WELCOME
Helping you and your family reach and maintain good health is very important to ICL. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. We remain committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees.
Please review the information below, including the member resources available with your plans and the educational videos for more information.
ELIGIBILITY
All regular employees working at least 30 hours per week are eligible to participate in the benefit plans effective your date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
- Medical | Dental | Vision: Last day of the calendar year the child turns age 26
- Voluntary Life: Age 19 or 26 if full time student
ENROLLMENT
NEW HIRES! Action Required: You must log into ADP and elect or waive benefits within 14 days of your hire date.
ENROLLMENT INSTRUCTIONS:
- Login your ADP Workforce Now portal
- You must make an active election, even if you are waiving coverage or not making any changes.
- Follow instructions and enroll in your benefits.
- Make sure to save your elections and print your confirmation statement.
The only opportunity to modify your plan elections for 2023 is if you have a Qualifying Life Event. These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a Qualifying Life Event, you may make changes to your benefits by contacting Human Resources and completing any election changes in ADP within 30 days of the event (60 days if due to adding a new child from birth or adoption). You will need to provide supporting documentation to support the changes based on the Qualifying Life Event. Exception: HSA contributions can be changed at any time for any reason.

Blue Care Network (BCN) will continue to be the administrator for you medical and prescription coverage. You may choose between two plans, the POS 1000 Plan or the POS HSA 2000 plan. Both plans offered are Point-of-Service (POS) plans and they function a bit differently depending on whether you reside in Michigan or outside of Michigan.
Michigan Residents:
- You must select a Primary Care Physician (PCP)
- No need to obtain a referral to see a specialist
- Both in and out of network coverage provided
- In network providers include BCN HMO providers
- When traveling outside of MI, you may visit a Blue Card Traditional provider to receive in network benefits
Non-Michigan Residents:
- No need to select a Primary Care Physician (PCP)
- No need to obtain a referral to see a specialist
- Both in and out of network coverage provided
- In network providers include BlueCard Traditional providers
With both plans, no matter your residency, the following applies -
- True Emergencies, whether in network or out of network are always treated as in-network
- Preventive Care is covered at 100% when received in-network

HEALTH SAVINGS ACCOUNT (HSA)

The Health Savings Account (HSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
To be eligible to contribute towards an HSA tax-free, you must meet the requirements below:
- Covered by a Qualified High Deductible Health Plan
- You cannot be covered by another health plan (including if your spouse has a Flexible Spending Account)
- You cannot be covered by Medicare or Medicaid
- You cannot be eligible to be claimed as a tax dependent
- You have not received VA benefits in the past 90 days
Important Reminder: The Eligibility rules for an HSA only apply for putting money in tax-free. If you are no longer eligible to put money in tax-free, you can still continue to use your HSA funds on qualified medical expenses tax free.
HSA IRS Maximums for 2023 are as follows:
- $3,850 for a single contract | $7,750 for a two person or family contract
- $1,000 "Catch Up" contribution for those age 55+
- If you are only covered by the POS HSA plan for part of the 2023 calendar year, your maximum allowable contribution is prorated based on the date you are covered by the POS HSA plan.
Reminder: You can make changes to your HSA elections at any time for any reason. Please contact Human Resources to request a change to your HSA.
VIRTUAL VISITS

ICL provides those employees and dependents who are covered by one of the BCN medical plans access to virtual visits with BCBSM Online Visits. This service provides members the opportunity to receive medical and mental health care from the comfort of your own home. Eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.
Common Medical Services Include:
Bladder Infections | Cold/Flu Symptoms | Sore Throat | Pink Eye | Rashes | Sinus Infections | Ear Infections
Plus more of your routine 'simpler' conditions!
Services for Mental Health:
Therapy Visits: Therapists such as psychologists, licensed clinical social workers, marriage and family therapists and professional counselors use talk therapy. Therapy is available to adults and children aged 10 and older by appointment from 7 a.m. to 11 p.m. Visits typically last 45 minutes.
Psychiatry Visits: Psychiatrists can make diagnoses and prescribe and manage medications. Psychiatry is available to adults aged 18 and over and visits are by appointment only. Extended hours during evenings and on weekends may be available. The initial visit usually lasts 45 minutes with 15-minute follow-up visits
How Much Does it Cost?
- Medical Services: POS $1000 - Equal to your Office Visit Copay / POS HSA - $59, upon satisfaction of your deductible the BCBSM Online Visits will be covered 100%
- Mental Health: Your plans standard cost sharing applies, subject to deductible and coinsurance in most cases. The applicable fees, that will apply towards your deductible depending on service:
- Therapy Visits: $85 (Masters Level) or $99 (Doctorates Level)
- Psychiatry Visits: $199 for initial visit and $95 for follow up visits
- Prescriptions: If prescribed a medication during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

DENTAL

The dental plan provides comprehensive coverage to help you and your family maintain good dental health. Dental coverage is provided through Delta Dental of MI and provides both in and out of network coverage. In Network providers include Delta Dental PPO and Delta Dental Premier dentists. Out of network benefits are offered at the same levels, however, if you see an out of network dentist you may experience balance billing not covered by your dental plan.

ICL offers vision coverage through EyeMed to help pay for eye exams, prescription glasses and contact lenses. The plan includes both in and out of network coverage, however, you receive a higher level of benefits when you see a provider in network.
