All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Liquid Web. New Hire Benefits are effective the first of the month following date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
Should your employment end with Liquid Web, your benefits will be terminated as of the end of the month in which the termination occurs.
NEW HIRES! Action Required: You must log into Workforce Now and elect or waive benefits within 5 days of your onboarding day.
ENROLLMENT INSTRUCTIONS:
The only opportunity to modify your plan elections for 2023 is if you have a Qualifying Life Event. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes in ADP within 30 days of the event. You will need to provide supporting documentation to Human Resources to support the changes based on the Qualifying Life Event (marriage/birth certificates, divorce decree's, proof of loss of coverage, etc.). Exception: HSA contributions can be changed at any time for any reason by contacting Human Resources.

You contribute toward the cost of some of your benefits based on your annual salary. The employee contributions for Medical, Dental and Vision benefits are below. Payroll contributions shown below are based on a weekly basis and are paid on a pre-tax basis.
Those employees who elect one of the medical plans will also be automatically covered by the dental coverage as well. However, you may choose to elect stand-alone dental coverage (no medical coverage) of which the dental plan is free whether you are a single, two party, or family.
If you choose to waive medical, dental, and vision benefits, and are not covered under another employees Liquid Web plans, you will receive $100/month.
