We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Garber remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Below are highlights of the of your 2023 Benefit Plans:
- A variety of PPO Medical/Rx plans to choose from with Blue Cross Blue Shield of MI (BCBSM)
- Free virtual visits with Teladoc for those covered by the medical insurance
- Two dental plans to choose from with BCBSM

ELIGIBILITY
All regular employees working at least 30 hours per week are eligible to participate in the benefit plans. New Hire Benefits are effective the first of the month following 60 days of employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse*, domestic partner*, and children. Children eligibility is as follows -
- Medical | Dental | Vision | Voluntary Whole Life: Age 26
- Voluntary Life: Age 19 or 26 if full time student
- Accident | Critical Illness: Age 25
*Medical Only: If your spouse or domestic partner is currently employed and has access to his/her own employers sponsored medical plan, he/she must enroll in that plan as primary in order to be eligible for the Garber Management Group medical plan. By electing coverage for your spouse or domestic partner you are confirming that you understand any misrepresentation or omission of facts may be cause for immediate dismissal. Please note, if employee & spouse work for Garber, they can remain on the same plan.
Note: All contributions associated with a domestic partner are not available on a pre-tax basis.
ENROLLMENT
NEW HIRES! Action Required: You must log into Garber Connection and elect or waive benefits within 14 days of hire.
ENROLLMENT INSTRUCTIONS:
- Go to www.garberconnection.com
- Enter your User ID: First Initial + Last Name + Birth Month + Birth Day - e.g.John Smith, July 30 = JSmith0730
- Enter your Password: Last 4 of your SSN
- Follow instructions and enroll in your benefits
- Make sure to save your elections and print your confirmation statement.
The only opportunity to modify your plan elections mid-year is if you have a Qualifying Life Event. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes in Garber Connection within 30 days of the event. You will need to provide supporting documentation to Human Resources to support the changes based on the Qualifying Life Event (marriage/birth certificates, divorce decree's, proof of loss of coverage, etc.). Exception: HSA contributions can be changed at any time for any reason by contacting Human Resources.

MEDICAL/RX

Blue Cross Blue Shield of MI (BCBSM) will continue to be the administrator for you medical and prescription coverage. All the medical plans offered are Preferred Provider Organizations (PPO); this means that both in and out of network coverage is provided, you are not required to select a primary care physician, and you are not required to obtain a referral to see a specialist.
The plans that will be offered effective January 1, 2023 are as follows:
- PPO SB 1000
- HDHP 2000 HSA - increase to the out-of-pocket maximum
- PPO SB 2500 - No prescription coverage included
- SB HSA 6350 MVP
PPO | In-Network & Out-of-Network Benefits Available: The PPO option offers the freedom to see any provider when you need care. When you use providers from within the PPO network, you receive benefits at the discounted network cost. Most expenses, such as office visits, emergency room and prescription drugs are covered by a copay. Other expenses are subject to a deductible and coinsurance.
PPO HSA | In-Network & Out-of-Network Benefits Available: The HDHP is similar to the PPO Plan in that you have the option to choose any provider when you need care. However, in exchange for a lower per-paycheck cost, you must satisfy a higher deductible that applies to almost all health care expenses, including those for prescription drugs. All expenses are your responsibility until the deductible is reached, with the exception of preventive care, which is covered at 100% when you visit a physician in the network. Once the deductible is met, you are responsible for coinsurance for medical expenses and a copay for prescription drug expenses. Enrolling in this plan allows you to contribute tax free dollars to a health savings account (HSA). Any dollars that you (and your employer) wish to contribute can be used towards any eligible medical, Rx, dental and vision expenses that you may incur while covered under the plan. HEALTH SAVINGS ACCOUNT (HSA)

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- Covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You have not received VA benefits in the past 90 days
HSA IRS Maximums are as follows (any employer provided HSA funds will apply to the maximum noted below):
$3,850 for a single contract | $7,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+
If you are only covered by an HSA plan for part of the 2023 calendar year, your maximum allowable contribution is prorated based on your effective date of coverage on an HSA qualified plan.

Garber provides those employees who are covered by one of the BCBSM medical plans access to virtual visits with Teladoc. Teladoc provides members the opportunity to receive medical and mental health care from the comfort of your own home. With Teladoc, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.
Common Medical Services Include: Bladder Infections; Cold/Flu Symptoms; Sore Throat; Pink Eye; Rashes; Sinus Infections; Ear Infections; Stomach Issues

How Much Does it Cost?
- Medical Services: Free!
- Mental Health: Licensed Therapist: $90 per consult | Psychiatrist: $220 first consult; $100 for ongoing visits
- Prescriptions: If prescribed a medication during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in (no coverage included for the PPO 2500 Medical plan)

DENTAL

Garber's dental plans provide comprehensive coverage to help you and your family maintain good dental health. Eligible employees may pick between two dental plans.
Plan 1 is the lower cost dental plan; it provides lesser benefits for in and out of network providers. This plan also does not include orthodontia coverage.
Plan 2 providers richer dental benefits, including orthodontia, and provides both in and out of network.

VISION

Garber offers vision coverage through BCBSM to help pay for eye exams, prescription glasses and contact lenses. BCBSM partners with VSP to utilize their network of vision providers, your VSP vision network is VSP Choice. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations.

FLEXIBLE SPENDING ACCOUNT (FSA)

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.
Contributions
The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.
You may contribute as follows:
Health Care FSA
- Up to $2,850 annually
- If you are enrolled in an HSA plan, you will be eligible for the Limited Purpose FSA only
- $570 carry over applies for unused funds, any leftover funds in excess of $570 will be forfeited
- Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses
Dependent Care FSA
- Up to $5,000 annually
- Limited to $2,500 if you are married and file separate tax returns
- You have until March 15th of the following year to incur and submit for expenses, after March 15th, any unused funds will be forfeited
LIFE/AD&D

Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
- Basic Life/AD&D Benefit - Employer Paid: $20,000
- Voluntary Life/AD&D - Employee Paid
- Employee: Choose in $10,000 increments up to the lesser of 5x your annual salary or $180,000; Guarantee Issue: $180,000
- Spouse: Choose in $5,000 increments up to the lesser of 100% of the amount you elect for yourself or $180,000; Guarantee Issue: $25,000
- Child: Choose in $2,000 increments up to $10,000; Live birth to age 6 months, $1,000 flat benefit; One policy premium will cover all dependent children; Guarantee Issue: $10,000
- Voluntary Whole Life - Employee Paid
- Employee: $10,000; $20,000; $30,000; $40,000
- Spouse: $10,000; $20,000
- Child: $5,000 up to $50,000 depending on issue age & premium selected (can cover child or grandchild)
- Child Term Rider: $1,000 to $10,000 in $1,000 increments; Must be added to employee coverage
Vol Life/AD&D Guarantee Issue: Newly Hired Employees (and your spouse/children) are eligible for Guarantee Issue coverage - No Medical Questions! These Guarantee Issue amounts also apply to a newly eligible spouse and/or children due to qualifying events such as marriage or birth. If the Voluntary Life is not elected when first eligible, you may be required to submit Evidence of Insurability (EOI) should you choose to elect at a later date.
Open Enrollment Increase Option: During the annual open enrollment period, you will be able to increase your election up to the Guarantee Issue without proof of good heath as long as you weren’t previously declined or withdrawn.
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.

Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
Voluntary Short-Term Disability
- Benefits begin on the 8th day for an accident or the 15th day for an illness
- Benefits are payable for up to 12 weeks
- Your benefit amount is 60% of your weekly earnings to a maximum of $1,000
Voluntary Long-Term Disability
- Benefits begin on the 91st day of a qualifying accident or illness
- Benefits are payable until your retirement age; if you are still working beyond your retirement age and suffer from a qualifying accident or illness, a benefit will be payable for a specified amount of time depending on your age
- Your benefit amount is 50% of your monthly earnings to a maximum of $10,000
If you choose to waive the Voluntary Disability plans during your new hire enrollment, you will be required to submit Evidence of Insurability should you choose to enroll at a later date.
