Introduction

The annual open enrollment is your opportunity to learn about the 2024 plan year benefit offerings and choose the best options for you and your family. Please review our 2024 benefit information found below.

How To Enroll

Action Required
  • All employees must complete the election form and any applicable carrier application form(s) and return to Doug Caplan and Nancy Henry by Friday, December 15th.
  • If you plan to newly enroll any dependents, make sure to have their SSN and their DOB to complete their enrollment.
  • If you do not wish to enroll in the medical plan, please check the box indicating you would like to Waive coverage on the enrollment form.
  • No changes can be made to your elections outside of the Open Enrollment period unless you experience a Qualifying Life Event.
