your life insurance if the primary beneficiary cannot. You can change beneficiaries at any time. 
  • You should review your beneficiary elections on a regular basis to ensure they are updated as life changes. Even if you are single, your beneficiary can use your Life Insurance to pay off your debts, such as: credit cards, mortgages, and other expenses.
