Introduction

Welcome! In this page, you will find all of the information regarding the benefits offered by Suburban Sports.


With that, you will find yourself equipped with resources to find care and plan information.


Summary of Changes Effective January 1st, 2024:

  • Medical is still with BCBSM, no benefit changes, 3% increase in contributions
  • Dental is still with Delta Dental, no changes in benefits, 5-6% increase in contributions
  • Vision is still with VBA, no changes in benefits or contributions
  • Life/AD&D, Vol Life/AD&D, STD, & LTD are still with MetLife, no changes in benefits
  • Voluntary option of ID & Legal Sheild, no changes

If you have gone through open enrollment before, you will see that there were not any changes to your plan offerings for the new year! Once in the enrollment system, you will be able to make changes as it is open enrollment or keep everything the same as previously elected.


Directions to enroll:

  1. Go to suburbansportsconnection.com
  2. Enter your user ID: First initial + last name + SSS
  3. Enter your password: Last 4 of your SSN
  4. Follow the prompts and enroll in benefits!
  5. Review all elections and changes and be sure to save!



Eligibility and Qualifying Life Events

Medical benefits are offered to full-time employees who work a minimum of 30 hours a week, and all other benefits are offered to employees working 40 hours a week. New employees are eligible on the 1st of the month following 60 days of active employment. Dependents, as defined as a legal spouse or child may be enrolled with employees as well. Dependent eligibility varies by line of coverage. If you have a child on medical, dental, vision or legal plan, they may remain until the end of the calendar year they turn 26. If you cover your child however on child life insurance, they are eligible until the date they turn 20 or 26 if they are a full-time student.


Qualifying life events allow you to change your coverage when something changes in your life. Some examples include, marriage and divorce, birth or adoption, and loss or gain of coverage related to those circumstances previously listed or a change in job if on a spousal plan. After these changes occur, you have 30 days from event date to change necessary elections by contacting human resources.


Important note: If your spouse is employed and has access to medical coverage, you are not allowed to enroll them under the Suburban Sports medical and dental benefits.


If you separate from employment, COBRA continuation of coverage may be available as applicable by law.

Teladoc Virtual Visit

NEW! Blue Cross Online Visits is transitioning to Virtual Care through Teladoc Health


Suburban Sports provides those employees who are covered by one of the BCBSM medical plans access to virtual care with Teladoc Health (formerly known as BCBSM Online Visits). Teladoc Health provides the opportunity to receive medical and mental health care from the comfort of your own home.


All eligible members will need to download the Teladoc Health app and create a new account with Teladoc Health. The Blue Cross Online Visits app will no longer be available as of 1/1/24.


Virtual Urgent Care

Have a virtual visit with a U.S. board-certified doctor for minor illnesses such as colds, sore throats, urinary tract infections and pink eye. Visits are available for adults and children 24/7.


Primary Virtual Care

See your PCP from the comfort of your home. Once registered for Virtual PCP you will be mailed an arm band and blood pressure reader.


Mental Health Services

Through the Mental Health option, you can connect with a licensed therapist or U.S. board-certified psychiatrist when you’re dealing with stressful situations or issues such as grief, anxiety and depression. Mental health visits require an appointment, but many therapists and psychiatrists have evening and weekend availability.


Prescriptions

If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.


With Teladoc you will also have access to Livongo - a hypertension, pre-diabetes, and diabetes resource. As well as myStrenght a mental health resource to those enrolled in Livongo.

Iris Cancer is difficult to navigate, emotionally and physically. That’s why BCBSM is working with OncoHealth® to introduce Cancer Support, a navigational program through OncoHealth’s virtual platform, Iris. Sempre Rx This discount program focuses on certain drugs treating common chronic conditions where following doctors’ orders can improve your health and prevent serious complications. Click the "Sempre Rx" button below to learn more.


Pillar Rx You may be contacted by BCBSM via phone or mail to sign up for PillarRx, it's important you don't ignore these calls or letters. The team will only contact you when a drug you're taking is now under a 'co-pay assistance program' called PillarRx. Flexible Spending Account

Flexible Spending Accounts (FSA) allow you to reduce your taxable income by setting aside pre-tax dollars from each paycheck to pay for eligible out-of-pocket health care and dependent care expenses* for yourself, your spouse and your dependent children.


In order to participate in the FSA, you must enroll each year. Your annual contribution stays in effect during the entire year January 1st through December 31st. The only time you can change your election is during the enrollment period or if you experience a change-in-status event. Also, you must elect this benefit when first benefit eligible.


MEDICAL & LIMITED PURPOSE FSA

MAXIMUM ANNUAL CONTRIBUTION | $3,200

All eligible health care expenses – such as deductibles, medical and prescription copays, dental expenses, and vision expenses – can be reimbursed from your medical FSA account. With the Medical FSA or Limited Purpose FSA, you can spend up to the full amount of your annual election as soon as your account has been set up.

FSA’s have a $640 rollover feature, which allows any amount of $640 or less remaining in your account at the end of the plan year to roll over into the new plan year, anything over that amount you forfeit.


DEPENDENT CARE FSA

The Dependent Care FSA allows you to pay for eligible dependent care expenses with tax-free dollars so that you and your spouse can work or attend school FT. Unlike the Medical FSA, funds in a Dependent Care FSA are only available once they have been deposited into your account and you cannot use the funds ahead of time.

– You may set aside up to $5,000 annually in pre-tax dollars, or $2,500 if you are married and file taxes separately from your spouse.

– If you participate in a Dependent Care FSA, you cannot apply the same expenses for a dependent care tax credit when you file your income taxes.


ELIGIBLE DEPENDENT CARE EXPENSES* INCLUDE:

1.‘Care’ for your dependent child who is under the age of 13 that you can claim as a dependent on your federal tax return;

2.‘Care’ for your dependent child who resides with you and who is physically or mentally incapable of caring for themselves; or

3.‘Care’ for your spouse, parent or grandparent who is physically or mentally incapable of caring for themselves and spends at least eight hours a day in your home.


Care is defined as:

  • In-home babysitting services (not by an individual you claim as a dependent)
  • Care of a preschool child by a licensed nursey or daycare provider
  • Before and after school care
  • Summer day camp
  • In-home dependent daycare

Life and Disability Insurance

Suburban Sports provides you with financial protection options for life, accidental death & dismemberment, short-term disability and long-term disability with MetLife.


Core Life/AD&D is paid for by Suburban Sports, all benefit eligible employees have a $15,000 benefit. An age reduction is applied at ages 65 and 70, see Benefit Summary below for more information.



Voluntary Life/AD&D is available in addition to the employer paid Life/AD&D for employees, spouses, and children. Based on your age, you can elect this coverage in various increments and will pay contributions out of your paycheck based on your age and the amount elected. More information please review the Benefit Summary below.



Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


When you are first eligible (at hire) for Voluntary Life and AD&D, you may purchase up to the Guaranteed Issue (GI) for yourself and your spouse without providing proof of good health (EOI). Late entrants (you did not elect at hire date) any amount elected over the GI, will require EOI. If you elect optional life coverage, and are required to complete an EOI, it is your responsibility to complete the EOI and send to the provider (address will be listed on your form). In addition, your spouse will need to provide EOI to be eligible for coverage amounts over GI, or if coverage is requested at a later date.


Short-Term Disability (STD) is available on a voluntary basis. STD offers coverage of 60% up to $1,000 per week based on your earnings. This benefit begins on the 8th day of an accident or illness.


Long-Term Disability (LTD) is also paid for by Suburban Sports. In the event of an accident if you are unable to work, this plan will pay 60% to $3,000 a month based on your earnings. LTD begins after a 90-day elimination period.
