We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. Belle Tire remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Highlights of the 2023 benefit plans include:
The Benefit choices you made during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 31 days of the event.

Blue Cross Blue Shield of MI (BCBSM) will be the administrator for your medical and prescription coverage. All the medical plans offered are Preferred Provider Organizations (PPO); this means that both in and out of network coverage is provided, you are not required to select a primary care physician, and you are not required to obtain a referral to see a specialist. Look up your prescriptions by viewing the BCBSM Drug List. Use the Control F feature to type in the name of your prescriptions and determine which level the drug is and whether a Prior Authorization (PA) or Quantity Limits (QL) apply. You can also determine whether there may be lower cost alternatives for your prescriptions.
Helpful Rx Cost Saving Tools & Tips!
MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
PillarRx - Our high-cost drug discount program helps you find and take advantage of manufacturer copayment assistance programs that significantly lower your out-of-pocket costs for these expensive medications. You may even pay nothing for your medicine. You’ll never pay more than your usual copayment. And the program is free!
How it works:
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
Belle Tire offers five medical coverage options with different benefit levels so that you may select the plan that best meets the needs of you and your family. These plans are provided through a Preferred Provider Organization (PPO), where a network of preselected hospitals and doctors are available for your use. If you use the network, you will receive the highest level of benefits offered by the plan. While the network is available, you are not required to use it. You always have the complete freedom to select any provider whenever you need care. However, the out-of-network benefits are lower and your out-of-pocket costs are higher.

Belle Tire provides those employees who are covered by one of the BCBSM medical plans access to virtual care with Teledoc Health (formerly known as BCBSM Online Visits). Teledoc Health provides members the opportunity to receive medical and mental health care from the comfort of your own home.
All eligible members will need to download the Teladoc Health app and create a new account with Teladoc Health. The Blue Cross Online Visits app will no longer be available as of 1/1/24.
HOW MUCH DOES IT COST?
Medical Visits:
Mental Health: For both non-HSA and HSA plans, the costs of these services are Subject to your plans normal cost sharing for mental health services, often, deductible & coinsurance. The charges applicable for the type of services, which will apply towards your deductible, are as follows:
Prescriptions: If prescribed during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in.

Please note, if you are currently receiving the wellness credit for the 2022 plan year, you will have until February 28, 2023 to get your physical with no lapse in your wellness credit. Should you miss the deadline, your wellness credit will discontinue until a new wellness form is submitted. Please note that if you take a leave of absence, you will not receive the wellness credit. As long as you return to work following an approved leave of absence, a retro wellness credit will be applied to a future payroll check, for any missed credits. If you do not return to work after a leave of absence, then any missed wellness credits will be forfeited.
To receive your Wellness Discount for 2023:
It is highly recommended that all employees receive a copy of their wellness paperwork, once their primary care physical fills out the paperwork.
Q: What is the difference between Wellness Rate and Non-Wellness Rate for Medical Plans?
A: In order to receive the Wellness Rate for your Medical Plan election you will need to get a Physical within 30 days of your coverage becoming active. The form that you will need to take with you to your appointment is located toward the back of the 2023 Benefits Guide. Human Resources will receive monthly files with the employee’s name and date of exam. The wellness rate will then be applied to your pay check. Please note that you will always see the non-wellness deduction on your pay check. You will receive an earning of the difference in the amount of non-wellness and wellness rate under the earnings codes Wellness 2023.
Example: If you elect the Medical 80/60 Plan for Employee + 1 your weekly deductions will be $148.15. If you complete your physical within 30 days of your coverage becoming active, you will receive the wellness rate. The wellness rate is $125.08. $148.15 - $125.08 = $23.07. You will receive a wellness earning each paycheck of $23.07.

Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in one of the Belle Tires Blue Cross Blue Shield (BCBSM) Blue Dental PPO Dental plans today and have not already done so, please be sure to register through the BCBSM Dental member portal where you can access important information pertaining to your dental plan with BCBSM Dental including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the BCBSM Dental portal as of your effective date of coverage.
Belle Tire offers two options to enroll in a Preferred Provider Organization (PPO) dental plan through Blue Cross Blue Shield of Michigan (BCBSM) utilizing the Blue Dental PPO network. You may utilize dentists who belong to the Blue Dental PPO network, as well as dentists who are not part of the Blue Dental PPO network. However, you save money when you visit in-network dentists. When visiting a non-BCBSM provider, you will be responsible for the higher amount.

Belle Tire understands the importance to you and your family to have strong financial protection. We provide all full-time employees Basic Life/AD&D coverage at no cost to you. Full-time Managers, Sales and Support Center positions are also provided with Disability coverage at no cost. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.

A sudden accident or death can leave you or your loved ones in a vulnerable position. Belle Tire provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage at no cost to you.
Employees have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.

Belle Tire offers both Voluntary Short & Long Term Disability benefits.

Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
Belle Tire provides a competitive benefits package for all employees as part of our total compensation package. We realize you make benefit choices based on what’s important to you and the needs of your changing lifestyle. Therefore, along with our core benefits, we offer these additional benefits which help to provide financial support to you. These benefits are paid for through payroll deduction on a post-tax basis so that the benefits are tax free.
Accident & Critical Illness is offered through Guardian. Hopefully you are never in the position to use it, but if you were to experience an accident, illness, or hospitalization, it could end up costing you a lot of money. With these plans, you will be paid money for qualifying situations/services to cover your of pocket expenses or you may use the funds for whatever you like.
Identity Protection and Legal Services are offered through ID Shield/Legal Shield. These benefits are offered in an effort to protect and assist with the expenses and services needed in the event you are faced with compromises to your identity or legal matters.

Belle Tire realizes that your needs go beyond just the standard insurance. This is why we also provide all employees and their families access to personal service and assistance resources at no cost. Here you will find information on our Employee Assistance Program and Travel Assistance resources, both to give you peace of mind and support when you need it the most!
Belle Tire is committed to ensuring our team is ready for retirement when that time comes. To help ensure this, employees will be automatically enrolled in a pre-tax 401K at 5% payroll deductions upon eligibility. Contributions to the plan will be automatically increased by 1.00% each year and will cap at 10.00%.
When can I start contributing to my 401K?
Employees who are at least 21 years of age are eligible to participate in the 401K plan on the first of the month following three full months (90 days) of employment.
How do I see my account or make changes?
If you would like to view more information regarding the Belle Tire 401K plan please visit www.netbenefits.com or call 800-924-4015 to speak to a representative at Fidelity Investments. You will be able to view plan documents and make any changes to your contributions, including opting to not participate in the plan, via the NetBenefits website shown above. Should you choose to make no changes you will be automatically enrolled in a pre-tax 401k upon your date of eligibility.
Did you know the more you contribute to your 401K the more Belle Tire will contribute?
The money you save today will grow in value as it accumulates compound interest. Compound interest is the addition of interest to the principal sum of a deposit (which means interest on interest). That is why the more time your money has to accumulate compound interest, the better!
Using an example of an employer match of $1,400, and an assumed interest rate of 7% your $1,400 could grow to:
Beneficiary Information
Please be sure to designate at least one person, trust, or estate as a beneficiary for your 401K Plan at www.netbenefits.com > “Accounts & Benefits” > “Update your beneficiaries”. If you wish to elect a beneficiary other than your legal spouse, your spouse must complete a Spousal Consent Form in the presence of a notary public and mail the form to Fidelity.
