WELCOME
Helping you and your family reach and maintain good health is very important to Liquid Web. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Please review the remainder of this page for more information regarding your 2023 benefit plans.
ELIGIBILITY
All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Liquid Web. New Hire Benefits are effective the first of the month following date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
- Medical, Dental & Vision: Until the last day of the year in which the child turns 26
- Voluntary Life: Until the date of their 26th birthday.
Should your employment end with Liquid Web, your benefits will be terminated as of the end of the month in which the termination occurs.
ENROLLMENT
NEW HIRES! Action Required: You must log into Workforce Now and elect or waive benefits within 5 days of your onboarding day.
ENROLLMENT INSTRUCTIONS:
- Go to www.workforcenow.adp.com
- Enter your User ID and password
- Follow instructions and enroll in your benefits
- Make sure to save your elections and print your confirmation statement.
The only opportunity to modify your plan elections for 2023 is if you have a Qualifying Life Event. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes in ADP within 30 days of the event. You will need to provide supporting documentation to Human Resources to support the changes based on the Qualifying Life Event (marriage/birth certificates, divorce decree's, proof of loss of coverage, etc.). Exception: HSA contributions can be changed at any time for any reason by contacting Human Resources.

EMPLOYEE CONTRIBUTIONS
You contribute toward the cost of some of your benefits based on your annual salary. The employee contributions for Medical, Dental and Vision benefits are below. Payroll contributions shown below are based on a weekly basis and are paid on a pre-tax basis.
Those employees who elect one of the medical plans will also be automatically covered by the dental coverage as well. However, you may choose to elect stand-alone dental coverage (no medical coverage) of which the dental plan is free whether you are a single, two party, or family.
If you choose to waive medical, dental, and vision benefits, and are not covered under another employees Liquid Web plans, you will receive $100/month.

BCBSM is the Medical carrier and CVS/Caremark is the Prescription Drug carrier. If you choose to elect one of the medical plans, dental insurance will be automatically included at no additional cost!
Highlights of your medical plans are as follows:
- The Simply Blue PPO Plan includes copays for your routine standard services, including prescriptions. The deductibles and out-of-pocket maximums on this plan are embedded, view the video below for more information.
- The PPO HSA Plan requires that all services, including prescriptions, are subject to deductible before any insurance applies. The deductibles and out-of-pocket maximums on this plan are aggregate, view the video below for more information.

HEALTH SAVINGS ACCOUNT (HSA)

When you enroll in the PPO HSA 1500 Plan the Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- Covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You have not received VA benefits in the past 90 days
HSA IRS Maximums are as follows:
$3,850 for a single contract | $7,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+
If you are only covered by the HSA plan for part of the 2023 calendar year, your maximum allowable contribution is prorated based on your effective date of coverage on the HSA plan.

Liquid Web provides those employees who are covered by one of the BCBSM medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provide members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.
HOW MUCH DOES IT COST?
Medical Visits
- Standard, Non-HSA Plans: Equal to your office visit copay
- HSA Plans: $59 Copay; 20% after satisfying deductible
Mental Health
- Standard, Non-HSA Plans: Subject to normal cost sharing as dictated by the plan for mental health. If subject to deductible, see HSA section below for service costs.
- HSA Plans:
- Therapy Visits: $85 (Masters Level) or $99 (Doctorates Level) – Once your deductible is satisfied, coinsurance will apply if applicable
- Psychiatry Visits: $199 for initial visit and $95 for follow up visits –Once your deductible is satisfied, coinsurance will apply if applicable Licensed
Prescription copays/cost sharing will apply if medication is prescribed.

Services for Mental Health:
- Therapy Visits: Therapists such as psychologists, licensed clinical social workers, marriage and family therapists and professional counselors use talk therapy. Therapy is available to adults and children aged 10 and older by appointment from 7 a.m. to 11 p.m. Visits typically last 45 minutes.
- Psychiatry Visits: Psychiatrists can make diagnoses and prescribe and manage medications. Psychiatry is available to adults aged 18 and over and visits are by appointment only. Extended hours during evenings and on weekends may be available. The initial visit usually lasts 45 minutes with 15-minute follow-up visits
DENTAL

Eligible employees may enroll in the PPO Dental Plan with BCBSM. You have the option of receiving care from a Blue Dental PPO In-Network dentist or you may receive care from an Out-of-Network dentist. Visiting an in network provider will result in the maximum level of benefits and avoidance of balance billing.
If you elect one of the BCBSM medical plans, dental insurance will automatically be included at no additional cost. However, you may enroll in dental alone without electing a medical plan.

Liquid Web offers Vision coverage through EyeMed Vision Care to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider In-Network, however, Out-of-Network coverage is provided but may only be handled as reimbursements in some situations. Review your coverage details in your Benefit Guide.

FLEXIBLE SPENDING ACCOUNT (FSA)

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
You may contribute as follows:
Health Care FSA
- Up to $3,050 annually
- $610 carry over maximum
- All benefit eligible can participate
- Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
- If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only for dental and vision expenses
Dependent Care FSA
- Up to $5,000 annually
- Limited to $2,500 if you are married and file separate tax returns

Liquid Web provides you with financial protection options by providing Employer-paid Life, and Accidental Death & Dismemberment (AD&D) coverage.
LIFE/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Eligible employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
- Basic Life/AD&D Benefit - Employer Paid: $50,000
- Voluntary Life/AD&D - Employee Paid
- Employee: Choose in $10,000 increments up to the lesser of 3x your annual salary or $500,000
- Spouse: Choose in $5,000 increments up to the lesser of 50% of the amount you elect for yourself or $250,000
- Child: $10,000
Newly Hired Employees (and your spouse/children) are eligible for Guarantee Issue coverage - No Medical Questions! These Guarantee Issue amounts also apply to a newly eligible spouse and/or children due to qualifying events such as marriage or birth. Guarantee Issue amounts for new employees (and their families) and for newly eligible spouses and children are as follows:
Employee $100,000 | Spouse $50,000 | Child $10,000.
If the Voluntary Life is not elected when first eligible, you may be required to submit Evidence of Insurability (EOI) should you choose to elect at a later date.
Future Increase Option: During the annual open enrollment period, you may be able to increase your election up to 2 increments, not to exceed the Guarantee Issue amount, without proof of good heath as long as you weren’t previously declined or withdrawn.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
DISABILITY - Voluntary, Employee Paid
An illness or injury can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs, and a more serious condition can leave you out of work for years. Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on your health rather than worrying about keeping up with your bills.
Newly Hired Employees are eligible for Guarantee Issue STD & LTD coverage - No Medical Questions! If you choose to waive these benefits during your new hire enrollment and wish to elect in the future, you may be subject to Evidence of Insurability.
