All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Liquid Web. New Hire Benefits are effective the first of the month following date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
Should your employment end with Liquid Web, your benefits will be terminated as of the end of the month in which the termination occurs.
NEW HIRES! Action Required: You must log into Workforce Now and elect or waive benefits within 5 days of your onboarding day.
ENROLLMENT INSTRUCTIONS:
The only opportunity to modify your plan elections for 2023 is if you have a Qualifying Life Event. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes in ADP within 30 days of the event. You will need to provide supporting documentation to Human Resources to support the changes based on the Qualifying Life Event (marriage/birth certificates, divorce decree's, proof of loss of coverage, etc.). Exception: HSA contributions can be changed at any time for any reason by contacting Human Resources.

You contribute toward the cost of some of your benefits based on your annual salary. The employee contributions for Medical, Dental and Vision benefits are below. Payroll contributions shown below are based on a weekly basis and are paid on a pre-tax basis.
Those employees who elect one of the medical plans will also be automatically covered by the dental coverage as well. However, you may choose to elect stand-alone dental coverage (no medical coverage) of which the dental plan is free whether you are a single, two party, or family.
If you choose to waive medical, dental, and vision benefits, and are not covered under another employees Liquid Web plans, you will receive $100/month.

When you enroll in the PPO HSA 1500 Plan the Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
HSA IRS Maximums are as follows:
$3,850 for a single contract | $7,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+
If you are only covered by the HSA plan for part of the 2023 calendar year, your maximum allowable contribution is prorated based on your effective date of coverage on the HSA plan.

BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.

Eligible employees may enroll in the PPO Dental Plan with BCBSM. You have the option of receiving care from a Blue Dental PPO In-Network dentist or you may receive care from an Out-of-Network dentist. Visiting an in network provider will result in the maximum level of benefits and avoidance of balance billing.
If you elect one of the BCBSM medical plans, dental insurance will automatically be included at no additional cost. However, you may enroll in dental alone without electing a medical plan.

Liquid Web offers Vision coverage through EyeMed Vision Care to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider In-Network, however, Out-of-Network coverage is provided but may only be handled as reimbursements in some situations. Liquid Web provides you with financial protection options by providing Employer-paid Life, and Accidental Death & Dismemberment (AD&D) coverage.
LIFE/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Eligible employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
Newly Hired Employees (and your spouse/children) are eligible for Guarantee Issue coverage - No Medical Questions! These Guarantee Issue amounts also apply to a newly eligible spouse and/or children due to qualifying events such as marriage or birth. Guarantee Issue amounts for new employees (and their families) and for newly eligible spouses and children are as follows:

If the Voluntary Life is not elected when first eligible, you may be required to submit Evidence of Insurability (EOI) should you choose to elect at a later date.
Future Increase Option: During the annual open enrollment period, you may be able to increase your election up to 2 increments, not to exceed the Guarantee Issue amount, without proof of good heath as long as you weren’t previously declined or withdrawn.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
DISABILITY - Voluntary, Employee Paid
An illness or injury can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs, and a more serious condition can leave you out of work for years. Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on your health rather than worrying about keeping up with your bills.
Newly Hired Employees are eligible for Guarantee Issue STD & LTD coverage - No Medical Questions! If you choose to waive these benefits during your new hire enrollment and wish to elect in the future, you may be subject to Evidence of Insurability.
